Amoxicillin Remedica is a broad-spectrum penicillin antibiotic containing the active substance amoxicillin (as amoxicillin trihydrate). It is authorised as 500 mg hard capsules; a 250 mg dose cannot be achieved with this medicine. The medicine is available on prescription.
Amoxicillin acts against bacteria sensitive to this antibiotic and is used for infections of the airways, ear, throat, urinary tract, teeth and other bacterial infections, as well as in combination therapy to eradicate Helicobacter pylori. Antibiotics do not work against viral infections such as colds and flu.
Important warning
- Supply status in Serbia: prescription-only medicine.
- Do not take the medicine if you are allergic to amoxicillin or other penicillins, or if you have ever had an allergic reaction to another beta-lactam antibiotic (e.g. cephalosporins).
- Stop taking it immediately and contact your doctor if you develop difficulty breathing, swelling of the face, lips, tongue or throat, or a widespread rash with blistering or peeling skin.
- If severe, persistent or bloody diarrhoea occurs, stop taking the medicine and contact your doctor; do not take medicines that slow bowel movement on your own.

What is Amoxicillin Remedica used for?
According to the approved summary of product characteristics, amoxicillin is indicated in adults and children for the treatment of the following infections:
- acute bacterial sinusitis and acute middle ear infection (otitis media);
- acute streptococcal tonsillitis and pharyngitis;
- acute exacerbations of chronic bronchitis and community-acquired pneumonia;
- acute cystitis, asymptomatic bacteriuria in pregnancy and acute pyelonephritis;
- typhoid and paratyphoid fever;
- dental abscess with spreading cellulitis;
- prosthetic joint infections;
- eradication of Helicobacter pylori (in combination with other medicines);
- Lyme disease.
Amoxicillin is also used to prevent endocarditis (infection of the heart valves) before certain procedures. The doctor follows official guidance on the appropriate use of antibiotics.
How is it used and dosed?
The dose and duration of treatment are decided by the doctor according to the type and severity of infection, age, body weight and kidney function. The doses below are from the official leaflet and are for information only.
| Patient group | Usual dose according to the PIL |
|---|---|
| Adults, elderly and children ≥ 40 kg | 250–500 mg three times daily or 750 mg–1 g every 12 hours, depending on severity and type of infection; maximum 6 g daily |
| Severe infections | 750 mg–1 g three times daily |
| Urinary tract infections (acute cystitis) | 3 g twice daily for one day |
| Lyme disease | early stage (isolated erythema migrans): 4 g daily; late stage (systemic manifestations): up to 6 g daily |
| Stomach / duodenal ulcer | 750 mg or 1 g twice daily for 7 days, with other antibiotics and ulcer medicines |
| Prevention of heart infections during surgery | dose depends on the type of procedure – decided by the doctor |
| Children < 40 kg | usually 40–90 mg/kg daily in two or three doses; maximum 100 mg/kg daily |
- Swallow the capsule whole (do not open) with a glass of water; spread doses evenly through the day, at least 4 hours apart.
- A 250 mg dose cannot be achieved with this medicine – another medicine of a suitable strength is used for that dose.
- If you have kidney problems the dose may be lower than usual.
Special patient groups
- Children: the dose is calculated by body weight (usually 40–90 mg/kg daily in two or three doses, maximum 100 mg/kg daily); from 40 kg adult doses apply. The 500 mg capsule is not suitable for children who cannot swallow capsules – the doctor then prescribes another form of amoxicillin.
- Elderly patients: dosed as adults; with reduced kidney function the doctor adjusts the dose.
- Kidney impairment: the dose is reduced depending on the degree of impairment.
- Liver impairment: caution and regular monitoring of liver function are needed.
Contraindications – when the medicine must not be used
Do not take Amoxicillin Remedica:
- if you are allergic to amoxicillin, penicillins or any of the excipients;
- if you have ever had an allergic reaction (rash, swelling of the face or throat) to any antibiotic; according to the SmPC this applies especially to a severe reaction to another beta-lactam antibiotic (cephalosporin, carbapenem, monobactam).
Precautions and warnings
- Tell your doctor if you have infectious mononucleosis (fever, sore throat, swollen glands, extreme tiredness), kidney problems or problems passing urine.
- According to the SmPC, generalised redness with fever and small pus-filled spots at the start of treatment (AGEP) requires stopping the medicine and is a contraindication to further use.
- When treating Lyme disease a Jarisch-Herxheimer reaction may occur (fever, chills, headache, muscle pain, rash).
- With longer use a fungal infection of moist body areas (candidiasis) is possible, and the doctor may check kidney, liver and blood counts. Drink enough fluids (risk of crystals in the urine).
- The medicine may affect urine glucose tests, liver function tests and oestriol tests in pregnancy – tell the doctor or laboratory that you are taking it.
- The medicine contains the azo dye Sunset yellow FCF (E110), which may cause allergic reactions.
Interactions with other medicines, food and drink
Tell your doctor about all medicines you take. According to the leaflet, the following are important:
- allopurinol (for gout) – greater likelihood of an allergic skin reaction;
- probenecid (for gout) – slows the excretion of amoxicillin, the doctor may adjust the dose;
- anticoagulants (warfarin, acenocoumarol) – additional blood tests may be needed;
- other antibiotics such as tetracyclines – amoxicillin may be less effective;
- methotrexate (for cancer and severe psoriasis) – side effects may increase.
Pregnancy and breastfeeding
Amoxicillin can be used in pregnancy when the potential benefit outweighs the risk; limited data do not indicate an increased risk of birth defects.
Amoxicillin passes into breast milk in small amounts, so the infant may develop diarrhoea, fungal infection or sensitisation; during breastfeeding the medicine is used only after the doctor’s assessment.
Driving and using machines
Possible side effects such as allergic reactions, dizziness and convulsions may affect your ability to drive and use machines. Do not drive if you do not feel well.
Possible side effects
Stop taking the medicine and contact your doctor immediately (very rare): allergic reactions (itching, rash, swelling of the face, lips, tongue or body, difficulty breathing); rash or small red spots and bruising due to inflamed blood vessels, sometimes with joint pain and kidney problems; delayed allergic reaction 7–12 days after starting (rash, fever, joint pain, enlarged lymph nodes); erythema multiforme and other severe skin reactions; DRESS; fever, chills, sore throat or easy bruising (blood cell problems); Jarisch-Herxheimer reaction; inflammation of the bowel with diarrhoea (sometimes bloody); serious liver problems (severe bloody diarrhoea, blisters, dark urine, pale stools, jaundice).
| Frequency | Other side effects (from the PIL) |
|---|---|
| Common (up to 1 in 10) | skin rash, nausea, diarrhoea |
| Uncommon (up to 1 in 100) | vomiting; mild itchy rash or hive-like swellings |
| Very rare (up to 1 in 10,000) | candidiasis (vagina, mouth, skin folds), kidney problems, convulsions (with high doses or reduced kidney function), dizziness, hyperactivity, crystals in the urine, yellow, brown or black tongue, haemolytic anaemia, low white blood cells and platelets, prolonged blood clotting |
These reactions may occur while taking the medicine or several weeks after finishing treatment.
Overdose, missed dose and stopping treatment
- Overdose: nausea, vomiting, diarrhoea or crystals in the urine (cloudy urine, problems passing urine) may occur. Contact your doctor as soon as possible and take the pack with you.
- Missed dose: take the prescribed dose as soon as you remember, making sure at least 4 hours pass before the next dose. Never take a double dose.
- Stopping treatment: keep taking the medicine for as long as prescribed – if some bacteria survive, the infection may return. If you still feel unwell after finishing the course, see your doctor again.
When to seek medical help immediately?
- difficulty breathing, swelling of the face, lips, tongue or throat;
- widespread rash with blistering or peeling skin, with fever;
- severe or persistent diarrhoea, especially with blood;
- yellowing of the skin or eyes, dark urine and pale stools;
- fever, chills, sore throat or unusual bruising and bleeding.
Pharmacist's advice
Take amoxicillin at evenly spaced intervals during the day (at least 4 hours between two doses) and finish the whole prescribed course – stopping too early is a common reason for the infection returning and for bacterial resistance.
Mild diarrhoea is common with antibiotics; many patients use probiotics with Saccharomyces boulardii or lactic acid bacteria, taken at a different time from the antibiotic. If diarrhoea is severe, persistent or bloody, contact your doctor.
The 500 mg capsule is intended mainly for adults and older children; if a child cannot swallow capsules, the doctor may prescribe amoxicillin as a suspension.

Frequently asked questions
Can I take half a capsule if 250 mg was prescribed?
No. According to the leaflet, a 250 mg dose cannot be achieved with this medicine, and the capsule should not be opened. For a 250 mg dose the doctor prescribes a medicine of a suitable strength.
How far apart should the doses be?
Doses should be spread evenly through the day, at least 4 hours apart – usually three times daily or every 12 hours, as prescribed by the doctor.
What if I get a rash while taking amoxicillin?
A mild itchy rash may occur uncommonly, but any rash should be reported to the doctor because it may be a sign of allergy. If the rash spreads, blisters form, or facial swelling and difficulty breathing occur, stop the medicine immediately and seek help.
Does amoxicillin work on a cold?
No. Antibiotics only work on bacterial infections; colds and flu are usually viral. Take amoxicillin only when prescribed by a doctor.
