Tamiflu is an antiviral medicine containing oseltamivir (as oseltamivir phosphate) and belongs to the group of neuraminidase inhibitors – medicines that prevent the flu virus from spreading in the body and relieve or prevent the symptoms of infection. In Serbia it is authorised as 75 mg hard capsules; the marketing authorisation holder is Roche d.o.o. Belgrade.
Tamiflu is used to treat influenza (flu) and, in certain cases, to prevent flu – for example after contact with an infected person. Treatment should start as soon as possible, ideally within two days of the first symptoms. Tamiflu is not a vaccine and does not replace flu vaccination.
Important warning
- Supply status in Serbia: prescription-only medicine.
- Treatment is ideally started within 2 days of the first symptoms and lasts 5 days (10 days with weakened immunity), even if you feel better.
- In children and adolescents watch especially for changes in behaviour (confusion, hallucinations, agitation) – rare but serious neuropsychiatric effects require urgent help.
- In kidney impairment the dose is adjusted; the 75 mg strength is intended for people weighing over 40 kg.
- This page is for information only and is not an offer to buy the medicine online.

What is Tamiflu used for?
According to the official leaflet, Tamiflu is used for:
- treatment of flu in adults, adolescents, children and infants (including full-term newborns) with flu symptoms when the flu virus is known to be circulating in the community;
- prevention of flu in adults, adolescents, children and infants over one year of age, on a case-by-case basis – e.g. after contact with an infected person;
- prevention in special circumstances, e.g. during a flu pandemic and when the seasonal vaccine does not provide sufficient protection.
Typical flu symptoms are a sudden rise in temperature (above 37.8 °C), cough, runny or blocked nose, headache, muscle aches and marked tiredness. Outside epidemic periods similar symptoms are more often caused by other infections.
How is it used and dosed?
The dose is based on body weight. The 75 mg capsule corresponds to the dose for people weighing over 40 kg (adults, adolescents aged 13 and over and children over 40 kg):
| Purpose | Dose | Duration |
|---|---|---|
| Treatment of flu | 75 mg twice daily (morning and evening) | 5 days |
| Treatment with weakened immune system | 75 mg twice daily | 10 days |
| Prevention after contact with an infected person | 75 mg once daily, preferably in the morning with breakfast | 10 days |
| Special circumstances (widespread flu, weakened immunity) | 75 mg once daily | as decided by the doctor, up to 6 or 12 weeks |
- Swallow the capsule whole with water; do not break or chew it.
- The medicine can be taken with or without food, but taking it with food reduces the chance of nausea and vomiting.
- Children under 40 kg and infants need lower weight-based doses set by the doctor; the 75 mg strength is not intended for these doses.
- For people unable to swallow capsules, the leaflet describes how to prepare a liquid form at home – ask your doctor or pharmacist.
Special patient groups
Kidney impairment – treatment doses for adults and adolescents aged 13–17 according to the summary of product characteristics:
| Creatinine clearance | Recommended treatment dose |
|---|---|
| > 60 mL/min | 75 mg twice daily |
| > 30 to 60 mL/min | 30 mg twice daily |
| > 10 to 30 mL/min | 30 mg once daily |
| ≤ 10 mL/min | not recommended (no data) |
| Haemodialysis | 30 mg after each haemodialysis session |
| Peritoneal dialysis | 30 mg, single dose |
- A 30 mg dose cannot be achieved with the 75 mg capsule – the doctor decides on the appropriate form.
- Dose adjustment is also needed for prevention in severe kidney impairment.
- Elderly: no dose adjustment needed unless there is moderate or severe kidney impairment.
- For infants and children up to 12 years with kidney impairment there are insufficient data for recommendations.
Contraindications – when the medicine must not be used
Do not take Tamiflu:
- if you are allergic (hypersensitive) to oseltamivir or any of the excipients.
The other conditions listed are not contraindications but require medical advice.
Precautions and warnings
- Allergy to other medicines.
- Kidney problems – the dose may need adjustment.
- Serious health problems requiring urgent hospitalisation.
- Weakened immune system (treatment lasts 10 days).
- Chronic heart or lung disease.
- Changes in behaviour or mood during treatment (neuropsychiatric events), especially in children and adolescents – tell the doctor immediately.
- Tamiflu does not affect the effectiveness of the flu vaccine; the doctor may prescribe Tamiflu to vaccinated people too.
- The medicine contains less than 1 mmol sodium per capsule (essentially sodium-free).
Interactions with other medicines, alcohol and food
- Of particular importance: chlorpropamide (diabetes), methotrexate (e.g. rheumatoid arthritis), phenylbutazone (pain and inflammation) and probenecid (gout).
- According to the SmPC, clinically relevant interactions are unlikely due to low protein binding and metabolism independent of CYP450.
- Food: the medicine can be taken with or without food; with food nausea is reduced.
Pregnancy and breastfeeding
Tell your doctor if you are pregnant or planning pregnancy so they can decide whether Tamiflu is suitable. According to the SmPC, data on more than 1,000 pregnancies exposed in the first trimester do not indicate harm to the fetus, while influenza itself is associated with pregnancy risks.
Oseltamivir and its active metabolite pass into breast milk in small amounts; use can be considered where there is a clear benefit for the breastfeeding mother – the doctor decides.
Driving and using machines
Tamiflu has no influence on the ability to drive and use machines.
Possible side effects
Many side effects may also be caused by the flu itself.
- Adults and adolescents – very common: headache, nausea; common: bronchitis, herpes infection, cough, dizziness, fever, pain (including in limbs), runny nose, sleep problems, sore throat and abdominal pain, tiredness, bloating, upper respiratory tract infections, stomach discomfort, vomiting.
- Uncommon: allergic reactions, altered consciousness, seizures, heart arrhythmias, liver function disorders, skin reactions; rare: low platelet count, visual disturbances.
- Children 1–12 years – very common: cough, blocked nose, vomiting; common: conjunctivitis, ear inflammation, headache, nausea, runny nose, abdominal pain, bloating.
- Rare, serious (post-marketing): anaphylactic reactions, fulminant hepatitis and jaundice, angioedema, Stevens-Johnson syndrome and TEN, gastrointestinal bleeding, seizures, delirium, confusion, hallucinations, nightmares – mostly in children and adolescents, in a few cases with self-harm.
Report suspected side effects to your doctor, pharmacist or ALIMS.
Overdose, missed dose and stopping treatment
- Overdose: stop taking the medicine and contact a doctor or pharmacist immediately. In most cases no side effects were reported; when they were, they were similar to those at normal doses. Overdose has been reported more often in children.
- Missed dose: do not take a double dose to make up for a missed capsule.
- Stopping treatment: there are no side effects on stopping, but if you stop early the flu symptoms may return – complete the full prescribed course.
When to seek medical help immediately?
- swelling of the face, skin, eyes or tongue, itchy rash, difficulty breathing, drop in blood pressure
- seizures, confusion, hallucinations, unusual or dangerous behaviour (especially in children and adolescents)
- yellowing of the skin and eyes, change in stool colour
- fever and sore throat followed by blisters and peeling skin
- blood in stools or coughing up blood
- worsening flu symptoms, difficulty breathing or persistent fever
Pharmacist's advice
Tamiflu works best when started within the first two days of illness – do not delay seeing a doctor when typical flu symptoms appear during the season. Take the capsule with a meal to reduce nausea and always complete the full five-day course.
While you are ill, drink plenty of fluids, monitor your temperature and rest. If a child is being treated with Tamiflu, do not leave them unsupervised for long in the first days and watch for behavioural changes.

Frequently asked questions
How soon should Tamiflu be started?
As soon as possible – ideally within two days of the first flu symptoms. Treatment lasts 5 days (10 days with weakened immunity) and should be completed even if you feel better quickly.
Does Tamiflu replace the flu vaccine?
No. Tamiflu is not a vaccine – it treats the infection or prevents the virus from spreading, whereas a vaccine stimulates antibody production. Tamiflu does not change the effectiveness of the vaccine and can also be prescribed to a vaccinated person.
Can Tamiflu be used for prevention?
Yes, the doctor decides case by case – e.g. after close contact with an infected person. For people over 40 kg the usual dose is 75 mg once daily for 10 days, preferably in the morning with breakfast.
Are there Tamiflu capsules for children or a syrup?
According to ALIMS lists, the authorisations for Tamiflu 30 mg and 45 mg capsules expired between December 2024 and March 2025, and for the 6 mg/mL oral suspension in 2019. The currently valid authorisation covers the 75 mg capsules; for children under 40 kg the doctor decides on dose and form.
Can I take Tamiflu if I have kidney problems?
Yes, but at an adjusted dose based on creatinine clearance, set by the doctor. In severe impairment (clearance ≤ 10 mL/min) use is not recommended.
