Prilinda Plus is a prescription medicine containing two active ingredients – ramipril and hydrochlorothiazide. It is available as 2.5 mg/12.5 mg and 5 mg/25 mg tablets, with 28 tablets in each pack.

Ramipril is an ACE inhibitor that relaxes and widens blood vessels. Hydrochlorothiazide is a thiazide diuretic that increases the removal of water and salts in the urine. Together, these actions help lower blood pressure.

The fixed combination is used in adults whose blood pressure has not been adequately controlled by ramipril or hydrochlorothiazide used separately.

Important warning
Do not start, change or stop Prilinda Plus without medical instruction. The selected strength depends on previous treatment, blood-pressure readings, kidney and liver function and blood-electrolyte levels.

Swelling of the lips, tongue, face or throat and difficulty breathing or swallowing may indicate angioedema. Stop treatment and seek urgent medical help.

Prilinda Plus – administration, dosage and major precautions

What is Prilinda Plus used for?

Prilinda Plus is used for arterial hypertension when treatment with either ramipril or hydrochlorothiazide alone has not provided adequate blood-pressure control.

It is normally introduced after the prescriber has assessed the dose requirements of the individual components. It is not a medicine to take occasionally only when a reading is high.

How is it taken?

  • Take the prescribed tablet once daily, usually in the morning.
  • Take it at approximately the same time each day.
  • It may be taken with or without food.
  • Swallow with sufficient liquid.
  • Do not chew or divide the tablet to produce a smaller dose.
  • Do not take a double dose after a missed dose.

The score line is intended only to make the tablet easier to break for swallowing. It does not provide two equal doses.

Older patients and patients with impaired kidney or liver function may require a lower dose and closer monitoring. Use is not recommended in patients under 18 years.

When must the medicine not be used?

  • allergy to ramipril, hydrochlorothiazide, another ACE inhibitor, thiazide, sulfonamide or excipient;
  • a previous episode of angioedema;
  • combined use with sacubitril/valsartan;
  • severe kidney impairment with creatinine clearance below 30 mL/min;
  • significant renal-artery narrowing;
  • severe liver impairment or hepatic encephalopathy;
  • a clinically significant sodium, potassium or calcium disturbance;
  • the second or third trimester of pregnancy;
  • breastfeeding;
  • aliskiren use in patients with diabetes or impaired kidney function.

A gap of at least 36 hours is required when switching between Prilinda Plus and sacubitril/valsartan.

Monitoring

The doctor may monitor blood pressure, kidney function, creatinine, potassium, sodium, magnesium, calcium, glucose, uric acid, lipids and blood-cell counts.

Ramipril may increase potassium, while hydrochlorothiazide may decrease it. Do not take potassium supplements, potassium-containing salt substitutes, magnesium, herbal diuretics or “water-loss” products without professional approval.

Dehydration and low blood pressure

Vomiting, diarrhoea, excessive sweating, a very low-salt diet, reduced fluid intake or another diuretic may increase the risk of hypotension and kidney impairment.

Contact a doctor if prolonged vomiting or diarrhoea occurs. Do not change the treatment plan without individual instructions.

Important interactions

  • NSAIDs such as ibuprofen and diclofenac may reduce the blood-pressure effect and increase kidney risk.
  • Potassium supplements, potassium-sparing diuretics, trimethoprim, ciclosporin and tacrolimus may increase potassium.
  • Other diuretics, corticosteroids, stimulant laxatives and amphotericin B may reduce potassium.
  • Lithium toxicity may increase.
  • Diabetes medicines may require dose adjustment.
  • Other antihypertensives, nitrates, anaesthetics, certain antidepressants and alcohol may cause excessive hypotension.
  • Heart-rhythm medicines and digoxin require caution when electrolytes change.

Pregnancy and breastfeeding

Women planning pregnancy should be transferred to an appropriate alternative treatment. Use is not recommended during the first trimester and is contraindicated after the third month of pregnancy.

Contact the prescriber immediately if pregnancy occurs. Prilinda Plus must not be used during breastfeeding.

Possible side effects

Common effects include headache, weakness, tiredness, dizziness, dry irritating cough, bronchitis and increases in glucose, uric acid or blood lipids.

Low blood pressure, fainting, nausea, abdominal discomfort, increased urination, thirst, electrolyte changes, kidney impairment, rash, palpitations and changes in blood-cell counts may also occur.

Stop treatment and seek urgent medical help for:
  • swelling of the face, lips, tongue or throat;
  • difficulty breathing or swallowing;
  • blistering, peeling skin or mouth ulcers;
  • severe abdominal pain spreading to the back;
  • yellow skin or eyes;
  • sudden visual reduction or severe eye pain;
  • shallow breathing, fever, weakness and confusion;
  • fainting, severe hypotension or an abnormal heartbeat;
  • markedly reduced urine production;
  • fever, sore throat, repeated infections, bruising or unusual bleeding.

Pharmacist’s advice
Take Prilinda Plus every day at the same time, normally in the morning. Do not skip it simply because the current blood-pressure reading is normal.

Do not introduce potassium supplements, salt substitutes containing potassium, herbal diuretics or electrolyte products without checking the complete treatment and laboratory results.
