How is Sinacilin used and dosed?
Take the medicine exactly as prescribed by your doctor. Spread the doses evenly over the day; the interval between two doses should not be shorter than 4 hours.
| Capsules – method of use | swallow the capsule whole (without opening it) with enough liquid |
|---|---|
| Capsules – adults, elderly and children > 40 kg | 250–500 mg three times a day or 750 mg–1 g every 12 hours; severe infections 750 mg–1 g three times a day; urinary tract infections 3 g twice a day; Lyme disease 4 g a day (early stage), up to 6 g a day (late stage); stomach and duodenal ulcer 750 mg–1 g twice a day for 7 days with another antibiotic and a proton pump inhibitor; maximum 6 g a day |
| Capsules – children < 40 kg who can swallow capsules | 40–90 mg/kg body weight a day, divided into two or three doses; maximum 100 mg/kg a day – the dose is set by the doctor |
| Suspension – preparation | just before use add 66 mL of purified water in two portions, shaking well each time; shake the bottle before each use; measure with the 5 mL spoon provided (marks at 1.25 mL and 2.5 mL) |
| Suspension – children < 40 kg | 40–90 mg/kg a day, divided into two or three doses (not more than 3 g a day); maximum 100 mg/kg a day; taken at the start of a meal or just before a meal |
| Suspension – adults and children > 40 kg | the suspension is not usually prescribed for this group; usually 250–500 mg every 8 hours or 750 mg–1 g every 12 hours, in more severe infections 750 mg–1 g every 8 hours (maximum 6 g a day) |
- Treatment should be continued for 2–3 days after symptoms have disappeared, so that the bacteria are completely eliminated (capsule leaflet).
- If kidney function is reduced, the doctor prescribes lower doses.
Special patient groups
- Children: the dose is calculated by body weight; the suspension is intended mainly for children, and capsules only if the child can swallow them.
- Elderly patients: the adult dose is used; serious liver function disorders occur mostly with long-term treatment and in older people.
- Impaired kidney function: the dose is adjusted to the degree of impairment.
- Newborns: the suspension contains sodium benzoate (E211), which may increase the risk of jaundice in newborns (up to 4 weeks old).
Contraindications – when the medicine must not be used
- allergy (hypersensitivity) to amoxicillin, penicillin or any of the excipients;
- an allergic reaction to any antibiotic in the past (e.g. rash or swelling of the face or throat).
Precautions and warnings
- impaired kidney function; difficult or irregular urination;
- high doses or risk factors for seizures (previous convulsions, treated epilepsy, meningeal disorders);
- infectious mononucleosis – the medicine may cause a rash;
- redness with blisters and fever at the start of treatment (possible acute generalised exanthematous pustulosis – AGEP) – stop the medicine immediately;
- diarrhoea during or after use – stop the medicine and contact your doctor;
- in Lyme disease a Jarisch-Herxheimer reaction may occur (fever, chills, headache, muscle pain, rash);
- during longer treatment the doctor periodically checks liver and kidney function and blood counts; candidiasis may occur;
- the medicine may affect the results of urine glucose tests, liver function tests and the oestriol test;
- the suspension contains sucrose (2190.85 mg per 5 mL) – important for people with diabetes or sugar intolerance.
Interactions with other medicines, food and drink
- allopurinol (gout) – possible allergic skin reaction;
- probenecid (gout) – the doctor may adjust the dose;
- anticoagulants (e.g. warfarin) – additional blood tests may be needed;
- other antibiotics, e.g. tetracyclines – Sinacilin may be less effective;
- methotrexate – side effects may be more pronounced.
Pregnancy and breastfeeding
- If you are pregnant, breastfeeding or planning a pregnancy, ask your doctor for advice – the doctor assesses whether the benefit of treatment outweighs the potential risk.
- Amoxicillin passes into breast milk.
Driving and using machines
- The medicine may cause side effects (allergic reactions, dizziness, convulsions) that can reduce your ability to drive. Do not drive or use machines unless you feel well.
Possible side effects
- Stop the medicine and contact a doctor immediately (very rare): allergic reactions (itching, rash, swelling of the face, lips, tongue, difficulty breathing); inflammation of blood vessels with rash and bruising; delayed allergic reaction 7–12 days after use; erythema multiforme and other severe skin reactions; DRESS (flu-like symptoms with rash and swollen lymph nodes); colitis with diarrhoea, sometimes bloody; serious liver function disorders; signs of blood disorders (fever, chills, sore throat, bruising).
- Common: skin rash, nausea, diarrhoea.
- Uncommon: vomiting; milder itchy rash and hive-like rash.
- Very rare: candidiasis (vagina, mouth, skin), kidney function disorder, convulsions, dizziness, hyperactivity, crystals in the urine, tongue discolouration, anaemia due to breakdown of red blood cells, low white blood cell and platelet counts, prolonged clotting time; with the suspension also tooth discolouration in children, which usually disappears after brushing.
- Serious symptoms may occur during treatment and up to several weeks after stopping the medicine.
Overdose, missed dose and stopping treatment
- Overdose: nausea, vomiting, diarrhoea or crystals in the urine (difficult urination, cloudy urine) may occur – contact your doctor or pharmacist as soon as possible and take the pack with you.
- Missed dose: take it as soon as you remember; if the next dose is due in less than 4 hours, skip the missed dose. Do not take a double dose.
- Stopping treatment: take the medicine for as long as your doctor recommended, even if you feel better – otherwise the infection may return.
When to seek medical help immediately?
- swelling of the face, lips, tongue or throat, difficulty breathing or a severe rash all over the body;
- skin redness with blisters, peeling or pus-filled spots, with fever;
- diarrhoea with blood in the stool, severe abdominal pain and fever;
- dark urine, pale stools, yellowing of the skin or whites of the eyes.
Pharmacist's advice
Take the antibiotic at even intervals – for example, with three-times-daily dosing about every 8 hours – and complete the whole prescribed course. Stopping too early increases the risk of the infection returning and of bacterial resistance.
Keep the prepared suspension below 25 °C for no longer than 7 days and shake it well before each use; measure the dose with the spoon provided, not a household spoon. If diarrhoea occurs, contact your doctor – many patients use probiotics during and after antibiotics, but as a supplement, not instead of medical advice. Drink enough fluids during treatment.
