Nitroxolin-MIP forte 250 mg, previously marketed as Nitroxolin forte, is a systemic antibacterial medicine containing nitroxoline. It is prescribed for selected acute and chronic urinary tract infections and for the prevention of recurrent infections caused by microorganisms susceptible to nitroxoline.
The medicine is intended for adults, is available only with a prescription and should not be started independently on the basis of burning, frequent urination or changes in urine alone.
The official name was changed in 2023 from Nitroxolin forte to Nitroxolin-MIP forte. The active substance, 250 mg strength and soft-capsule dosage form remain the same.
Nitroxoline is not appropriate for every urinary symptom. Treatment should be selected following clinical assessment, urine testing and, when required, urine culture with susceptibility testing. Do not use medicine left over from a previous infection or stop the prescribed course when symptoms begin to improve.

What is nitroxoline used for?
- acute urinary tract infections;
- chronic urinary tract infections;
- cystitis;
- urethritis;
- ureteritis;
- prevention of recurrent infections caused by susceptible microorganisms.
The approved indications include urinary infections caused by bacteria and fungi susceptible to nitroxoline. This does not mean that the medicine is suitable for every fungal infection or for vaginal thrush.
How is Nitroxolin-MIP forte taken?
- For an acute infection, adults usually take one capsule three times daily.
- For a chronic infection or recurrence prevention, one or two capsules are usually taken daily.
- Take divided doses according to the schedule prescribed by the doctor.
- When prescribed for prevention of recurrence, the dose may be taken in the evening before bedtime.
Swallow the capsule whole with a full glass of water, preferably before a main meal. Patients with a particularly sensitive stomach may take it one or two hours after food.
The doctor determines treatment duration. Do not reduce the number of doses, extend treatment or stop it independently when symptoms improve.
When is urgent medical assessment required?
- fever, chills or marked weakness;
- pain in the flank, back or side;
- nausea and vomiting;
- visible blood in the urine;
- difficulty or inability to pass urine;
- worsening symptoms despite treatment;
- swelling of the face, tongue or throat, difficulty breathing or severe hives.
Who must not take it?
- patients allergic to nitroxoline;
- patients allergic to soya or peanuts, because the capsule contains soya oil;
- patients hypersensitive to Cochineal Red A – E124;
- patients with severe renal impairment;
- patients with severe hepatic impairment.
Kidney and liver function
Tell the doctor about impaired kidney function. Elimination may be delayed in moderate impairment, while effective urinary concentrations may not be achieved in severe renal impairment.
Liver-function parameters should be monitored regularly during prolonged treatment.
Discolouration of urine
Nitroxoline is eliminated in the urine and may cause marked yellow discolouration. This is an expected treatment effect and does not by itself indicate blood in the urine or kidney damage.
Urine may stain underwear. The stains are generally removed by normal washing, and synthetic underwear should preferably be avoided during treatment.
Interactions with medicines and minerals
No important direct medicine interactions have been observed. High-dose mineral products could theoretically reduce nitroxoline activity.
Antacids or supplements containing larger amounts of magnesium or calcium should be taken at least two to three hours apart from nitroxoline.
Pregnancy and breastfeeding
During pregnancy, nitroxoline may be used only when the doctor concludes, after assessment of all risks and the susceptibility test, that treatment is essential.
Use during breastfeeding is not recommended because there are insufficient data on transfer into breast milk. Effects on the infant’s intestinal flora, diarrhoea, fungal overgrowth or sensitisation are possible.
Driving and using machines
Nitroxoline generally has no effect on concentration or reaction ability. Very rarely, dizziness and unsteadiness while walking may occur. Do not drive or operate machinery if affected.
Possible side effects
Common effects include nausea, vomiting and diarrhoea.
Uncommon effects include skin redness, itching and other allergic reactions. Soya oil and the colour E124 may cause severe allergic reactions in susceptible patients.
A reduction in blood platelets may occur rarely. Fatigue, headache, dizziness and unsteady gait are very rare.
Rare excretion in sweat may cause harmless and temporary yellow discolouration of the skin, hair or nails. Temporary yellow discolouration of the whites of the eyes is very rare.
Missed dose, stopping treatment and overdose
Continue according to the usual schedule after a missed dose. Do not take a double dose.
Stopping treatment may allow the infection to return. Contact the doctor if treatment has been interrupted for more than one day or symptoms recur.
Contact a doctor or healthcare facility after taking a substantially larger amount than prescribed.
When requested by the doctor, a urine culture is ideally obtained before antimicrobial treatment begins. Take every dose regularly and follow the doctor’s advice regarding fluid intake, particularly if heart or kidney disease requires fluid restriction. Cranberry, D-mannose, herbal products and probiotics do not replace prescribed treatment. Check the timing carefully when using antacids, calcium or magnesium. Yellow urine may be expected with nitroxoline, whereas blood, fever, severe pain or flank pain require assessment.
