Lantus SoloStar contains insulin glargine, a modified insulin very similar to human insulin, with a long-lasting and steady effect. It is used to treat diabetes mellitus in adults, adolescents and children aged 2 years and older.
One millilitre of solution contains 100 units of insulin glargine (3.64 mg); one pre-filled SoloStar pen contains 3 mL, i.e. 300 units. The medicine is injected under the skin once daily, at the same time every day, alone or combined with a short-acting insulin or diabetes tablets, as decided by the doctor.
Important warning
- Dispensing status: prescription-only medicine (R).
- Inject under the skin only – never into a vein. The pen is for personal use only and must not be shared with anyone, even if the needle is changed.
- The most important risk is hypoglycaemia (blood sugar too low) – always carry at least 20 g of sugar.
- Lantus (100 units/mL) and Toujeo (insulin glargine 300 units/mL) are not interchangeable; any switch from another insulin must be done only under medical supervision.
- This page is for information only and is not an offer to buy the medicine online.

What is Lantus SoloStar used for?
- treatment of diabetes mellitus in adults, adolescents and children aged 2 years and above;
- in type 2 diabetes the medicine may also be given together with oral antidiabetic medicines.
How is Lantus SoloStar used?
- Dose: the doctor decides how many units you need and when, based on your lifestyle, blood sugar results and previous experience with insulin. Lantus units apply only to Lantus and are not the same as units of other insulins.
- Frequency: one injection a day, at the same time every day.
- Method: subcutaneously, in the body area recommended by the doctor; change the injection site with each injection within the chosen area. Do not shake or mix – the solution must be clear, colourless and water-like.
- SoloStar pen: attach a new needle approved for SoloStar before each use and perform a safety test. The dose can be set from 1 to 80 units in steps of 1 unit. Empty pens must not be refilled.
- Switching from other insulins (SmPC): when switching from twice-daily NPH insulin the basal insulin dose is usually reduced by 20–30% in the first weeks; when switching from insulin glargine 300 units/mL by about 20% – always as instructed by the doctor and with careful glucose monitoring.
- Check the insulin label before each injection to avoid mix-ups with other insulins.
Special patient groups
- Children and adolescents: used from 2 years of age; there are no data in children under 2. Injection-site and skin reactions are relatively more frequent than in adults.
- Elderly (65+): progressive decline in kidney function may reduce insulin requirements.
- Kidney or liver impairment: insulin requirements may be reduced – the doctor adjusts the dose.
Contraindications – when the medicine must not be used
- allergy (hypersensitivity) to insulin glargine or to any of the excipients.
Warnings and precautions
- Follow your doctor’s instructions on dose, blood (and urine) glucose monitoring, diet and physical activity carefully.
- Skin changes: injecting into the same spot can cause thickening or thinning of the skin and lumps (including cutaneous amyloidosis); insulin works less well in such an area – consult your doctor before changing the site.
- Illness, injury, insufficient food intake: blood sugar may rise or fall sharply; in type 1 diabetes never stop insulin and continue to eat enough carbohydrates.
- Travelling: discuss insulin supply and storage, time zones and meal schedules with your doctor in advance.
- Heart failure has been reported in patients with long-standing type 2 diabetes and heart disease treated with pioglitazone and insulin – contact your doctor if you notice shortness of breath, rapid weight gain or swelling.
- Always use a new pen if your blood sugar worsens unexpectedly or the pen is damaged.
Interactions with other medicines, food and drink
- May lower blood sugar (hypoglycaemia): other diabetes medicines, ACE inhibitors, disopyramide, fluoxetine, fibrates, MAO inhibitors, pentoxifylline, propoxyphene, salicylates (e.g. acetylsalicylic acid), sulphonamide antibiotics.
- May raise blood sugar: corticosteroids, danazol, diazoxide, diuretics, glucagon, isoniazid, oestrogens and progestogens (e.g. contraceptive pills), phenothiazines, somatropin, sympathomimetics (adrenaline, salbutamol, terbutaline), thyroid hormones, atypical antipsychotics (olanzapine, clozapine), protease inhibitors.
- May either lower or raise blood sugar: beta-blockers, clonidine, lithium salts; pentamidine may cause hypoglycaemia sometimes followed by hyperglycaemia.
- Beta-blockers and other sympatholytics (clonidine, guanethidine, reserpine) may weaken or mask the warning signs of hypoglycaemia.
- Alcohol may raise or lower blood sugar.
Pregnancy and breast-feeding
- Pregnancy: tell your doctor if you are planning a pregnancy or are pregnant; the insulin dose may change during pregnancy and after delivery. Data from more than 1,000 pregnancies do not indicate specific adverse effects; use may be considered if clinically needed (SmPC). Good glucose control and avoiding hypoglycaemia are especially important.
- Breast-feeding: consult your doctor – the insulin dose and diet may need adjustment.
Driving and using machines
- Concentration and reactions may be impaired during hypoglycaemia, hyperglycaemia or visual problems. Ask your doctor whether you may drive if you have frequent hypoglycaemia or weak warning signs.
Possible side effects
- Very common: hypoglycaemia – may be very serious, with loss of consciousness.
- Common: injection-site reactions (redness, unusually intense pain, itching, rash, swelling, inflammation); thickening of the skin at the injection site (lipohypertrophy).
- Uncommon: thinning of the skin at the injection site (lipoatrophy).
- Rare: severe allergic reactions; temporary worsening of vision with marked changes in glucose control (in proliferative retinopathy severe hypoglycaemia may cause temporary loss of vision); water retention with swelling of the calves and ankles.
- Very rare: taste disturbance, muscle pain.
- Not known: cutaneous amyloidosis (lumps under the skin).
Overdose, missed dose and stopping treatment
- Too much insulin: blood sugar may fall too low – check it often, eat more and follow the hypoglycaemia instructions.
- Missed dose: blood sugar may rise too high – check it often. Do not inject a double dose to make up for a missed one.
- Stopping treatment: may lead to severe hyperglycaemia and ketoacidosis. Do not stop without talking to your doctor.
- What to do in hypoglycaemia (PIL): do not inject insulin; immediately take 10–20 g of sugar (glucose, sugar cubes, a sugared drink – artificial sweeteners do not help), then something with a longer effect (bread, pasta). If it recurs, take another 10–20 g of sugar. Recovery may take time because Lantus acts for a long time.
When to seek medical help immediately?
- signs of severe hypoglycaemia: confusion, speech or visual disturbances, convulsions, loss of consciousness (people around you should know that an injection of glucose or glucagon is then needed);
- hypoglycaemia you cannot control yourself or that keeps recurring;
- signs of ketoacidosis: thirst, frequent urination, abdominal pain, rapid deep breathing, drowsiness or loss of consciousness;
- widespread rash and itching over the whole body, severe swelling of skin or mucous membranes, shortness of breath, drop in blood pressure with fast heartbeat and sweating (severe allergic reaction);
- rapid weight gain, swelling or unusual shortness of breath (possible heart failure).
Pharmacist's advice
Make yourself a “map” of injection sites and move the needle a few centimetres with every injection – this helps avoid lumps under the skin where insulin works less well. Keep the pen in use at room temperature (up to 30 °C) for no more than 4 weeks, and spare pens in the fridge.
Always carry a source of fast-acting sugar (at least 20 g) and check your blood sugar regularly, especially when your diet, physical activity, health or other medicines change. Teach your family the signs of hypoglycaemia and what to do if you lose consciousness.
