Tritace is a medicine containing the active substance ramipril, which belongs to the group of ACE inhibitors (angiotensin-converting enzyme inhibitors). It is available as 2.5 mg, 5 mg and 10 mg tablets.
Ramipril reduces the production of substances in the body that can raise blood pressure, relaxes and widens blood vessels and makes it easier for the heart to pump blood around the body. This is why Tritace is used for high blood pressure and also to protect the heart, blood vessels and kidneys.
Important warning
- Tritace is a prescription-only medicine (regime R in Serbia); the dose is set and gradually adjusted by the doctor.
- The medicine must not be taken after week 12 of pregnancy and is not recommended in the first trimester either – if you are planning a pregnancy or become pregnant, contact your doctor straight away.
- Swelling of the face, lips, tongue or throat with difficulty breathing or swallowing (angioedema) requires stopping the medicine and urgent medical help.
- Do not combine the medicine on your own with potassium salts, potassium-containing salt substitutes or NSAID painkillers (ibuprofen, indometacin, aspirin).
- This page is for information only and is not an offer to buy the medicine online. Read the official package leaflet before use.

What is Tritace used for?
- treatment of high blood pressure (hypertension);
- reducing the risk of heart attack or stroke (cardiovascular prevention in people with established heart and vascular disease or with diabetes and at least one other risk factor);
- reducing the risk or delaying the worsening of kidney disease (whether or not you have diabetes);
- treatment of heart failure, when the heart cannot pump enough blood;
- treatment after a heart attack (myocardial infarction) complicated by heart failure.
How is it used and dosed?
Tritace is taken by mouth at the same time every day, with or without food. Swallow the tablet whole with enough liquid – do not crush or chew it. The doses in the leaflet are a guide; the doctor adjusts them until the condition is under control.
| High blood pressure | usual starting dose 1.25 mg or 2.5 mg once daily; maximum dose 10 mg once daily |
|---|---|
| Reducing the risk of heart attack or stroke | starting dose 2.5 mg once daily; usual dose 10 mg once daily |
| Kidney disease | starting dose 1.25 mg or 2.5 mg once daily; usual dose 5 mg or 10 mg once daily |
| Heart failure | starting dose 1.25 mg once daily; maximum 10 mg daily, preferably divided into two doses |
| After a heart attack | starting from 1.25 mg once daily to 2.5 mg twice daily; usual dose 10 mg daily divided into two doses |
If you already take diuretics (water tablets), the doctor may reduce their dose or stop them before starting treatment. A 1.25 mg dose is obtained with half of a 2.5 mg tablet.
Special patient groups
- Children and adolescents under 18: not recommended because there are no data on safety and efficacy.
- Elderly: the doctor starts with a lower dose and increases it gradually; in very old and frail patients a starting dose of 1.25 mg may be considered.
- Kidney impairment: the dose is adjusted to creatinine clearance – with clearance of 30–60 mL/min the maximum daily dose is 5 mg, and with 10–30 mL/min the starting dose is 1.25 mg and the maximum 5 mg daily (SmPC). On haemodialysis the medicine is taken a few hours after dialysis.
- Liver impairment: treatment is started under close medical supervision and the maximum daily dose is 2.5 mg (SmPC).
Contraindications – when the medicine must not be used
- allergy to ramipril, other ACE inhibitors or any of the excipients;
- a past serious allergic reaction called angioedema (swelling of the face, lips, tongue or throat, difficulty breathing and swallowing);
- current or recent treatment with sacubitril/valsartan (a medicine for chronic heart failure);
- dialysis or another type of blood filtration for which the medicine may not be suitable;
- narrowing of the kidney arteries (renal artery stenosis) with reduced blood flow to the kidneys;
- the second and third trimester of pregnancy;
- abnormally low blood pressure;
- diabetes or impaired kidney function together with a blood pressure medicine containing aliskiren.
Precautions and warnings
Talk to your doctor before starting treatment:
- if you have heart, liver or kidney problems;
- if you have lost a lot of salt or fluid (vomiting, diarrhoea, heavy sweating, low-salt diet, long-term diuretics, dialysis);
- if you are going to have desensitisation treatment for bee or wasp stings;
- before anaesthesia, surgery or dental treatment – you may need to stop the medicine one day before, as agreed with your doctor;
- if you have high potassium in your blood, or conditions or medicines that lower blood sodium;
- if you take mTOR inhibitors (temsirolimus, everolimus, sirolimus), vildagliptin or racecadotril – increased risk of angioedema;
- if you have collagen vascular disease (e.g. scleroderma, systemic lupus erythematosus);
- if you take sartans (e.g. valsartan, telmisartan, irbesartan) or aliskiren.
During treatment the doctor may regularly check your kidney function, blood pressure and blood electrolytes (e.g. potassium).
Interactions with other medicines, food and drink
Medicines that may reduce the effect of Tritace: NSAIDs (ibuprofen, indometacin, acetylsalicylic acid) and medicines such as ephedrine, noradrenaline or adrenaline.
Medicines that may increase the risk of side effects with Tritace:
- sacubitril/valsartan, NSAIDs, chemotherapy;

- sartans and aliskiren (the doctor may need to change the dose).
Tritace may increase the effect of diabetes medicines (tablets and insulin – blood sugar should be checked more often) and raise blood levels of lithium.
Alcohol with Tritace may cause dizziness and light-headedness and lower blood pressure further – ask your doctor. The medicine can be taken with or without food.
Pregnancy and breastfeeding
Tritace should not be taken during the first 12 weeks of pregnancy and must not be used after week 13, as it may seriously harm the unborn baby. If you become pregnant during treatment, tell your doctor straight away; if you are planning a pregnancy, you should switch to another treatment in advance.

When to seek medical help immediately?
- swelling of the face, lips or throat, difficulty swallowing or breathing, itching and rash (severe allergic reaction);
- severe skin reactions – rash, mouth ulcers, blisters, peeling skin;
- fast heartbeat, palpitations, chest pain or tightness, signs of heart attack or stroke;

Pharmacist's advice
Take Tritace at the same time every day. At the start of treatment and after each dose increase, get up slowly from lying or sitting, as dizziness is most common then. Measuring and recording your blood pressure at home helps the doctor adjust the dose correctly.
A dry, tickly cough is a common side effect of ACE inhibitors – if it persists, tell your doctor instead of stopping the medicine yourself. Do not take potassium supplements or potassium-containing salt substitutes without your doctor's agreement, and ask your pharmacist which painkiller is suitable with ramipril.

Tritace can be taken with or without food, at the same time every day. Swallow the tablet whole with enough liquid, without crushing or chewing.
What if I become pregnant while taking Tritace?
