Isocard prolonged-release tablets contain isosorbide mononitrate, a medicine from the group of organic nitrates. It relaxes and widens the blood vessels, making it easier for the heart to pump blood around the body.

It is used to prevent (prophylaxis) attacks of angina pectoris, which presents as tightness and pain in the chest. The 60 mg tablet releases the medicine gradually over several hours, so it is taken once daily, in the morning.

Important warning

  • The medicine is available only on prescription.
  • Isocard is not a medicine for an acute attack of chest pain – for a sudden angina attack glyceryl trinitrate is used as instructed by your doctor.
  • Isocard must not be combined with sildenafil or similar medicines for erectile dysfunction (phosphodiesterase type 5 inhibitors) – a dangerous drop in blood pressure is possible.
  • Do not drink alcohol during treatment – it increases the risk of low blood pressure.
  • This page is for information only and is not an offer to buy the medicine online.

What is Isocard used for?

Isocard is used to prevent attacks of angina pectoris (chest tightness and pain). The medicine is taken regularly to reduce the frequency of attacks, not to stop an attack that has already started.

How is Isocard taken and dosed?

  • The recommended dose is one tablet (60 mg) once daily, in the morning.
  • The maximum recommended dose is 120 mg (2 tablets) daily, with both tablets taken together.
  • Your doctor may recommend starting with 30 mg (half a tablet) for the first days to reduce the likelihood of headache.
  • Swallow the tablet whole or halved, with half a glass of water. The tablets must not be crushed or chewed.
  • Remnants of the tablet are sometimes visible in the stool – this is normal and does not mean the medicine has not been released in the body.

Special patient groups

  • Children: the medicine is not used in children.
  • Older people: alcohol increases the risk of low blood pressure especially in older people.
  • Severe liver or kidney disease: caution is needed – tell your doctor.
  • Unstable circulation: nitrates may cause collapse after the first dose; this can largely be avoided by starting treatment with 30 mg.

Contraindications – when the medicine must not be used

  • allergy to isosorbide mononitrate, other nitrates (glyceryl trinitrate, isosorbide dinitrate) or any of the excipients;
  • a history of stroke;
  • very low blood pressure, marked anaemia, reduced blood volume (hypovolaemia);
  • serious heart damage such as a heart attack;
  • cardiomyopathy, heart failure, pericarditis or damaged heart valves;
  • a head injury that may cause bleeding in the brain;
  • concomitant use of sildenafil or other phosphodiesterase type 5 inhibitors.

Warnings and precautions

  • very low body temperature (hypothermia);
  • malnutrition;
  • underactive thyroid (hypothyroidism);
  • severe liver or kidney disease;
  • the medicine contains lactose – if you have an intolerance to some sugars, talk to your doctor.

Interactions with other medicines, food and drink

  • Contraindicated: sildenafil and similar medicines (phosphodiesterase type 5 inhibitors) for impotence.
  • Medicines for high blood pressure and the heart (ACE inhibitors, beta blockers, calcium channel blockers) – may increase the drop in blood pressure.
  • Alcohol – increased risk of low blood pressure; do not drink alcohol during treatment.

Pregnancy and breast-feeding

Isocard should not be used during pregnancy or breast-feeding unless your doctor considers it necessary.

Driving and using machines

If you feel dizzy after taking the medicine, do not drive or operate machinery.

Possible side effects

At the start of treatment headache, nausea and dizziness may occur; they usually pass over time.

Common (up to 1 in 10): low blood pressure with weakness or dizziness, fast heartbeat, headache, dizziness, nausea. Uncommon (up to 1 in 100): vomiting, diarrhoea. Rare (up to 1 in 1,000): loss of consciousness, rash, itching. Very rare (up to 1 in 10,000): muscle pain.

Overdose, missed dose and stopping treatment

  • Overdose: tell your doctor immediately or go to the emergency department and take the pack with you. A throbbing headache is most common; more serious symptoms are excitation, flushing, cold sweat, nausea, vomiting, vertigo, loss of consciousness, fast heartbeat and low blood pressure.
  • Missed dose: take it as soon as you remember; if it is nearly time for the next dose, skip the missed one. Do not take a double dose.
  • Stopping treatment: do not stop treatment without first consulting your doctor.

When to seek medical help immediately?

  • chest pain that does not pass after using glyceryl trinitrate as instructed by your doctor;
  • loss of consciousness or severe dizziness with very low blood pressure;
  • signs of overdose: severe throbbing headache, cold sweat, fast heartbeat, fainting.

Pharmacist's advice

Take Isocard in the morning, at the same time every day. Headache is common in the first days and usually passes; do not increase the dose yourself. Always carry the medicine for acute attacks prescribed by your doctor (glyceryl trinitrate).

Never combine Isocard with medicines for erectile dysfunction (sildenafil and similar) and avoid alcohol. If you feel dizzy, get up slowly; measuring your blood pressure at home helps your doctor monitor treatment.

Frequently asked questions

Can I take Isocard when I get chest pain?

No. Isocard is for preventing angina attacks and does not act quickly enough for an acute attack. For sudden chest pain glyceryl trinitrate (sublingual tablets or spray) is used as instructed by your doctor.

Why does Isocard give me a headache?

Headache is a common side effect of nitrates and usually passes over time. That is why your doctor may recommend taking half a tablet (30 mg) for the first days.

Can I split the tablet?

Yes. The tablet has a score line and can be divided into equal doses, but it must not be crushed or chewed, as that would destroy the prolonged release.

Why must I not take sildenafil with Isocard?

The combination of nitrates and phosphodiesterase type 5 inhibitors (e.g. sildenafil) can cause a dangerous drop in blood pressure, so concomitant use is contraindicated.
