Sermion 30 mg is a prescription medicine containing nicergoline. Nicergoline is an ergot derivative that acts on several mechanisms associated with cerebral circulation, nerve-cell function and cognitive performance.
According to the current Serbian product information, Sermion is used for symptomatic treatment of dementia-related diseases and selected disturbances of cognition, mood and attention following a stroke.
Sermion is not an emergency treatment for an acute stroke and must not be started independently for forgetfulness, dizziness or headache. Sudden one-sided weakness, facial drooping, difficulty speaking, severe confusion or loss of balance requires emergency medical care.

What is Sermion used for?
Sermion is used for symptomatic treatment of:
- dementia-related diseases of the Alzheimer type;
- dementia associated with Parkinson’s disease;
- vascular and multi-infarct dementia;
- other dementia-related diseases when assessed as appropriate by a doctor;
- post-stroke conditions involving disturbances of cognitive function, mood or attention.
Sermion may reduce selected symptoms, but it does not cure the underlying cause of dementia, restore permanently damaged brain tissue or treat an acute stroke.
Changes in memory, behaviour or orientation require medical assessment. Similar symptoms may be caused by depression, thyroid disorders, vitamin B12 deficiency, infection, dehydration or adverse effects of other medicines.
How is Sermion taken?
Take the medicine exactly as prescribed. The recommended adult dose is:
- one 30 mg tablet once daily; or
- one 30 mg tablet twice daily, providing 60 mg daily, when prescribed by the doctor.
The usual daily dose is 30 mg. It may be temporarily increased to 60 mg only under medical supervision.
Swallow the tablet whole with a small amount of liquid, without chewing, preferably before a meal:
- when taken once daily – take it in the morning;
- when taken twice daily – take it in the morning and before the main meal.
The effect develops gradually. The doctor should reassess the need for continued treatment periodically and at least once every six months.
Older patients and kidney impairment
Age alone does not normally require automatic dose adjustment. However, low blood pressure, a slow heart rate, impaired kidney function and multiple concurrent medicines are more common in older patients.
Nicergoline and its metabolites are mainly eliminated through the kidneys. A lower dose may be required in patients with impaired kidney function.
Use in children
The safety and effectiveness of nicergoline in children and adolescents have not been established. Sermion 30 mg is intended for adult patients.
Who must not take Sermion?
- people allergic to nicergoline;
- people allergic to another ergot alkaloid or an ingredient of the medicine;
- patients experiencing an acute myocardial infarction;
- patients with acute bleeding;
- patients with orthostatic hypotension;
- patients with marked bradycardia or a very slow heart rate.
Low blood pressure and slow heart rate
Nicergoline may lower blood pressure, particularly when combined with antihypertensive treatment. Possible symptoms include:
- dizziness or light-headedness;
- unsteadiness when standing;
- blurred vision;
- weakness and cold sweating;
- fainting;
- a slow pulse.
Stand up gradually from a bed or chair. Repeated dizziness or impaired balance should be reported because falls may have serious consequences in older patients.
- loss of consciousness;
- a very slow or irregular heartbeat;
- chest pain or tightness;
- severe weakness with cold sweating;
- sudden speech difficulty or one-sided weakness;
- bleeding that is difficult to stop.
Gout and increased uric acid
Nicergoline may increase blood uric-acid levels. Particular caution is required in patients with:
- current or previous gout;
- high uric-acid levels;
- treatment affecting uric-acid metabolism or excretion.
Sudden pain, redness, warmth and swelling of a joint, particularly the big toe, may indicate an acute gout attack.
Fibrosis and ergot-related symptoms
Nicergoline is an ergot derivative. Excessive formation of fibrous tissue involving the lungs, heart, heart valves or retroperitoneal area has been associated with certain medicines in this group.
Report persistent cough, breathlessness, chest pain, swollen legs, unexplained abdominal or back pain or unexplained weight loss.
Nausea, vomiting, diarrhoea, abdominal pain and marked coldness, tingling or pain in the hands and feet may be signs of excessive ergot-related effects.
Interactions with other medicines
Tell the doctor about all medicines, nasal decongestants, cold remedies, dietary supplements and herbal products.
Particular caution is required with:
- antihypertensive medicines, because their effects may be increased;
- beta blockers, because certain cardiac effects may be enhanced;
- antiplatelet and anticoagulant medicines, including aspirin, clopidogrel and warfarin, because bleeding time may be prolonged;
- sympathomimetic medicines, including certain nasal and cold products containing ephedrine or related substances;
- medicines metabolised through CYP2D6;
- gout medicines and other treatments affecting uric acid.
Do not automatically add ginkgo biloba. Ginkgo may affect blood clotting and requires professional assessment, particularly in patients taking aspirin, clopidogrel, warfarin or another anticoagulant.
Pregnancy and breastfeeding
Use during pregnancy is expected to be uncommon because of the approved indications. Sermion may be used only when the doctor determines that the expected benefit outweighs the potential risk to the fetus.
It is not known whether nicergoline passes into breast milk. Use while breastfeeding is therefore not recommended.
Driving and using machines
The effect of nicergoline on driving has not been specifically studied. The underlying disease may impair attention, and dizziness or drowsiness may occur during treatment.
Do not drive or operate machinery if drowsiness, dizziness, unsteadiness or low blood pressure occurs.
Possible side effects
A common side effect is abdominal discomfort.
Uncommon side effects include:
- agitation, confusion or insomnia;
- drowsiness, dizziness or headache;
- low blood pressure;
- flushing;
- itching;
- diarrhoea or constipation;
- nausea;
- increased blood uric acid.
Rash, a sensation of warmth and fibrosis have been reported with an unknown frequency.
Sudden or marked confusion in an older person should not automatically be attributed to dementia. It may be caused by infection, dehydration, low blood pressure or an adverse medicine effect.
Overdose and missed dose
Taking too many tablets may cause a marked fall in blood pressure. Contact a doctor or the nearest hospital immediately and take the original packaging.
Do not take a double dose to make up for a missed tablet. Continue according to the prescribed schedule.
Sermion should not be started simply because a person has become forgetful. The cause of the symptoms should first be investigated, together with the complete medicine list, blood pressure, heart rate and potentially reversible causes of cognitive decline. During treatment, monitor dizziness when standing, unusual bruising or bleeding and symptoms of gout. Tell the doctor if the patient uses aspirin, clopidogrel, warfarin, blood-pressure medicines or a ginkgo product.

Composition, appearance and packaging
Each film-coated tablet contains 30 mg of nicergoline.
The tablet core contains calcium hydrogen phosphate dihydrate, microcrystalline cellulose, magnesium stearate and sodium carboxymethylcellulose.
The coating contains hydroxypropyl methylcellulose, polyethylene glycol 6000, titanium dioxide, yellow iron oxide and silicone.
The tablets are round, biconvex and coated with an opaque yellow film. The pack contains two blisters of 15 tablets, providing 30 tablets in total.

Storage
Keep out of the sight and reach of children. Store below 25°C.
Do not use after the expiry date. Return unused tablets to a pharmacy that collects pharmaceutical waste.
Frequently asked questions
Does Sermion improve memory?
It may reduce selected symptoms involving cognition, mood and attention in appropriately assessed patients. The response is individual.
Does Sermion cure Alzheimer’s disease?
No. It is symptomatic treatment and does not remove the underlying cause or reliably stop disease progression.
Is Sermion used during an acute stroke?
No. It is not an emergency stroke treatment. It may be prescribed later for selected post-stroke symptoms.
Should it be taken before or after food?
It is preferably taken before a meal with a small amount of liquid and without chewing.
Can Sermion lower blood pressure?
Yes. It may cause or worsen low blood pressure, particularly with antihypertensive medicines.
Can Sermion be combined with ginkgo?
Not automatically. Professional assessment is required, particularly when aspirin, clopidogrel, warfarin or another anticoagulant is used.
How long is treatment continued?
Treatment may be prolonged, but the need to continue should be reviewed at least once every six months.
