Cinnarizine (Serbian: cinarizin) is the name of an active substance from the group of anti-vertigo medicines. It works in two ways: it blocks calcium entry into the smooth muscle cells of blood vessels, preventing them from contracting, and it has an antihistamine effect. This is why it calms symptoms of inner ear and balance (vestibular) disorders and helps in some peripheral circulation disorders.

Cinnarizine is not a brand name. In Serbia one cinnarizine medicine is currently registered – Cinedil 75 mg tablets (Alkaloid), available on prescription. The information on this page therefore comes from the official leaflet and summary of product characteristics of that medicine.

Important warning

  • Supply status in Serbia: the cinnarizine medicine (Cinedil 75 mg) is prescription-only.

  • Must not be taken with extrapyramidal symptoms (movement disorders), after a recent heart attack, during pregnancy or while breastfeeding.
  • The 75 mg tablets are not recommended for children.
  • This page is for information only and is not an offer to buy the medicine online.

Which complaints is cinnarizine used for?

  • vestibular disorders – vertigo, ringing in the ears (tinnitus), nausea and vomiting, similar to those seen in Ménière’s disease;
  • adjunctive treatment of symptoms of peripheral circulation disorders: Raynaud’s syndrome (pale, cold fingers due to vessel spasm), intermittent claudication (leg pain when walking) and cold extremities in vasospastic disorders.

Vertigo and ringing in the ears can have different causes – the cause should be established by a doctor before treatment.

Dosing of cinnarizine (Cinedil 75 mg)

The medicine is taken by mouth by adults and older people. Doses for older people are the same as for adults.

PurposeDose according to the leaflet
Vestibular disorders – initial75 mg (1 tablet) once or twice daily
Peripheral circulation disorders – initial75 mg (1 tablet) two to three times daily
Maintenance75 mg (1 tablet) once daily
Maximum daily225 mg (3 tablets)

Taking it after meals may reduce stomach discomfort. This is general dosing – the dose and duration are set by your doctor.

Special patient groups

  • Children: the 75 mg tablets are not recommended for children.
  • Older people: a fall in body temperature (hypothermia) and movement disorders (extrapyramidal symptoms) may occur.
  • Kidney or liver failure: talk to your doctor before use.

Contraindications – when cinnarizine must not be used

  • allergy to cinnarizine or the excipients (rash, itching, swelling of the face or lips, shortness of breath);
  • extrapyramidal symptoms (tremor, stiffness, involuntary movements);
  • a recent heart attack;
  • pregnancy and breastfeeding.

Precautions and warnings

  • Talk to your doctor if you have kidney or liver failure, porphyria, Parkinson’s disease or low blood pressure.
  • Cinnarizine may give false-negative results in allergy skin tests – stop the medicine at least 4 days before testing.
  • Contains less than 1 mmol sodium per dose, i.e. essentially “sodium-free”.

Interactions with other medicines and alcohol

  • Anti-anxiety medicines, sleeping pills and certain antidepressants: increased drowsiness.
  • Alcohol: reduces psychophysical abilities – avoid during treatment.
  • Medicines that can cause extrapyramidal syndrome (neuroleptics, metoclopramide, reserpine, methyldopa): do not take at the same time.
  • Other calcium channel blockers: cinnarizine increases their effect; vasodilator medicines increase the vasoactive effect of cinnarizine.
  • Domperidone: synergistic effect.

Pregnancy and breastfeeding

According to the contraindications in the leaflet, cinnarizine must not be taken during pregnancy or while breastfeeding. If you are pregnant, think you may be pregnant or are breastfeeding, ask your doctor before any treatment.

Driving and using machines

Especially at the start of treatment cinnarizine may cause drowsiness and prolong reaction time, which is important for drivers and machine operators.

Possible side effects

  • Common: drowsiness, nausea, weight gain, indigestion.
  • Uncommon: marked lethargy and sleepiness, tiredness, upper abdominal pain, vomiting, increased sweating, rash.
  • Frequency not known: abnormal movements (trembling of hands and fingers, twisting of the body, difficulty walking, muscle stiffness), jaundice, a rash known as subacute cutaneous lupus erythematosus.

Report suspected side effects to your doctor, pharmacist or ALIMS.

Overdose, missed dose and stopping treatment

  • Overdose: contact a doctor or emergency service immediately. Vomiting, drowsiness up to impaired consciousness and coma, hypotonia and movement disorders may occur; according to the SmPC, symptoms are mainly due to the anticholinergic effect, and seizures have been reported in small children. There is no specific antidote.
  • Missed dose: do not take the missed dose – take the next one at the usual time and continue as scheduled. Do not take a double dose.

When to seek medical help immediately?

  • trembling, muscle stiffness, involuntary body movements or difficulty walking
  • yellowing of the skin or eyes
  • swelling of the face or lips, shortness of breath, itchy rash
  • severe drowsiness, confusion or loss of consciousness (especially if several tablets were taken or swallowed by a child)
  • sudden severe dizziness with headache, weakness of an arm or leg, speech or vision disturbance

Pharmacist's advice

With vertigo it is important to get up slowly and take care not to fall, especially in older people. Because cinnarizine causes drowsiness in some people and the leaflet advises caution in low blood pressure, check how you react during the first days and measure your blood pressure if you tend to have low values.

In Raynaud’s syndrome and cold extremities the medicine is only an adjunctive treatment – keep your hands and feet warm. If you notice trembling hands, stiffness or a less steady gait, see your doctor, as these may be side effects on movement. Do not combine the medicine with alcohol or sleep aids without asking your pharmacist.

According to the ALIMS register, the registered medicine is Cinedil 75 mg tablets (Alkaloid). It is available on prescription.

Does cinnarizine help with ringing in the ears?

Ringing in the ears (tinnitus) is listed among the vestibular symptoms for which the medicine is approved, together with vertigo, nausea and vomiting. Because tinnitus has various causes, the doctor decides on its use.

Is cinnarizine also used for circulation in the legs?

Yes, as adjunctive treatment of symptoms of peripheral circulation disorders – Raynaud’s syndrome, leg pain when walking (intermittent claudication) and cold extremities. The usual dose is 1 tablet two to three times daily, as advised by a doctor.

Can I drink alcohol while taking cinnarizine?

It is not recommended. The combination of cinnarizine and alcohol increases drowsiness and reduces psychophysical abilities.
