Ondasan 4 mg and 8 mg contains the active substance ondansetron. It is an antiemetic belonging to the group of selective serotonin 5-HT3 receptor antagonists.

It is used to prevent and control nausea and vomiting caused by cytotoxic chemotherapy and radiotherapy and to prevent postoperative nausea and vomiting in adults. In children and adolescents, it may be used according to a specialist dosing plan for chemotherapy-induced nausea and vomiting.

Ondasan is not intended for independent treatment of gastroenteritis, food poisoning, motion sickness, alcohol-related nausea or pregnancy sickness.

What is Ondasan used for and how should it be taken?

In adults, Ondasan is indicated for nausea and vomiting caused by chemotherapy or radiotherapy and for the prevention of postoperative nausea and vomiting.

Ondansetron may also be used for chemotherapy-induced nausea and vomiting in children aged six months to 17 years. The tablet formulation is not recommended in children under six years because a more accurately measurable pharmaceutical form may be required.

Adult chemotherapy- and radiotherapy-related regimen

  • 8 mg one to two hours before treatment;
  • a further 8 mg 12 hours later;
  • then 8 mg twice daily;
  • treatment may continue for up to five days according to medical instructions.

For postoperative prevention in adults, the official information describes an oral dose of 16 mg approximately one hour before anaesthesia.

In moderate or severe liver impairment, the total daily dose must not exceed 8 mg.

This information is not a self-dosing guide. The strength, timing and duration are determined by the oncology, surgical or anaesthetic team.

  • Swallow the tablet with water.
  • Follow the prescribed timing precisely.
  • Do not divide the tablet on the basis of the score line.
  • Do not take an additional dose without medical advice.
  • Do not take a double dose after a missed tablet.
Important cardiac warning

Ondansetron may prolong the QT interval in a dose-dependent manner and may rarely cause a serious rhythm disorder, including Torsade de Pointes.

Particular caution is required in congenital long-QT syndrome, heart failure, bradycardia, electrolyte abnormalities and concomitant treatment affecting cardiac rhythm.

Low potassium and magnesium should be corrected before ondansetron is administered.

Do not take Ondasan:

  • if you are allergic to ondansetron or another ingredient;
  • together with apomorphine;
  • during the first trimester of pregnancy;
  • during breastfeeding.

Caution is required in heart disease, arrhythmias, liver impairment, electrolyte disorders, severe constipation, reduced intestinal motility and previous allergy to another 5-HT3 antagonist.

Important interactions

Tell the doctor about antiarrhythmics, beta blockers, erythromycin, ketoconazole, cardiotoxic cancer treatment, phenytoin, carbamazepine, rifampicin, tramadol, methadone, haloperidol and serotonergic antidepressants.

Concomitant use with SSRIs or SNRIs may increase the risk of serotonin syndrome. Symptoms may include agitation, confusion, hallucinations, fever, sweating, tremor, muscle twitching, stiffness and impaired coordination.

Pregnancy and breastfeeding

Ondasan should not be used during the first trimester because ondansetron may slightly increase the risk of cleft lip and/or cleft palate. Pregnancy status should be checked before treatment in women of childbearing potential, and effective contraception should be considered.

Ondasan should not be used during breastfeeding because a risk to the infant cannot be excluded.

Possible side effects

Common effects include headache, flushing, a sensation of warmth, constipation and changes in liver-function tests.

Less common or rare effects include hiccups, hypotension, slow or irregular heartbeat, chest pain, seizures, involuntary movements, severe allergic reactions, QT prolongation, temporary visual disturbance and severe blistering skin reactions.

Seek urgent medical help for:
  • sudden chest pain or tightness;
  • fainting or a markedly abnormal heartbeat;
  • wheezing or difficulty breathing;
  • swelling of the face, lips, mouth or tongue;
  • a widespread rash, blistering or peeling skin;
  • seizures or marked involuntary movements;
  • sudden visual deterioration or temporary loss of vision;
  • features of serotonin syndrome;
  • severe abdominal distension with inability to pass stools or gas;
  • persistent vomiting preventing fluids or essential medicines from being retained.

Storage and composition

  • Store below 25°C in the original packaging, protected from light.
  • Keep out of the sight and reach of children.
  • Each tablet contains 4 mg or 8 mg ondansetron.
  • The tablets contain lactose monohydrate.
Pharmacist’s advice

Take Ondasan at the exact times determined by the oncology, surgical or anaesthetic team. When prescribed preventively, do not wait until vomiting becomes severe.

Monitor vomiting frequency, fluid intake, urination and bowel movements. Severe constipation, marked abdominal swelling or inability to retain fluids requires medical advice.

Check ginger products, electrolyte solutions, vitamins and other nausea remedies with the medical team before use.
