Trental 400 mg is a medicine containing the active substance pentoxifylline. It belongs to a group of medicines known as peripheral vasodilators and is used in adults for the treatment of peripheral vascular disease, including intermittent claudication and rest pain caused by impaired circulation in the legs.
Pentoxifylline affects certain properties of the blood and helps improve blood flow through small blood vessels. The medicine must be used only according to the treatment plan prescribed by a doctor. Trental is not an ordinary circulation supplement and should not be started independently for cold feet, tingling or leg pain without identifying the cause.

Trental 400 mg – what is it used for and how should it be taken?
What is Trental used for?
The medicine is used for peripheral vascular disease, including:
- intermittent claudication – pain, cramping or discomfort in the legs during walking due to reduced blood flow;
- rest pain associated with impaired peripheral circulation;
- other manifestations of peripheral vascular disease when a doctor considers pentoxifylline appropriate.
Leg pain during walking may have several causes, including arterial, venous, neurological, spinal or joint disorders. Trental should not be started solely on the basis of symptoms without medical assessment.
How should Trental be taken?
The usual initial dose stated in the official patient leaflet is one 400 mg tablet three times daily. The doctor may reduce the dose to one tablet twice daily in some patients, particularly during maintenance treatment.
This information is not an instruction for self-dosing. The doctor selects the dose according to the symptoms, blood pressure, kidney and liver function, tolerability and other medicines being used.
- Swallow the tablet whole with plenty of water.
- Do not chew the tablet.
- Take it during or immediately after a meal.
- Do not increase the dose if improvement is not immediately noticeable.
- A lower dose may be required in severe kidney or liver impairment.
- Do not stop treatment unless instructed by the doctor.
If a dose is missed: do not take a double dose. Take the next dose according to the usual schedule unless the doctor has instructed otherwise.
Trental is not intended for the unsupervised treatment of every sensation described as “poor circulation”. Leg pain during walking may indicate significant narrowing of the arteries and increased cardiovascular risk.
Sudden severe leg pain, marked coldness, pallor or blue discolouration, loss of sensation or weakness require urgent medical assessment. Do not wait for Trental to take effect.
When should Trental not be taken?
Do not take the medicine:
- if you are allergic to pentoxifylline, theophylline, aminophylline, another methylxanthine or any ingredient;
- during or shortly after an acute myocardial infarction;
- if you have a severe abnormal heart rhythm;
- if you have had a haemorrhagic stroke;
- if you have severe retinal bleeding;
- during pregnancy.
Special warnings and precautions
Tell the doctor before treatment if you have:
- low blood pressure, dizziness or fainting when standing up;
- severe coronary artery disease;
- impaired kidney or liver function;
- an increased risk of bleeding;
- recent surgery or active bleeding;
- diabetes treated with insulin or tablets;
- an abnormal heart rhythm or chest pain.
Patients with low blood pressure or severe coronary artery disease may experience a further fall in blood pressure. Dose adjustment may be required in severe kidney or liver impairment.
Interactions with other medicines
Tell the doctor or pharmacist if you use:
- warfarin or another anticoagulant;
- acetylsalicylic acid, clopidogrel or another antiplatelet medicine;
- non-steroidal anti-inflammatory medicines, particularly ketorolac;
- blood-pressure-lowering medicines;
- insulin or oral diabetes medicines;
- ciprofloxacin;
- theophylline;
- cimetidine.
Combining Trental with medicines affecting blood clotting may increase the risk of bleeding. Additional anticoagulation monitoring may be required in patients treated with vitamin K antagonists.
Trental may enhance the effects of antihypertensive and antidiabetic treatment. More frequent blood-pressure or blood-glucose monitoring may therefore be needed.
Pregnancy and breastfeeding
Trental should not be used during pregnancy. Tell the doctor if you are pregnant, think you may be pregnant or are planning a pregnancy.
Small amounts of pentoxifylline pass into breast milk. Use during breastfeeding requires an individual assessment of the expected benefit and possible risk.
Driving and operating machinery
The medicine may cause dizziness, light-headedness or low blood pressure. Do not drive or operate machinery if these effects occur.
Possible side effects
Reported adverse effects include:
- nausea, vomiting or diarrhoea;
- abdominal discomfort or bloating;
- constipation;
- headache, dizziness or light-headedness;
- flushing;
- rash, itching or hives;
- agitation or sleep disturbance;
- low blood pressure;
- a rapid or irregular heartbeat;
- changes in liver-enzyme test results.
Changes in blood-cell counts, including reduced platelets, white blood cells or neutrophils, and cases of bleeding involving the skin, mucous membranes, gastrointestinal or urinary system have also been reported.
Stop taking the medicine and seek urgent medical help if any of the following occurs:
- difficulty breathing, bronchospasm or difficulty swallowing;
- swelling of the face, lips, tongue or throat;
- vomiting blood, black stools or another serious bleeding event;
- unusual bruising or bleeding that is difficult to stop;
- a strong or irregular heartbeat, chest pain or loss of consciousness;
- high temperature, severe chills, sore throat or mouth ulcers;
- headache, stiff neck and marked sensitivity to light;
- yellowing of the skin or eyes and dark urine;
- sudden severe pain, coldness, pallor, numbness or weakness of an arm or leg.
If more Trental has been taken than prescribed
An overdose may cause low blood pressure, drowsiness, seizures and an abnormal heart rhythm. Contact a doctor or emergency department immediately and take the medicine pack with you.
How to store Trental
- Keep out of the sight and reach of children.
- The medicine does not require special storage conditions.
- Do not use tablets that are crushed, damaged or discoloured.
- Do not use after the expiry date.
- Return unused medicine to a pharmacy that collects pharmaceutical waste.
Composition
Each prolonged-release tablet contains 400 mg of pentoxifylline.
The other ingredients include hydroxyethylcellulose, povidone K25, talc, magnesium stearate, hypromellose, titanium dioxide, erythrosine and macrogol 8000.
Take Trental during or immediately after a meal and swallow the tablet whole with plenty of water. Do not independently increase the dose if walking pain does not improve immediately.
Report anticoagulants, aspirin, clopidogrel, pain medicines, antihypertensive treatment and diabetes medicines. Blood in the stool, black stools, vomiting blood or unusual bruising require urgent assessment.
A structured walking programme may form part of the management of peripheral arterial disease, but its intensity and safety should be agreed with a doctor, particularly if rest pain or foot wounds are present.
