What is Zyllt used for?

According to the patient leaflet, Zyllt is used in adults to prevent blood clots in arteries affected by atherosclerosis when:

  • you have previously had a heart attack or stroke, or you have peripheral arterial disease;
  • you have had unstable angina or a myocardial infarction (including patients who have had a stent placed) – it is then usually combined with acetylsalicylic acid;
  • you have had a transient ischaemic attack (TIA) or a mild ischaemic stroke – combined with acetylsalicylic acid, started within the first 24 hours;
  • you have atrial fibrillation and cannot take oral anticoagulants (vitamin K antagonists) and are not at risk of major bleeding – combined with acetylsalicylic acid.

Your doctor decides whether the medicine is suitable for you and how long you should take it.

How is it used and dosed?

The usual dose is one 75 mg tablet once a day, taken by mouth with or without food, at the same time every day.

SituationDosage according to the leaflet (unless prescribed otherwise)
Usual treatment (including atrial fibrillation)1 tablet of 75 mg once daily
Unstable angina or heart attackat the start the doctor may prescribe a single loading dose of 300 mg or 600 mg (4 or 8 tablets), then 75 mg daily
Transient ischaemic attack or mild ischaemic strokea single dose of 300 mg (4 tablets), then 75 mg daily with acetylsalicylic acid for 3 weeks; afterwards the doctor prescribes only one of the two medicines
  • The loading dose is determined and given by the doctor – do not take it on your own initiative.
  • In patients over 75 years of age, treatment of acute ST-elevation myocardial infarction is started without a loading dose (SmPC).

This is the general dosage from the leaflet – your doctor determines your dose and the duration of treatment.

Special patient groups

  • Children and adolescents: the medicine is not intended for use in children and adolescents.
  • Elderly patients: the usual dose of 75 mg is used; in patients over 75 years the 600 mg loading dose is not recommended because of a higher risk of bleeding.
  • Kidney impairment: experience is limited – caution is required.
  • Liver impairment: the medicine must not be used in severe liver disease; caution is required in moderate impairment.

Contraindications – when the medicine must not be used

  • allergy to clopidogrel or any of the excipients
  • active bleeding or a condition that may cause bleeding, e.g. a stomach ulcer or bleeding in the brain
  • severe liver disease

Warnings and precautions

  • increased risk of bleeding: stomach ulcer, a blood disorder with a tendency to internal bleeding, a recent serious injury or recent surgery (including dental surgery)
  • planned surgery or dental procedure – according to the SmPC, if an antiplatelet effect is not desired, the medicine is stopped 7 days before the procedure; this is decided by the doctor
  • ischaemic stroke within the previous 7 days, a history of non-traumatic bleeding in the brain
  • kidney or liver disease
  • previous allergic reaction to medicines, especially to other thienopyridines (ticlopidine, prasugrel) – cross-sensitivity is possible
  • thrombotic thrombocytopenic purpura (TTP) – fever, tiny red spots or bruises, unexplained extreme tiredness, confusion or jaundice require urgent medical examination
  • cuts and injuries may bleed longer than usual; minor cuts (e.g. when shaving) are usually not a cause for concern
  • the medicine contains lactose – if you have an intolerance to some sugars, contact your doctor

Interactions with other medicines, food and drink

Tell your doctor or pharmacist about all medicines you are taking, including those obtained without a prescription. The following are particularly important:

  • medicines that increase the risk of bleeding: oral anticoagulants, heparin and other injectable anticoagulants, ticlopidine and other antiplatelet medicines, non-steroidal anti-inflammatory drugs (medicines for pain and inflammation), selective serotonin reuptake inhibitors (e.g. fluoxetine, fluvoxamine)
  • rifampicin
  • omeprazole or esomeprazole
  • fluconazole, voriconazole
  • efavirenz and other antiretroviral medicines
  • carbamazepine, moclobemide
  • repaglinide, paclitaxel
  • opioids (medicines for severe pain) – tell your doctor that you take clopidogrel before they are prescribed

Occasional use of acetylsalicylic acid (no more than 1000 mg in 24 hours) should generally not cause problems, but prolonged concomitant use should be discussed with your doctor. Food and non-alcoholic drinks do not affect how the medicine works.

Pregnancy and breast-feeding

Zyllt is not recommended during pregnancy. If you are pregnant, think you may be pregnant or become pregnant during treatment, tell your doctor immediately. Breast-feeding should be stopped while you are taking this medicine.

Driving and using machines

Clopidogrel does not affect the ability to drive or use machines.

Possible side effects

The most common side effect is bleeding: in the stomach or intestines, bruises and haematomas, nosebleeds, blood in the urine; rarely bleeding in the eye, inside the head, in the lungs or joints.

  • Common: diarrhoea, abdominal pain, indigestion, heartburn.
  • Uncommon: headache, stomach ulcer, vomiting, nausea, constipation, wind, rash, itching, dizziness, tingling and numbness.
  • Rare: vertigo, breast enlargement in men.
  • Very rare: jaundice, severe abdominal pain with or without back pain, fever, breathing difficulties sometimes with cough, generalised allergic reactions, swelling in the mouth, skin blisters, stomatitis, drop in blood pressure, confusion, hallucinations, joint and muscle pain, taste disturbance.
  • Frequency not known: hypersensitivity reactions with chest or abdominal pain, persistent symptoms of low blood sugar.

Your doctor may also find changes in blood and urine test results. Report suspected side effects to your doctor, pharmacist or ALIMS.

Overdose, missed dose and stopping treatment

  • Overdose: contact your doctor or the nearest healthcare facility immediately because of the increased risk of bleeding. There is no antidote; if necessary, the doctor may give a platelet transfusion.
  • Missed dose: if you remember within 12 hours of your usual time, take the tablet straight away and continue as usual; if more than 12 hours have passed, take the next tablet at the usual time. Never take a double dose.
  • Stopping treatment: do not stop treatment without your doctor’s advice.

When to seek medical help immediately?

  • bleeding that does not stop, vomiting blood, black or bloody stools, blood in the urine
  • sudden severe headache, weakness of one side of the body, speech or vision disturbance
  • fever, signs of infection or extreme tiredness (possible drop in blood cell counts)
  • yellowing of the skin or the whites of the eyes, tiny red spots on the skin, confusion
  • swelling of the lips or face, rash, skin blisters or difficulty breathing (allergic reaction)

Pharmacist's advice

Take Zyllt once a day, always at the same time – a weekly pill organiser helps you not to skip or double a dose. Before taking any over-the-counter painkiller (e.g. ibuprofen, diclofenac), ask your pharmacist, because these medicines further increase the risk of bleeding.

Tell every dentist and doctor that you take clopidogrel. Use a soft toothbrush, and if you notice bleeding gums, unexplained bruises or dark stools, contact your doctor.

Frequently asked questions

Should Zyllt be taken with food?

It can be taken with or without food – what matters is taking the tablet at the same time every day.

What if I forget to take a tablet?

If less than 12 hours have passed since your usual time, take it straight away; if more than 12 hours have passed, take the next tablet at the usual time. Do not take a double dose.

Do I have to stop Zyllt before a tooth extraction or surgery?

Your doctor decides. Always tell your dentist and surgeon that you take clopidogrel; according to the SmPC, when an antiplatelet effect is not desired, the medicine is stopped 7 days before the procedure.

Can I take a painkiller with Zyllt?

Non-steroidal anti-inflammatory drugs increase the risk of bleeding, so take them only on your doctor’s advice. Occasional use of acetylsalicylic acid up to 1000 mg in 24 hours is usually not a problem, but agree on long-term use with your doctor.
