Tenox is a medicine containing the active substance amlodipine (as amlodipine maleate) and belongs to the group of calcium antagonists – selective calcium channel blockers with mainly vascular effects. It is available as 5 mg and 10 mg tablets.

In high blood pressure, Tenox relaxes and widens the blood vessels so that blood passes through them more easily. In angina pectoris, it improves the blood and oxygen supply to the heart muscle and thus prevents chest pain. The medicine does not provide immediate relief of chest pain that has already started.

Important warning

  • Tenox is a prescription-only medicine (regime R in Serbia); the dose is set by the doctor.
  • It must not be used in severe hypotension, significant aortic stenosis, cardiogenic shock or heart failure after a heart attack.
  • Do not consume grapefruit or grapefruit juice during treatment; St John's wort and some antibiotics, antifungals and HIV medicines change the effect of amlodipine.
  • The medicine is taken regularly, once a day – do not stop treatment without consulting your doctor.
  • This page is for information only and is not an offer to buy the medicine online. Read the official patient leaflet before use.

Special patient groups

  • Children and adolescents: amlodipine is used only for high blood pressure in patients aged 6 to 17 years, at a maximum of 5 mg daily.
  • Elderly: according to the summary of product characteristics, the usual dosage is used, but dose increases are made with caution.
  • Kidney impairment: according to the SmPC, amlodipine is used at normal doses.
  • Liver disease: inform your doctor before treatment.

Contraindications – when the medicine must not be used

  • allergy to amlodipine, other calcium antagonists or any excipient (redness, rash, itching, difficulty breathing);
  • severely low blood pressure (hypotension);
  • clinically significant narrowing of the aortic valve (aortic stenosis) or cardiogenic shock;
  • heart failure after a heart attack.

Precautions and warnings

Tell your doctor if you have or have had:

  • a recent heart attack;
  • heart failure;
  • a severe increase in blood pressure (hypertensive crisis);
  • liver disease;
  • if you are elderly and your dose may need to be increased.

Interactions with other medicines, food and drink

Some medicines affect Tenox, and Tenox affects other medicines:

  • ketoconazole, itraconazole (antifungals);
  • ritonavir, indinavir, nelfinavir (HIV medicines);
  • rifampicin, erythromycin, clarithromycin (antibiotics);
  • St John's wort (Hypericum perforatum);
  • verapamil, diltiazem (heart medicines);
  • dantrolene (infusion for severe body temperature disorders);
  • tacrolimus, sirolimus, temsirolimus, everolimus and ciclosporin;
  • simvastatin (cholesterol medicine).

The blood-pressure-lowering effect is stronger if you already take other medicines for blood pressure. Grapefruit and grapefruit juice can increase amlodipine levels in the blood and cause an unpredictable drop in blood pressure.

Pregnancy and breastfeeding

The safety of amlodipine in pregnancy has not been established – if you are pregnant or planning a pregnancy, consult your doctor before starting treatment. Amlodipine passes into breast milk in small amounts; if you are breastfeeding or plan to, tell your doctor.

Driving and using machines

Tenox may affect your ability to drive or operate machines. If you feel sick, dizzy, tired or have a headache, do not drive or use machines and contact your doctor.

Possible side effects

  • Very common: oedema (swelling due to fluid retention).
  • Common: headache, dizziness, sleepiness (especially at the start), palpitations, flushing, abdominal pain, nausea, changed bowel habits, tiredness, weakness, visual disturbances, muscle cramps, shortness of breath, ankle swelling.

  • Very rare: blood count disorders, hyperglycaemia, swollen gums, gastritis, pancreatitis, hepatitis and jaundice, angioedema, severe skin reactions, sensitivity to light.

Overdose, missed dose and stopping treatment

  • Overdose: too many tablets may cause a dangerous drop in blood pressure (dizziness, fainting, weakness, even shock with cold, clammy skin) – contact a doctor or the nearest hospital immediately.
  • Missed dose: skip it and take the next dose at the usual time; do not take a double dose.
  • Stopping treatment: keep taking the medicine until your doctor tells you to stop – symptoms may return.

When to seek medical help immediately?

  • sudden wheezing, chest pain, shortness of breath or difficulty breathing;
  • swelling of the eyelids, face, lips, tongue or throat;
  • severe skin reactions – intense rash, redness of the whole body, blisters, peeling skin, inflamed mucous membranes;
  • signs of a heart attack or heart rhythm disorder;
  • severe pain in the abdomen and lower back with a general feeling of weakness (possible pancreatitis).

Pharmacist's advice

Take Tenox at the same time every day – many patients find it helpful to link the tablet to a daily routine. Measuring blood pressure at home (at rest, ideally at the same time of day) and recording the values helps your doctor assess the treatment.

Ankle swelling is common with amlodipine; if it is marked or lasts longer than a week, contact your doctor – do not stop the medicine yourself. Avoid grapefruit and ask the pharmacist before any new medicine or herbal product (especially St John's wort).
