What is Orvagil used for and how should it be taken?

Orvagil may be prescribed for:

  • infections caused by anaerobic bacteria;
  • septicaemia, bacteraemia, peritonitis and selected deep infections;
  • pelvic and postoperative wound infections;
  • prevention of selected postoperative infections;
  • urogenital trichomoniasis;
  • bacterial vaginosis;
  • amoebiasis;
  • giardiasis;
  • acute ulcerative gingivitis;
  • acute dental infections;
  • infected leg ulcers and pressure sores involving anaerobic bacteria.

Metronidazole does not treat viral infections and should not be independently used for every digestive, vaginal, dental or urinary symptom.

Administration

  • Swallow the tablet with water without chewing.
  • Take it with food or immediately after food.
  • Use each dose at the prescribed time.
  • Complete the prescribed treatment course.
  • Do not independently increase, reduce or extend treatment.

For confirmed anaerobic bacterial infections, the official information describes an initial adult dose of 800 mg followed by 400 mg every eight hours. Treatment commonly lasts approximately seven days, but the regimen differs significantly for trichomoniasis, bacterial vaginosis, amoebiasis, giardiasis and dental infections.

This information is not a self-dosing guide. The doctor determines the appropriate regimen.

Children require weight- and age-adjusted dosing. A different strength or pharmaceutical form may be required for accurate administration.

If a dose is missed, take it when remembered unless the next dose is near. Do not take a double dose.

Do not drink alcohol

Avoid alcohol during treatment and for at least 48 hours after the final dose.

Combining metronidazole with alcohol may cause nausea, vomiting, abdominal pain, flushing, headache, sweating and palpitations.

Check whether liquid medicines, drops or tinctures contain alcohol.

Important warning

Stop treatment and contact a doctor immediately if tingling, burning, numbness or weakness of the limbs, impaired coordination, confusion, hallucinations or seizures occur.

Prolonged or repeated treatment requires particular caution. Blood tests may be required when treatment lasts longer than ten days.

Patients with Cockayne syndrome have an increased risk of severe liver injury and require specialist monitoring.

Do not take Orvagil:

  • if you are allergic to metronidazole;
  • if you are allergic to tinidazole or another nitroimidazole;
  • if you are allergic to another tablet ingredient.

Tell the doctor about liver disease, kidney disease or dialysis, epilepsy, another neurological condition, a previous blood disorder or a previous serious reaction to metronidazole.

Important interactions

Tell the doctor if you use warfarin or another oral anticoagulant, lithium, phenytoin, phenobarbital, disulfiram, busulfan, ciclosporin, fluorouracil or medicines that affect cardiac rhythm.

Metronidazole may increase the anticoagulant effect of warfarin. Its combination with lithium may increase lithium levels and kidney toxicity. Concomitant disulfiram use may cause confusion or psychotic reactions.

Pregnancy and breastfeeding

Orvagil should be used during pregnancy only when the doctor considers treatment necessary.

Metronidazole passes into breast milk. Treatment during breastfeeding should generally be avoided or temporary interruption of breastfeeding should be discussed with the doctor.

Possible side effects

  • nausea, vomiting and abdominal discomfort;
  • diarrhoea and reduced appetite;
  • an unpleasant or metallic taste;
  • coated tongue;
  • headache and dizziness;
  • drowsiness;
  • rash and itching;
  • dark urine;
  • muscle or joint pain.
Seek urgent medical attention for:
  • swelling of the face, lips, tongue or throat;
  • difficulty breathing or swallowing;
  • a widespread rash, blistering, mucosal sores or peeling skin;
  • yellowing of the skin or eyes;
  • severe abdominal pain radiating to the back;
  • tingling, numbness or weakness of the limbs;
  • confusion, hallucinations, seizures or loss of consciousness;
  • a stiff neck with severe headache and sensitivity to light;
  • unusual bruising, bleeding gums or recurrent infections;
  • sudden changes in vision or hearing.

Storage

  • Store in the original packaging, protected from light and moisture.
  • No special temperature conditions are required.
  • Keep out of the sight and reach of children.
  • Do not use after the expiry date.

Composition: each film-coated tablet contains 400 mg metronidazole.

Pharmacist’s advice

Take Orvagil with or immediately after food and a glass of water. This may reduce nausea and abdominal discomfort.

Do not stop treatment as soon as symptoms improve. Premature discontinuation may result in incomplete treatment and recurrence.

Avoid alcohol during treatment and for at least 48 hours after the final tablet.

When using a bacterial probiotic, an interval of at least two hours from the antibiotic is commonly recommended. Follow the instructions for the particular product.
