Prospan 20 mg/mL oral drops, solution is a herbal medicine containing dry ivy leaf extract. It is used as an expectorant to make coughing up mucus easier in adults, adolescents and children aged two years and over who have a productive cough.

The medicine helps loosen thick mucus in the airways and makes it easier to clear by coughing. One millilitre of oral drops contains 20 mg of dry ivy leaf extract. The pack contains 20 mL of solution.

Important alcohol warning: Prospan oral drops contain 47% v/v ethanol. People whose doctor has strictly prohibited even the smallest amount of alcohol, for example after treatment for alcohol dependence, must not take this medicine. Special caution is required in children, people with liver disease or epilepsy, and during pregnancy and breastfeeding.

The information on this page does not replace advice from a doctor or pharmacist. Consult a doctor if the cough lasts longer than seven days, becomes worse or is accompanied by difficulty breathing, fever or purulent sputum.

What do Prospan oral drops contain?

  • Active substance: dry ivy leaf extract – Hedera helix L.
  • Strength: 20 mg of dry ivy leaf extract per 1 mL of solution.
  • Drug-to-extract ratio: 5–7.5:1.
  • Extraction solvent: ethanol 30% m/m.
  • Pharmaceutical form: oral drops, solution.
  • Pack size: dark glass bottle with a dropper containing 20 mL of solution.
  • Age group: adults and children aged two years and over.
  • Dispensing status: available without a medical prescription.

The other ingredients are sodium saccharin, anise essential oil, fennel essential oil, peppermint essential oil, ethanol 96% and purified water.

The solution is a dark brown liquid with an alcohol and fennel odour. Occasional cloudiness, suspended dark particles or sediment may occur in this herbal medicine.

What are Prospan oral drops used for?

Prospan oral drops are used as an expectorant for a productive cough, meaning a cough accompanied by thick mucus that needs to be cleared.

Dry ivy leaf extract helps loosen thick mucus in the airways and makes it easier to cough up.

Prospan is not automatically suitable for every type of cough. A dry, irritating cough without mucus may require a different approach. Ask a doctor or pharmacist for advice if you are unsure about the type of cough.

Medical assessment is needed: Contact a doctor if difficulty or rapid breathing, fever, purulent or bloody sputum, chest pain, a worsening general condition or a cough lasting longer than seven days develops.

How are Prospan oral drops taken?

Always take this medicine exactly as stated in the official package leaflet or as advised by your doctor or pharmacist.

Adults, older people and adolescents aged 12 years and over:

  • 24 drops three times daily;
  • each dose corresponds to 16.8 mg of dry ivy leaf extract.

Children aged 6 to 11 years:

  • 16 drops three times daily;
  • each dose corresponds to 11.2 mg of dry ivy leaf extract.

Children aged 2 to 5 years:

  • 12 drops three times daily;
  • each dose corresponds to 8.4 mg of dry ivy leaf extract.

Take the drops before meals. For young children, the drops may be given with fruit juice.

Duration of use: Consult a doctor or pharmacist if symptoms last longer than seven days while using the medicine. Do not increase the number of drops or extend treatment on your own.

Use in children

Prospan oral drops are intended for children aged two years and over.

The medicine must not be given to children under two years because secretolytic cough medicines may worsen respiratory symptoms in this age group.

A persistent or recurrent cough in a child aged two to four years requires medical diagnosis before treatment is started.

Because the medicine contains a high concentration of ethanol, measure the child’s dose carefully with the dropper and use it only according to the official instructions or professional advice.

How much alcohol do Prospan drops contain?

Prospan oral drops contain 47% v/v ethanol.

  • Adult dose of 24 drops: 322 mg of alcohol, approximately equivalent to the amount in 8 mL of beer or 3.2 mL of wine.
  • Child dose of 16 drops: 215 mg of alcohol, approximately equivalent to the amount in 5.4 mL of beer or 2.2 mL of wine.
  • Child dose of 12 drops: 161 mg of alcohol, approximately equivalent to the amount in 4 mL of beer or 1.6 mL of wine.

The medicine is harmful to people with alcoholism. Special caution is required in children, people with liver disease or epilepsy, and during pregnancy and breastfeeding.

When must Prospan oral drops not be taken?

Do not take this medicine:

  • if you are allergic to ivy leaf;
  • if you are allergic to other plants in the Araliaceae family;
  • if you are allergic to any of the other ingredients;
  • in children under two years of age;
  • if your doctor has strictly prohibited even the smallest amount of alcohol.

Warnings and precautions

Speak to a doctor or pharmacist before taking the medicine if you have:

  • gastritis;
  • a stomach ulcer;
  • liver disease;
  • epilepsy;
  • a history of alcohol-related problems.

Taking Prospan together with cough suppressants such as codeine or dextromethorphan is not recommended without prior medical advice. Suppressing the cough reflex may make it harder to clear loosened mucus.

Interactions with other medicines

Interaction studies have not been performed and no specific interactions with other medicines are currently known.

Nevertheless, tell your doctor or pharmacist about all medicines you are taking, have recently taken or might take, including medicines obtained without a prescription.

Because of the alcohol content, it is particularly important to mention medicines that affect the central nervous system, epilepsy treatment or other medicines for which alcohol should be avoided.

Pregnancy and breastfeeding

If you are pregnant or breastfeeding, think you may be pregnant or are planning to have a baby, ask a doctor or pharmacist for advice before taking the medicine.

Use of Prospan oral drops during pregnancy and breastfeeding is not recommended because there are insufficient safety data for ivy leaf extract and because the product contains a high concentration of alcohol.

Driving and using machines

No studies have been conducted on the effects of the medicine on the ability to drive or use machines. Take into account that the medicine contains 47% v/v alcohol.

Possible side effects

Like all medicines, Prospan oral drops may cause side effects, although not everybody gets them.

Side effects of unknown frequency include:

  • nausea;
  • vomiting;
  • diarrhoea;
  • hives;
  • skin rash;
  • difficulty breathing;
  • anaphylactic reactions.

Seek emergency medical help: Stop taking the medicine and get immediate help if swelling of the face, lips, tongue or throat, difficulty breathing or swallowing, widespread hives or other signs of a serious allergic reaction occur.

Tell a doctor or pharmacist about any suspected side effect.

What if too much Prospan is taken?

Do not exceed the recommended daily dose.

Taking more than recommended may cause:

  • nausea;
  • vomiting;
  • diarrhoea;
  • restlessness or aggressive behaviour.

Contact a doctor if you or a child have taken more drops than recommended. Because of the high alcohol content, seek advice promptly if a child has swallowed a larger amount.

What if a dose is forgotten?

Do not take a double dose to make up for a forgotten dose. Continue according to the usual schedule or the instructions given by your doctor.

How should Prospan oral drops be stored?

  • Keep out of the sight and reach of children.
  • Store at a temperature of up to 25 °C.
  • After first opening, the medicine may be used for up to six months.
  • Continue to store at up to 25 °C after opening.
  • Do not use after the expiry date stated on the package.

Do not dispose of unused medicines through wastewater or household waste. Return them to a pharmacy that collects unused medicines from the public.

Pharmacist’s advice

Pharmacist’s advice: Prospan oral drops are intended for a productive cough with mucus. Do not automatically use them for a dry cough and do not combine them with medicines that suppress the cough reflex without professional advice.

This Prospan formulation contains a high concentration of alcohol. Carefully count a child’s dose, give it before a meal and, when appropriate, mix it with a small amount of fruit juice. Another pharmaceutical form may be more suitable for some patients, but the choice should be checked with a pharmacist.

Contact a doctor if fever, difficulty breathing or purulent sputum develops, or if the cough lasts longer than seven days.
