Nexium 20 mg and 40 mg contains esomeprazole, a proton-pump inhibitor that reduces the amount of acid produced by the stomach.

It is prescribed for gastro-oesophageal reflux disease – GERD, erosive reflux oesophagitis, ulcers associated with Helicobacter pylori, ulcers caused by non-steroidal anti-inflammatory medicines and other conditions involving excessive gastric-acid secretion.

Registered packs contain 7 or 14 gastro-resistant tablets in strengths of 20 mg or 40 mg. The medicine is prescription only in Serbia.

Important warning
Esomeprazole may relieve symptoms of a more serious stomach or oesophageal condition and delay diagnosis.

Contact a doctor promptly for unexplained weight loss, difficulty swallowing, persistent vomiting, vomiting blood, severe abdominal pain or black stools.

Nexium – indications, dosage and precautions

Approved uses

  • treatment of erosive reflux oesophagitis;
  • long-term prevention of relapse after the oesophagus has healed;
  • symptomatic treatment of GERD;
  • eradication of Helicobacter pylori together with prescribed antibiotics;
  • treatment and prevention of ulcers associated with NSAID therapy;
  • extended treatment after intravenous prevention of recurrent peptic-ulcer bleeding;
  • Zollinger–Ellison syndrome and other conditions involving excessive gastric-acid secretion.

In adolescents aged 12 years and over, the tablets are used for GERD and, under specialist supervision, as part of antibiotic treatment for an H. pylori-associated duodenal ulcer.

Common adult dosage schedules

  • Erosive reflux oesophagitis: 40 mg once daily for 4 weeks; a further 4 weeks may be prescribed.
  • Prevention of relapse: 20 mg once daily.
  • Symptomatic GERD without oesophagitis: 20 mg once daily.
  • On-demand GERD treatment: up to 20 mg once daily when approved by the doctor.
  • H. pylori treatment: 20 mg twice daily together with prescribed antibiotics, commonly for 7 days.
  • NSAID-associated gastric ulcer: 20 mg once daily for 4–8 weeks.
  • Prevention of NSAID-associated ulcers: 20 mg once daily in patients at risk.
  • After intravenous prevention of recurrent bleeding: 40 mg once daily for 4 weeks.
  • Zollinger–Ellison syndrome: an initial dose of 40 mg twice daily, followed by individual dose adjustment.

The strength, duration and number of daily doses must not be changed without medical advice.

How to take Nexium

  • It may be taken with food or on an empty stomach.
  • Swallow the tablet whole with sufficient water.
  • Do not chew, crush or grind the tablet.

For swallowing difficulties, disperse the tablet in approximately half a glass of still water. Stir until the tablet breaks apart and drink the liquid with the pellets immediately or within 30 minutes.

Rinse the glass with more water and drink it. Do not chew or crush the pellets and do not use milk, juice or carbonated water.

Do not use Nexium

  • if allergic to esomeprazole or an excipient;
  • if allergic to another proton-pump inhibitor;
  • together with nelfinavir.

Long-term treatment

Patients receiving long-term treatment, particularly for more than one year, require regular medical review.

Long-term acid suppression may be associated with reduced vitamin B12 absorption, low blood magnesium, a small increase in fracture risk and a slightly increased risk of certain gastrointestinal infections.

Important interactions

Tell the prescriber about nelfinavir, atazanavir, clopidogrel, warfarin, digoxin, phenytoin, diazepam, cilostazol, tacrolimus, methotrexate, rifampicin, St John’s wort, azole antifungals and medicines whose absorption depends on gastric acidity.

Pregnancy and breastfeeding

Use during pregnancy requires an individual medical assessment. The current Serbian leaflet advises against use while breastfeeding.

Possible side effects

Common effects include headache, abdominal pain, diarrhoea, constipation, flatulence, nausea and vomiting.

Other effects may include dizziness, sleep disturbance, dry mouth, rash, itching, ankle swelling and changes in liver-test results.

Stop treatment and seek urgent medical help for:
  • swelling of the face, lips, tongue or throat;
  • difficulty breathing or swallowing;
  • fainting or a serious allergic reaction;
  • a widespread rash, blisters, mucosal sores or peeling skin;
  • yellow skin or eyes and dark urine;
  • vomiting blood or black stools;
  • markedly reduced urination or severe flank pain.

Pharmacist’s advice
Nexium is not an antacid that immediately neutralises acid already present in the stomach. It reduces the production of new acid and should be taken regularly according to the prescribed schedule.

Do not crush or chew the tablet. Persistent symptoms despite regular treatment require a review of administration, interactions and possible warning symptoms rather than an independent dose increase.

For goods ordered up to 5.000,00 RSD, delivery charge is 260.00 RSD (VAT included). For goods over 5.000,00 RSD, shipping is free of charge.

For international delivery:

For the export of goods abroad through the PostExport service of Post of Serbia, the shipping price is for:

  • Zone I (Slovenia, Croatia, Bosnia and Herzegovina, Montenegro, Northern Macedonia) - 1,280.00 RSD
  • Zone II (Other European countries) - 1,790.00 RSD

For export of goods abroad through DHL service, the shipping price is 2,700.00 RSD. Delivery time is 1-5 business days (depending on the country of delivery)
