Nifelat 20 mg contains nifedipine, a calcium-channel blocker from the dihydropyridine group. It is supplied as a prolonged-release tablet that releases the active substance gradually over an extended period.
Nifelat is prescribed for adults for the treatment of mild to moderate hypertension and for the prevention of attacks of chronic stable angina pectoris. It is a prescription-only medicine.
Nifelat does not provide rapid relief from an acute angina attack. Sudden, severe or new chest pain, pressure, shortness of breath, cold sweating, nausea or pain spreading to the arm, back or jaw requires immediate action according to the emergency plan provided by the doctor and urgent medical assessment.

How does Nifelat work?
Nifedipine reduces calcium entry into vascular smooth-muscle cells. This relaxes and widens blood vessels, reduces peripheral resistance and lowers blood pressure.
In stable angina, nifedipine widens coronary and peripheral arteries, improves the supply of blood and oxygen to the heart muscle and reduces cardiac workload. It can reduce the frequency and severity of stable angina attacks but does not provide immediate relief from acute chest pain.
How is Nifelat taken?
- The usual maintenance dose is one 20 mg tablet every 12 hours.
- The doctor may gradually adjust the dose according to blood-pressure control, angina symptoms and tolerability.
- The maximum dose is 40 mg every 12 hours, providing 80 mg daily.
- Older patients and patients with impaired liver function may require a lower dose.
- The medicine may be taken with or without food.
Swallow the tablet whole with a glass of water. Do not break, crush or chew it. The score line is not intended for dividing the tablet.
In selected cases, the doctor may prescribe an initial nifedipine dose of 10 mg every 12 hours. A Nifelat 20 mg tablet must not be divided to provide a 10 mg dose. A suitable formulation or another dosing plan is required.
Grapefruit and Nifelat
Do not eat grapefruit or drink grapefruit juice during treatment. Grapefruit may increase blood concentrations of nifedipine and increase the risk of excessive blood-pressure reduction, headache, flushing, dizziness and other adverse effects.
The interaction may persist for at least three days after grapefruit was consumed. Contact the doctor or pharmacist if grapefruit was consumed during treatment and do not change the prescribed dose independently.
Who must not take Nifelat?
- patients allergic to nifedipine, another dihydropyridine or an ingredient of the tablet;
- patients during the first month after myocardial infarction;
- patients with unstable angina;
- patients with cardiogenic shock;
- patients with clinically significant aortic stenosis;
- patients requiring immediate treatment of an acute angina attack;
- patients using nifedipine for secondary prevention after a previous myocardial infarction;
- patients taking rifampicin;
- patients with malignant hypertension, because safety in this indication has not been established.
When is additional caution required?
Tell the doctor before treatment if you have:
- very low blood pressure;
- heart failure or poor cardiac reserve;
- impaired liver function;
- diabetes;
- dialysis treatment;
- a tendency to faint or experience marked dizziness;
- planned assisted-reproduction treatment;
- pregnancy or plans to become pregnant;
- a need to continue breastfeeding.
Patients receiving dialysis, particularly those with very high blood pressure and reduced circulating blood volume, may experience a substantial fall in blood pressure.
Calcium-channel blockers such as nifedipine have been associated with reversible changes in sperm function. This may be relevant to men undergoing assisted-reproduction treatment.
Interactions with other medicines
Nifedipine is metabolised by CYP3A4. Several medicines can therefore increase or reduce its concentration and effect. Tell the doctor about all prescription medicines, non-prescription products and herbal preparations.
Particular caution is required with:
- other blood-pressure medicines;
- beta blockers, diltiazem, digoxin or quinidine;
- rifampicin – this combination is contraindicated;
- erythromycin and other macrolide antibiotics;
- ketoconazole, itraconazole or fluconazole;
- ritonavir and other HIV-protease inhibitors;
- phenytoin, carbamazepine, phenobarbital or valproic acid;
- fluoxetine or nefazodone;
- cimetidine;
- tacrolimus;
- quinupristin with dalfopristin;
- intravenous magnesium sulphate during pregnancy.
Combining nifedipine with other antihypertensive medicines may further lower blood pressure. Additional monitoring or dosage adjustment may be required.
Pregnancy and breastfeeding
Nifelat is not routinely used during pregnancy. It may be considered for severe hypertension when standard treatment has failed or cannot be used, following careful medical assessment of benefit and risk.
Nifedipine passes into breast milk. Nifelat is not recommended during breastfeeding, and the doctor may advise stopping breastfeeding or selecting a different treatment.
Driving and using machines
Nifelat may cause dizziness, light-headedness, fainting, marked tiredness or visual disturbance. These effects are more likely at the beginning of treatment, after a dosage change or with alcohol.
Do not drive or operate machinery until you know how the medicine affects you.
Possible side effects
Common side effects include:
- headache;
- vasodilation and flushing or warmth;
- a general feeling of being unwell;
- constipation;
- swelling, particularly around the ankles and lower legs.
Uncommon effects may include:
- low blood pressure, dizziness or fainting;
- palpitations or a rapid heartbeat;
- abdominal pain, nausea, indigestion and flatulence;
- dry mouth;
- muscle cramps and swollen joints;
- sleep disturbance and anxiety;
- nasal congestion or nosebleeds;
- migraine, tremor or visual disturbance;
- frequent or difficult urination;
- erectile dysfunction;
- temporary increases in liver enzymes.
Gum enlargement may occur rarely. Careful oral hygiene and regular dental assessment may be particularly important during long-term treatment.
- swelling of the face, lips, tongue or throat;
- difficulty breathing or choking;
- a severe rash, hives or sudden fall in blood pressure;
- blisters, peeling skin or sores on mucous membranes;
- new or worsening chest pain;
- yellowing of the skin or eyes;
- marked faintness or loss of consciousness.
Missed dose and stopping treatment
Take the usual dose when remembered, then wait at least 12 hours before the next dose. Do not take two doses together.
Nifelat may be part of long-term treatment. Do not stop it or change the dose without consulting the doctor, even when blood-pressure readings are normal or angina symptoms are absent.
Overdose
An overdose may cause very low blood pressure, a fast or slow heartbeat, impaired consciousness, high blood glucose, metabolic acidosis, reduced oxygen levels, cardiogenic shock and fluid accumulation in the lungs.
Seek emergency medical help immediately after taking more than the prescribed amount. Because this is a prolonged-release formulation, symptoms may develop over an extended period and medical observation may be required even when the patient initially feels well.
Measure blood pressure at approximately the same time of day after resting for several minutes, and record the readings for the doctor. Ankle swelling can occur with nifedipine, but do not independently start a diuretic or stop treatment. Report marked or increasing swelling, sudden weight gain, breathing difficulty, fainting or worsening chest pain. Always swallow the tablet whole and completely avoid grapefruit during treatment.

Composition, appearance and packaging
Each prolonged-release tablet contains 20 mg of nifedipine.
The tablet core contains lactose monohydrate, maize starch, colloidal anhydrous silica, povidone K30, microcrystalline cellulose and magnesium stearate.
The coating contains hypromellose, macrogol 6000, titanium dioxide E171 and red iron oxide E172.
The medicine contains lactose. Patients with rare hereditary galactose intolerance, lactase deficiency or glucose-galactose malabsorption should consult the doctor.
The tablets are round, biconvex and pink, with a score line that is not intended for dividing the tablet. Each pack contains three blisters of 10 tablets.

Frequently asked questions
Is Nifelat used for high blood pressure?
Yes. It is prescribed for mild to moderate hypertension in adults.
Does it provide immediate relief from an angina attack?
No. It helps prevent chronic stable angina attacks but is not a rapid-relief medicine for acute chest pain.
Can the tablet be divided?
No. The score line is not intended for division, and the prolonged-release tablet must be swallowed whole.
Is it taken with food?
It may be taken with or without food and should be swallowed with water.
Can grapefruit juice be consumed?
No. Grapefruit can increase nifedipine concentrations, and its effect may persist for at least three days.
Can Nifelat cause swollen ankles?
Yes. Ankle and lower-leg swelling is a common adverse effect and should be reported when marked or worsening.
Can children use Nifelat?
It is not recommended for patients younger than 18 because safety and efficacy data are limited.
Can treatment be stopped when blood pressure becomes normal?
No. Normal readings often indicate that treatment is working. Only the doctor should adjust or stop it.
