Leponex contains clozapine, a medicine from the group of antipsychotics used to treat mental illnesses such as psychosis. It is intended for patients with schizophrenia in whom treatment with at least two other antipsychotics has not worked or has caused severe side effects, and for people with Parkinson's disease who have severe disturbances of thinking, emotions and behaviour.

The medicine is available as 25 mg and 100 mg tablets; both strengths are scored and can be divided into equal doses. Because of the risk of agranulocytosis, treatment requires regular blood count checks.

Important warning

  • Dispensing status: prescription-only medicine (R). The pack carries a triangle symbol – driving is not permitted during treatment.
  • Agranulocytosis: the medicine can cause a serious fall in white blood cells. The blood count is checked weekly for the first 18 weeks, then at least monthly and for another 4 weeks after stopping. With fever, flu, sore throat or mouth sores, contact your doctor immediately – before the next tablet.
  • Myocarditis and cardiomyopathy: the risk is highest in the first two months. A fast or irregular heartbeat at rest, chest pain, shortness of breath or unexplained fatigue require urgent assessment.
  • Severe constipation can be dangerous – it must be treated in time. Alcohol is not allowed.
  • This page is for information only and is not an offer to buy the medicine online.

What is Leponex used for?

  • treatment-resistant schizophrenia – when at least two previous medicines for schizophrenia did not work or caused severe side effects that could not be treated;
  • severe disturbances of thinking, emotions and behaviour in people with Parkinson's disease in whom other treatments have not been successful.

How is Leponex taken and dosed?

The doctor increases the dose gradually to reduce the risk of low blood pressure, seizures and drowsiness. Tablets are swallowed with enough water. Usual dosing (PIL):

IndicationStart of treatmentUsual dose
Treatment-resistant schizophrenia12.5 mg (half a 25 mg tablet) once or twice on day 1, 25 mg once or twice on day 2; then increases of 25–50 mg to 300 mg daily over 2–3 weeksusually 200–450 mg daily in divided doses; maximum 900 mg daily; a 200 mg dose can be taken once in the evening; treatment lasts at least 6 months
Thought disorders in Parkinson's disease12.5 mg in the evening; increases of 12.5 mg no more than twice a week, up to 50 mg by the end of week twousually 25–37.5 mg once daily in the evening; maximum 100 mg daily
  • At doses above 450 mg daily the risk of side effects, especially seizures, increases – the lowest effective dose is always used.
  • If you have not taken the medicine for more than 48 hours, do not restart on your own – contact your doctor; treatment is restarted at 12.5 mg.

Special patient groups

  • Children and adolescents under 16: the medicine is not intended for this group (insufficient data).
  • People aged 60 and over: treatment is started at a lower dose and increased very slowly; fainting on changing position, dizziness, fast heartbeat, difficulty urinating and constipation are more common. In elderly people with dementia, antipsychotics are associated with a slightly increased mortality.
  • Liver disease: the medicine must not be taken with severe liver disease or acute jaundice; with milder disorders – regular liver function tests.
  • Kidney and heart disease: the medicine is contraindicated in severe disease.

Contraindications – when the medicine must not be used

  • inability to have regular blood count checks;

Pharmacist's advice

Write the dates of your blood count checks in a calendar and do not skip them – only regular tests allow treatment to continue safely. With any fever or sore throat, first measure your temperature and contact your doctor before the next tablet.

Constipation is common and can be dangerous: drink enough fluids, eat fibre and contact your doctor if you have not had a bowel movement for several days. Tell your doctor if you change your smoking or coffee habits, as this changes the level of the medicine in the blood.
