Amoksiklav 2X is a combination antibiotic containing two active substances: amoxicillin, a penicillin antibiotic, and clavulanic acid, which prevents bacteria from breaking down amoxicillin and making it inactive. This page refers to Amoksiklav 2X film-coated tablets of 875 mg/125 mg. A powder for oral suspension of 400 mg/57 mg per 5 mL for children is also authorised under the same name; it has a separate leaflet and weight-based dosing, which is not given here. The medicine is available on prescription.
The combination extends the action of amoxicillin to bacteria that produce beta-lactamases, so it is used for infections of the ear and sinuses, airways, urinary tract, skin and soft tissues (including dental infections) and bones and joints. Antibiotics do not work against viral infections such as colds and flu.
Important warning
- Supply status in Serbia: prescription-only medicine.
- Do not take the medicine if you are allergic to amoxicillin, clavulanic acid or penicillins, if you have ever had a severe allergic reaction to any antibiotic, or if you have ever had liver problems or jaundice when taking an antibiotic.
- Stop taking it immediately and contact your doctor if you develop swelling of the face or mouth, difficulty breathing, a widespread rash with blistering or peeling skin, or watery diarrhoea with blood and mucus.
- Take the medicine at the start of a meal; do not take it for longer than two weeks without seeing the doctor again.
- This page is for information only and is not an offer to buy the medicine online.

- urinary tract infections;
- skin and soft tissue infections, including dental infections;
- bone and joint infections.
In some cases the doctor may test which bacterium is causing the infection and, depending on the result, prescribe a different strength or another medicine.
How is it used and dosed?
The dose and duration of treatment are decided by the doctor. The dosing below is from the official leaflet and is for information only.
| Patient group | Dose according to the PIL |
|---|---|
| Adults and children weighing 40 kg and more | usually 1 tablet twice daily; higher dose – 1 tablet three times daily |

- Swallow the tablet whole with a glass of water at the start of a meal or just before a meal; it can be broken along the score line to make swallowing easier, but both halves are taken at the same time.
- Spread doses evenly through the day, at least 4 hours apart; do not take two doses within one hour.
- Do not take the medicine for longer than two weeks; if you still do not feel well, see your doctor again.
Special patient groups
- Children: 875 mg/125 mg tablets are not suitable for children weighing less than 25 kg; for children aged 6 years and under Amoksiklav 2X oral suspension is recommended, and in children under 40 kg tablets are used only on the doctor’s advice.
- Kidney impairment: the dose may be changed; the doctor may decide that a different strength or another medicine is needed.
- Liver impairment: more frequent blood tests may be needed to check liver function; if you have previously had liver problems or jaundice caused by an antibiotic, the medicine must not be taken.
- Elderly patients: dosed as adults, with adjustment for reduced kidney function.
Contraindications – when the medicine must not be used
Do not take Amoksiklav 2X:
- if you are allergic to amoxicillin, clavulanic acid, penicillin or any of the excipients;
- if you have ever had a severe allergic reaction to any antibiotic (skin rash, swelling of the face or neck);
- if you have ever had liver problems or jaundice (yellowing of the skin and eyes) when taking an antibiotic.
Precautions and warnings
- Tell your doctor if you have infectious mononucleosis (glandular fever), have been treated for liver or kidney problems, or do not pass urine regularly.
- The medicine may worsen certain conditions or cause serious side effects – allergic reactions, convulsions and inflammation of the large bowel; watch for the symptoms listed in the side effects section.
- The medicine may affect blood tests (red blood cells, liver function) and urine glucose tests – tell the doctor or laboratory that you are taking it.
- The medicine contains less than 1 mmol (23 mg) sodium per tablet, i.e. it is essentially sodium-free.
Interactions with other medicines, food and drink
Tell your doctor about all medicines, including herbal and non-prescription medicines. According to the leaflet:
- allopurinol (for gout) – greater likelihood of an allergic skin reaction;
- probenecid (for gout) – the doctor may adjust the dose;
- anticoagulants (e.g. warfarin) – additional blood tests may be needed;
- methotrexate (for cancer or rheumatic diseases) – the medicine may affect its action;
- mycophenolate mofetil (to prevent transplant rejection) – the medicine may affect its action.
Food: take the medicine at the start of a meal or just before a meal.
Pregnancy and breastfeeding
Use during pregnancy should be avoided unless the doctor considers it necessary.
During breastfeeding the medicine is taken only after the doctor has assessed the benefits and risks.
Driving and using machines
The medicine may cause side effects (e.g. allergic reactions, dizziness, convulsions) that affect your ability to drive and use machines. If you do not feel well, do not drive or use machines.
Possible side effects
Tell your doctor immediately and stop taking the medicine in case of: allergic reaction (rash, vasculitis with red or purple spots, fever, joint pain, swollen lymph glands, swelling of the face and mouth with difficulty breathing, collapse); inflammation of the large bowel (watery diarrhoea, usually with blood and mucus, abdominal pain, fever). Drug-induced enterocolitis syndrome (DIES), reported mainly in children – repeated vomiting 1–4 hours after a dose, abdominal pain, lethargy, diarrhoea, low blood pressure; acute pancreatitis – severe, persistent abdominal pain; chest pain as part of an allergic reaction (Kounis syndrome). Also tell your doctor immediately if you notice: linear IgA disease (blisters in a ring or string-of-pearls pattern), hepatitis or jaundice, inflammation of the kidney tubules, prolonged blood clotting, hyperactivity, convulsions, black hairy-looking tongue.
| Frequency | Side effects (from the PIL) |
|---|---|
| Very common (more than 1 in 10) | diarrhoea (in adults) |
| Common (up to 1 in 10) | thrush (candida infection of the vagina, mouth or skin folds), nausea (especially with higher doses – take with a meal), vomiting, diarrhoea (in children) |
| Uncommon (up to 1 in 100) | rash, itching, hives, indigestion, dizziness, headache; increased liver enzymes in blood tests |

| Not known | aseptic meningitis; severe skin reactions – Stevens-Johnson syndrome, toxic epidermal necrolysis, bullous exfoliative dermatitis, exanthematous pustulosis, DRESS, SDRIFE (red rash on the buttocks, inner thighs, armpits, neck); severe reduction of white blood cells, haemolytic anaemia, crystals in the urine with acute kidney injury |
Overdose, missed dose and stopping treatment
- Overdose: stomach upset (nausea, vomiting, diarrhoea) or convulsions may occur. Contact your doctor as soon as possible and take the pack with you.
- Missed dose: take the tablet as soon as you remember, and the next dose only about 4 hours later; do not take a double dose.
- Stopping treatment: keep taking the medicine until the end of the prescribed course, even if you feel better – surviving bacteria may cause the infection to return. Do not take it for longer than two weeks without seeing the doctor again.
When to seek medical help immediately?
- swelling of the face, lips or throat, difficulty breathing or collapse;
- widespread rash with blistering, peeling skin or pus-filled spots;
- watery diarrhoea with blood and mucus, with abdominal pain and fever;
- yellowing of the skin or eyes;
- repeated vomiting 1–4 hours after a dose (especially in children) or severe, persistent abdominal pain;
- convulsions (fits).
Pharmacist's advice
Amoxicillin with clavulanic acid is best tolerated when taken at the very start of a meal – this reduces nausea and stomach upset. Keep the intervals between doses even, at least 4 hours.
Diarrhoea is very common in adults; probiotics with Saccharomyces boulardii or lactic acid bacteria, taken at a different time from the antibiotic, may support the gut flora. If diarrhoea is watery, bloody or accompanied by fever, contact your doctor.
If you have previously had jaundice or liver problems after an antibiotic, be sure to tell your doctor – this medicine must not be used in that case. Women prone to fungal infections should react early to itching or discharge.
