Atacor is a medicine containing the active substance atorvastatin, a statin – a medicine that lowers blood fats. It slows cholesterol production in the liver and so lowers total and LDL (“bad”) cholesterol and triglycerides.

It is made by the Serbian company Zdravlje AD Leskovac. In Serbia it is authorised in three strengths of film-coated tablets – 10 mg and 20 mg (30 tablets each) and 40 mg (30 tablets) – and is available on prescription. It is taken alongside a cholesterol-lowering diet.

Important warning

  • Prescription-only medicine.
  • Must not be taken in active liver disease, pregnancy, breast-feeding or by women of childbearing potential not using reliable contraception.
  • Report muscle pain, tenderness or weakness (especially with fever or dark urine) to your doctor immediately – a possible sign of muscle damage.
  • Do not drink more than 1–2 small glasses of grapefruit juice a day and avoid large amounts of alcohol.
  • This page is for information only and is not an offer to buy the medicine online.

What is Atacor used for?

According to the Summary of Product Characteristics, atorvastatin is indicated:

  • as an adjunct to diet to reduce elevated total cholesterol, LDL cholesterol, apolipoprotein B and triglycerides in adults, adolescents and children aged 10 years or older with primary hypercholesterolaemia (including heterozygous familial) or mixed hyperlipidaemia, when diet and other measures are inadequate
  • in adults with homozygous familial hypercholesterolaemia, as an adjunct to other treatments (e.g. LDL apheresis) or if these are unavailable
  • for prevention of cardiovascular events in adults at high risk of a first cardiovascular event, as an adjunct to correcting other risk factors

How is it taken and dosed?

ConditionDosageNote
Primary hypercholesterolaemia and mixed hyperlipidaemiausually 10 mg once dailyresponse within 2 weeks, maximum usually within 4 weeks
Heterozygous familial hypercholesterolaemiastart 10 mg, increase every 4 weeks up to 40 mgthen up to a maximum of 80 mg, or 40 mg with a bile acid sequestrant
Homozygous familial hypercholesterolaemia10–80 mg dailytogether with other treatments (e.g. LDL apheresis)
Prevention of cardiovascular disease10 mg daily in studieshigher doses may be needed according to guidelines
Children from 10 years (heterozygous FH)start 10 mg dailyincrease at intervals of at least 4 weeks, up to 80 mg; not indicated under 10 years

Take the dose once daily, all at once, at any time of day, with or without food. Dose changes are made at intervals of at least 4 weeks; the maximum dose is 80 mg daily (achieved with a combination of strengths decided by your doctor). Dosing refers to the prescribed tablet strength – never switch strengths without medical advice. This is the general dosage from the Summary of Product Characteristics – your doctor decides on your treatment.

Special patient groups

  • Kidney impairment: no dose adjustment needed.
  • Liver impairment: caution; contraindicated in active liver disease.
  • Over 70 years: efficacy and safety similar to the general population.
  • With elbasvir/grazoprevir or letermovir the atorvastatin dose should not exceed 20 mg daily; not recommended with letermovir plus ciclosporin.

Contraindications – when the medicine must not be used

  • hypersensitivity to atorvastatin or any of the excipients
  • active liver disease or unexplained persistent rises in transaminases (more than three times the upper limit)
  • pregnancy, breast-feeding and women of childbearing potential not using adequate contraception
  • concomitant use of glecaprevir/pibrentasvir (hepatitis C medicine)

Warnings and precautions

  • Muscles: myalgia, myopathy and rarely rhabdomyolysis may occur; creatine kinase (CK) should be measured before treatment in people at risk (kidney disease, hypothyroidism, personal or family history of muscle disorders, previous statin muscle toxicity, liver disease or high alcohol intake, older age).
  • Atorvastatin must not be taken with systemic fusidic acid or within 7 days after it.
  • Liver: liver function tests before and during treatment; caution with high alcohol intake.
  • In patients with recent stroke or TIA (without coronary disease) a higher rate of haemorrhagic stroke was seen with 80 mg.
  • Blood sugar may rise; monitoring is needed in people at high risk of diabetes.
  • Rarely interstitial lung disease; statins may induce or worsen myasthenia gravis.

Interactions with other medicines, alcohol and food

  • Increased risk of muscle damage: ciclosporin, erythromycin, clarithromycin, telithromycin, ketoconazole, itraconazole, voriconazole, fluconazole, posaconazole, fusidic acid, gemfibrozil and other fibrates, some HIV and hepatitis C medicines, letermovir.
  • Heart and blood pressure medicines: amlodipine, diltiazem, verapamil, amiodarone, digoxin.
  • Others: ezetimibe, warfarin, oral contraceptives, stiripentol, cimetidine, phenazone, colchicine, aluminium- and magnesium-containing antacids, rifampicin, St John’s wort.
  • Grapefruit juice – no more than 1–2 small glasses a day.
  • Alcohol – avoid large amounts.

Driving and using machines

Atacor has negligible influence on the ability to drive and use machines.

Possible side effects

  • Common: inflamed nasal passages, sore throat, nosebleed, allergic reactions, raised blood sugar and creatine kinase, headache, nausea, constipation, wind, indigestion, diarrhoea, joint, muscle and back pain, abnormal liver tests.
  • Uncommon: loss of appetite, weight gain, low blood sugar, insomnia, nightmares, dizziness, tingling in fingers and toes, taste disturbance, memory loss, blurred vision, ringing in the ears, vomiting, pancreatitis, hepatitis, rash, hives, hair loss, muscle weakness, tiredness, swollen ankles.

Report suspected side effects to your doctor, pharmacist or ALIMS.

Overdose, missed dose and stopping treatment

  • If you have taken more tablets than you should, contact your doctor or the nearest healthcare facility immediately. There is no specific treatment; liver function and CK are monitored.
  • If you forget a dose, take the next one as scheduled; do not take a double dose.
  • If you want to stop treatment, talk to your doctor first.

When to seek medical help immediately?

  • Stop taking the medicine and contact a doctor or emergency services immediately if you notice:
  • muscle weakness, tenderness or pain, especially with feeling unwell, fever or dark red-brown urine
  • swelling of the face, tongue or throat with difficulty breathing
  • peeling and swelling of the skin, blisters on the skin, mouth, eyes or genitals with fever
  • unexpected bleeding or bruising, yellowing of the skin or eyes

Pharmacist's advice

Atacor only works while you take it and together with dietary change – choose fewer saturated fats and more fibre, vegetables and fish, and stay physically active. Take the tablet at the same time every day; a pill organiser helps, especially if you also take other heart and blood pressure medicines.

Cardiovascular risk is also reduced by controlling blood pressure, so regular home monitoring is useful. Ask a pharmacist before taking any cholesterol supplement – red yeast rice products contain monacolin K, a statin-like substance, and St John’s wort may affect how the medicine works.

How quickly does Atacor lower cholesterol?

A response occurs within 2 weeks and the maximum effect is usually reached within 4 weeks. That is why the doctor changes the dose at intervals of at least 4 weeks.

May I drink grapefruit juice?

No more than 1–2 small glasses a day; larger amounts can change the effect of the medicine.

What should I do if my muscles hurt?

Contact your doctor straight away, especially if the pain comes with weakness, fever or dark urine. The doctor may check your creatine kinase (CK) and decide on further treatment.

Can I take cholesterol supplements with Atacor?

Ask your doctor or pharmacist first. Some products (e.g. red yeast rice) contain statin-like substances, and St John’s wort is listed among products that interact with the medicine.
