Ceftriaxon-MIP is a third-generation cephalosporin antibiotic containing the active substance ceftriaxone (as ceftriaxone sodium). It destroys bacteria that cause infections and is given to adults and children, including newborns.

The medicine is a powder for solution for injection/infusion in strengths of 1 g and 2 g ceftriaxone per vial. It is used in a healthcare facility – it is prepared and given by a doctor or nurse as an infusion or as an injection into a vein or muscle.

Important warning

  • The medicine is used only in a healthcare facility, under medical supervision.
  • It must not be given in case of allergy to ceftriaxone or other cephalosporins, or after a sudden or severe allergic reaction to penicillin or similar antibiotics.
  • Ceftriaxon-MIP must not be mixed or given at the same time as injections or infusions containing calcium; it must not be given to newborns who need intravenous calcium.
  • Report diarrhoea with blood and mucus, severe abdominal pain or reduced urination during treatment to your doctor immediately.

What is Ceftriaxon-MIP used for?

Ceftriaxon-MIP is used to treat infections of the:

  • brain (bacterial meningitis)
  • lungs (community- and hospital-acquired pneumonia)
  • middle ear (acute otitis media)
  • abdomen and abdominal lining (peritonitis)
  • urinary system and kidneys (complicated infections, including pyelonephritis)
  • bones and joints
  • skin and soft tissues (complicated infections)
  • blood and heart (bacteraemia, bacterial endocarditis)

It can also be used to treat gonorrhoea and syphilis, patients with a low white blood cell count (neutropenia) and fever caused by a bacterial infection, lung infections in adults with chronic bronchitis (acute exacerbation of COPD), Lyme disease (in adults and children, including newborns older than 15 days) and to prevent infections during surgery.

How is it given and dosed?

The dose depends on the severity and type of infection, other antibiotics, body weight and age, and on the condition of the liver and kidneys; the length of treatment depends on the type of infection.

Patient groupUsual doseNote
Adults, elderly and children from 12 years (≥ 50 kg)1–2 g once dailyup to 4 g daily for more severe infection; a dose above 2 g may be given once daily or in two doses
Newborns, infants and children from 15 days to 12 years (< 50 kg)50–80 mg/kg once dailyup to 100 mg/kg for serious infection, maximum 4 g daily; children of 50 kg or more receive the adult dose
Newborns (0–14 days)20–50 mg/kg once dailymust not exceed 50 mg/kg daily
Gonorrhoea (adults)500 mgsingle dose, into a muscle
Pre-operative prophylaxis (adults)2 gsingle dose, 30–90 minutes before surgery
Disseminated Lyme disease (adults)2 g once dailyfor 14–21 days

The medicine is given as an intravenous infusion (preferred, over more than 30 minutes), a slow injection into a vein (over more than 5 minutes) or a deep injection into a muscle, dissolved in lidocaine – such a solution is never given into a vein. No more than 1 g is given at one intramuscular site, and doses above 2 g are given intravenously. In newborns intravenous doses are given over more than 60 minutes. This is general dosing from the leaflet – the doctor determines the dose for each patient.

Special patient groups

  • Impaired kidney function: if the liver works normally, no dose reduction is needed; with creatinine clearance below 10 mL/min 2 g daily must not be exceeded. Ceftriaxone is not removed by dialysis.
  • Impaired liver function: in mild or moderate impairment with normal kidney function no dose adjustment is needed; in severe impairment of both liver and kidneys careful monitoring is required.
  • Elderly: with satisfactory kidney and liver function the dose is not changed.
  • Newborns: the medicine must not be given to premature babies or to newborns up to 28 days with jaundice, certain blood disorders or a need for intravenous calcium.
  • People on a low-sodium diet: 1 g contains 83 mg (3.6 mmol) and 2 g contains 166 mg (7.2 mmol) of sodium.

Contraindications – when the medicine must not be given

  • allergy to ceftriaxone or any other cephalosporin (e.g. cefalexin, cefaclor, cefuroxime)
  • sudden or severe allergic reaction to penicillin or similar antibiotics (cephalosporins, carbapenems, monobactams)
  • allergy to lidocaine – if the medicine is to be given into a muscle
  • premature babies; newborns up to 28 days with blood disorders or jaundice, or who need to receive a calcium-containing medicine into a vein

Warnings and precautions

  • Tell the doctor if you have had serious skin reactions (rash, blisters, peeling skin, high fever, enlarged lymph nodes) or if you have recently received or need to receive calcium-containing products.
  • Caution with previous diarrhoea after antibiotics and colitis, liver or kidney disease, gallstones or kidney stones, haemolytic anaemia and a low-sodium diet.
  • Regular blood tests may be needed during longer treatment. The medicine may affect urine sugar tests and the Coombs test.
  • Some blood glucose monitoring systems may show incorrect values during treatment – people with diabetes should check the device instructions and tell their doctor; another testing method should be used if necessary.

Interactions with other medicines

  • aminoglycoside antibiotics
  • chloramphenicol (an antibiotic)
  • oral anticoagulants, e.g. warfarin
  • calcium-containing solutions and infusions (e.g. Ringer’s or Hartmann’s solution, parenteral nutrition) – must not be mixed or given at the same time

Tell your doctor about all medicines you are taking, have recently taken or might take.

Pregnancy and breast-feeding

If you are pregnant, breast-feeding or planning a pregnancy, ask your doctor – they will weigh the benefit of treatment against the risk to the child. Ceftriaxone crosses the placenta, and in pregnancy, especially in the first trimester, it is used only if the benefit outweighs the risk.

Ceftriaxone passes into breast milk in small amounts; a risk of diarrhoea and fungal infections of the mucous membranes in the infant cannot be ruled out, so the doctor decides whether to stop breast-feeding or treatment.

Driving and using machines

Ceftriaxon-MIP may cause dizziness. If you feel dizzy, do not drive or use machines and tell your doctor.

Possible side effects

  • Common (up to 1 in 10 patients): changes in white blood cell and platelet counts, soft stools or diarrhoea, changes in liver function tests, rash.
  • Uncommon (up to 1 in 100): fungal infections (e.g. candidiasis), granulocytopenia, anaemia, clotting problems (bruising, joint pain and swelling), headache, dizziness, nausea or vomiting, itching, pain or burning along the vein and pain at the injection site, fever, raised creatinine.
  • Rare (up to 1 in 1,000): inflammation of the large bowel (diarrhoea with blood and mucus, abdominal pain, fever), encephalopathy (especially in elderly patients with serious kidney or nervous system disease), bronchospasm, hives, blood or sugar in the urine, oedema, shivering.
  • Frequency not known: serious allergic and skin reactions (SJS, TEN, DRESS), Jarisch-Herxheimer reaction in Lyme disease, secondary infection, haemolytic anaemia, agranulocytosis, convulsions, pancreatitis, stomatitis, glossitis, gallbladder and liver problems, kernicterus in newborns with severe jaundice, ceftriaxone-calcium deposits in the kidneys, false-positive Coombs test and galactosaemia test.

Report any suspected side effects to your doctor, nurse or ALIMS.

Overdose, missed dose and stopping treatment

  • If you accidentally receive more than the prescribed dose, tell your doctor immediately.
  • A missed injection should be given as soon as possible; if it is almost time for the next one, the missed dose is skipped. Two injections are not given at the same time.
  • Treatment is not stopped unless the doctor advises it.

When to seek medical help immediately?

  • sudden swelling of the face, throat, lips or mouth, difficulty breathing or swallowing; sudden swelling of the hands, feet or ankles
  • severe, rapidly spreading rash with blisters or peeling skin and blisters in the mouth
  • widespread rash with high fever and enlarged lymph nodes
  • diarrhoea with blood and mucus, severe abdominal pain spreading to the back
  • pain on urination or reduced urination, yellowing of the skin, dark urine

Pharmacist's advice

Before the first dose tell the doctor about all allergies – to antibiotics, but also to lidocaine if you receive the medicine into a muscle – as well as about medicines against blood clotting and calcium preparations. If you have diabetes and use a glucose meter, ask whether another testing method is needed during treatment.

Soft stools and mild diarrhoea are common. Ask your doctor or pharmacist about using probiotics, and report diarrhoea with blood, mucus or severe abdominal pain immediately. During the infection monitor your body temperature and tell the doctor if fever returns or does not settle.

Frequently asked questions

Can Ceftriaxon-MIP be obtained from a pharmacy?

No. The medicine is used in a healthcare facility and is given by a doctor or nurse.

Why must ceftriaxone not be given with calcium?

Ceftriaxone and calcium can form a precipitate. They are therefore not mixed or given at the same time, including parenteral nutrition infusions, and the medicine must not be given to newborns receiving intravenous calcium.

Why is lidocaine used for the injection into a muscle?

Lidocaine is used as the solvent for intramuscular injection. It is therefore important to tell the doctor if you are allergic to lidocaine; such a solution is never given into a vein.

Can ceftriaxone affect blood sugar measurement?

Yes – some blood glucose monitoring systems may show incorrect values during treatment. Check the device instructions and ask your doctor.

How many times a day is ceftriaxone given?

Usually once a day. For doses above 2 g the doctor may divide the dose into two administrations (every 12 hours).
