Amlodipin Alkaloid is a medicine containing the active substance amlodipine (as amlodipine besilate) and belongs to the group of calcium antagonists (calcium channel blockers), dihydropyridine derivatives. It is authorised as 5 mg and 10 mg tablets and is available on prescription.
Amlodipine relaxes and widens blood vessels so that blood flows through them more easily – this lowers high blood pressure. In angina pectoris it improves the supply of blood and oxygen to the heart muscle and thereby prevents chest pain, but it does not immediately stop an angina attack that has already started.
Important warning
- Supply status in Serbia: prescription-only medicine.
- The medicine must not be used in severe hypotension, shock (including cardiogenic shock), obstruction of the left ventricular outflow tract (e.g. severe aortic stenosis) and unstable heart failure after a heart attack.
- Do not consume grapefruit or grapefruit juice during treatment – they can unpredictably increase the effect of the medicine.
- Amlodipin Alkaloid does not stop an angina attack (chest pain) that has already started – the doctor prescribes another medicine for that.
- This page is for information only and is not an offer to buy the medicine online.

What is Amlodipin Alkaloid used for?
According to the approved leaflet, Amlodipin Alkaloid is used to treat:
- high blood pressure (hypertension);
- chronic stable angina pectoris (chest pain due to reduced blood flow to the heart muscle);
- vasospastic (Prinzmetal’s, variant) angina – a rare form of angina.
In children and adolescents aged 6 to 17 years, amlodipine is used only to treat high blood pressure.
How is it used and dosed?
| Patient group | Dose according to PIL/SmPC |
|---|---|
| Adults – hypertension and angina pectoris | starting dose 5 mg once daily; can be increased to a maximum of 10 mg once daily depending on response |
| Children and adolescents 6–17 years (hypertension only) | initially 2.5 mg once daily; if target blood pressure is not reached after 4 weeks, up to 5 mg once daily (doses above 5 mg have not been studied) |
| Children under 6 years | no data available – use has not been studied |
- The tablet is taken once daily at the same time every day, before or after a meal, with enough water or another drink – but not with grapefruit juice.
- The 5 mg tablet has a score line for dividing into equal doses (2.5 mg); the score line on the 10 mg tablet is only to make swallowing easier, not to divide the dose.
- In hypertension, thiazide diuretics, beta blockers or ACE inhibitors may be used together with amlodipine without the need to adjust the amlodipine dose.
- It is important to take the prescribed treatment regularly; before the pack runs out, see your doctor to continue treatment.
Special patient groups
- Elderly patients: the usual dosage is used, but the dose is increased with caution.
- Liver impairment: elimination of amlodipine is prolonged; treatment is started at the lowest dose and increased cautiously, and in severe liver impairment gradual titration and careful monitoring are needed.
- Kidney impairment: usual doses are used; amlodipine is not removed by dialysis.
- Heart failure: caution is needed; pulmonary oedema was reported more often in severe heart failure.
- Children: only from 6 to 17 years and only for hypertension, at 2.5–5 mg daily.
Contraindications – when the medicine must not be used
Do not take Amlodipin Alkaloid:
- if you are allergic to amlodipine, other dihydropyridines (calcium antagonists) or any of the excipients;
- if you have severe hypotension (very low blood pressure);
- in shock, including cardiogenic shock;
- if you have obstruction of the left ventricular outflow tract, e.g. high-grade aortic stenosis;
- if you have haemodynamically unstable heart failure after an acute heart attack.
Precautions and warnings
- Tell your doctor if you have recently had a heart attack, have heart failure or liver disease, or are an elderly person whose dose is being increased.
- The safety and efficacy of amlodipine in hypertensive crisis (a sudden, severe rise in blood pressure) have not been established.
- The medicine contains lactose monohydrate – if you are intolerant to some sugars, consult your doctor before use.
Interactions with other medicines, food and drink
The following medicines may affect amlodipine or be affected by it:
- ketoconazole, itraconazole (antifungals); ritonavir, indinavir, nelfinavir (HIV medicines); clarithromycin, erythromycin – may increase amlodipine levels;
- rifampicin and St John’s wort (Hypericum perforatum) – may reduce the effect of amlodipine;
- verapamil, diltiazem (heart medicines); dantrolene (infusion);
- tacrolimus, sirolimus, temsirolimus, everolimus and ciclosporin (immunosuppressants) – their blood levels may need monitoring;
- simvastatin – according to the SmPC, in patients taking amlodipine the simvastatin dose is limited to 20 mg daily.
The blood-pressure-lowering effect is stronger if you already take other blood pressure medicines. Grapefruit and grapefruit juice should not be consumed during treatment.
Pregnancy and breastfeeding
The safety of amlodipine in pregnancy has not been established – if you are pregnant, think you may be pregnant or are planning a pregnancy, consult your doctor before taking the medicine.
Amlodipine passes into breast milk; if you are breastfeeding or plan to breastfeed, ask your doctor before taking the medicine.
Driving and using machines
The medicine may affect your ability to drive and use machines. If you feel sick, dizzy, tired or have a headache, do not drive or use machines and contact your doctor – especially at the start of treatment.
Possible side effects
Contact your doctor immediately if you experience: sudden wheezing, chest pain, shortness of breath or difficulty breathing; swelling of the eyelids, face, lips, tongue or throat; a severe skin reaction (widespread rash, blisters, peeling skin); heart attack or abnormal heart rhythm; severe abdominal and back pain with feeling very unwell (inflamed pancreas).

If any side effect causes discomfort or lasts longer than one week, contact your doctor.
Overdose, missed dose and stopping treatment
- Overdose: too many tablets at once can cause a dangerous drop in blood pressure (dizziness, fainting, weakness, shock with cold clammy skin and loss of consciousness); fluid may also build up in the lungs 24–48 hours later. Contact a doctor or the nearest hospital immediately.
- Missed dose: skip the missed dose and take the next one at the usual time; never take a double dose.
- Stopping treatment: the doctor decides how long to take the medicine; if you stop earlier, symptoms may return.
When to seek medical help immediately?
- sudden wheezing, difficulty breathing or shortness of breath;
- swelling of the face, lips, tongue or throat;
- severe chest pain or irregular heartbeat;
- widespread rash with blistering or peeling skin;
- severe abdominal pain spreading to the back, with feeling very unwell;
- marked dizziness, fainting or suspected overdose.
Pharmacist's advice
Amlodipine works evenly over 24 hours, so one dose a day is enough – it is easiest to link it to a fixed daily routine, such as breakfast. Measure your blood pressure regularly at home and note the values for your check-up.
Ankle swelling is the most common side effect of amlodipine and is usually not dangerous, but it should be reported to the doctor, especially if it is marked or accompanied by shortness of breath. Avoid grapefruit and grapefruit juice throughout treatment.
Before taking St John’s wort preparations or a new medicine (antibiotic, antifungal, statin), check with a pharmacist whether there is an interaction.
