Propafen is an antiarrhythmic medicine (class IC) containing propafenone hydrochloride. It slows the heart rate and helps regulate the heart rhythm, so it is used to treat and prevent irregular heart rhythm (arrhythmias).
The medicine is available as film-coated tablets of 150 mg and 300 mg propafenone hydrochloride. Both strengths have a score line and can be divided into equal doses.
Important warning
- Dispensing status: the medicine is available only on prescription.
- It is advisable to start treatment in hospital, under the supervision of a doctor experienced in treating arrhythmias, with ECG monitoring and blood pressure measurement.
- Grapefruit and grapefruit juice must not be consumed during treatment, as they increase the level of the medicine in the blood.
- Do not stop treatment on your own – your condition may get worse.
- This page is for information only and is not an offer to buy the medicine online.

What is Propafen used for?
According to the summary of product characteristics, Propafen is intended:
- for the prevention and treatment of ventricular arrhythmias (rhythm disorders originating in the heart chambers);
- for the prevention and treatment of paroxysmal supraventricular tachyarrhythmias, including paroxysmal atrial flutter/fibrillation and paroxysmal re-entrant tachycardia (involving the AV node or accessory pathways), when standard therapy is ineffective or contraindicated.
How is Propafen used and dosed?
The dose is determined by the doctor with ECG and blood pressure monitoring. According to the leaflet and the summary of product characteristics:
- Adults – starting dose: 150 mg three times daily.
- Dose increase: at intervals of at least three days (the dose may only be increased after 3 to 4 days of treatment) to 300 mg twice daily and, if necessary, up to a maximum of 300 mg three times daily.
- A lower total daily dose is recommended for people weighing less than 70 kg.
- Swallow the tablet whole without chewing (bitter taste and local anaesthetic effect), after a meal, with sufficient liquid.
Special patient groups
- Children: use is not advised – a suitable dose for children is not available.
- Older people: the doctor may reduce the dose; starting treatment and increasing the dose should be gradual, and the dose should not be increased before 5 to 8 days of treatment.
- Liver and/or kidney impairment: the medicine may accumulate; the dose is adjusted with ECG monitoring and measurement of drug levels in the blood.
- Pacemaker: adjustment of the settings may be required.
Contraindications – when the medicine must not be used
- allergy to propafenone hydrochloride or the excipients;
- Brugada syndrome;
- myocardial infarction in the last 3 months;
- severe heart failure or another significant heart disease not related to the rhythm disorder;
- cardiogenic shock (unless caused by an arrhythmia);
- very slow heartbeat, markedly low blood pressure;
- electrolyte disturbances in the blood (e.g. sodium or potassium);
- severe obstructive lung disease (chronic bronchitis, emphysema);
- myasthenia gravis;
- concomitant use of ritonavir;
- without an implanted pacemaker: sinus node dysfunction, atrial conduction defects, second- or higher-degree AV block, bundle branch block or distal block.
Precautions and warnings
- risk of developing heart failure;
- obstructive lung disease or asthma;
- pregnancy, planning a pregnancy or breastfeeding;
- planned surgery or dental treatment – tell the surgeon or dentist that you take propafenone, as it may affect the action of anaesthetics;
- implanted pacemaker – adjustment may be needed;
- before and during treatment your doctor may perform an ECG and measure your blood pressure to determine the dose.
Interactions with other medicines, food and drink
Tell your doctor or pharmacist about all medicines you take, including non-prescription medicines, especially:
- amiodarone, digoxin, quinidine;
- anticoagulants such as warfarin;
- erythromycin, rifampicin, ketoconazole;
- beta blockers (metoprolol, propranolol);

- cimetidine, ciclosporin, theophylline, phenobarbital.
Grapefruit and grapefruit juice must not be consumed during treatment. The medicine is taken after a meal with sufficient liquid.
Pregnancy and breastfeeding
- Pregnancy: use is not recommended unless necessary because the pregnant woman’s health is at risk and the benefit for the mother outweighs the risk to the baby.
- Breastfeeding: the medicine should be used with caution in breastfeeding women.
Driving and using machines
- Driving, operating machinery and activities requiring increased attention are not recommended during treatment. The medicine may cause blurred vision, dizziness, tiredness and a fall in blood pressure on standing up.
Possible side effects
- Contact your doctor immediately: seizures (convulsions); life-threatening rhythm disorder; difficulty breathing and swelling around the ankles; rash, itching, redness or difficulty breathing (allergy); yellowing of the skin or eyes; bruising or sore throat with high fever (low platelet or white blood cell count).

Overdose, missed dose and stopping treatment

- Missed dose: take it as soon as possible; if it is nearly time for the next dose, skip the missed one and continue as scheduled. Never take a double dose.
- Stopping treatment: do not stop the medicine without your doctor’s advice, even if you feel better – your condition may get worse.
When to seek medical help immediately?
- seizures or loss of consciousness;
- very fast, irregular or markedly slow heartbeat, fainting;
- difficulty breathing and swelling around the ankles (signs of heart failure);
- yellowing of the skin or eyes;
- high fever with sore throat or unexplained bruising.
Pharmacist's advice
Take Propafen after meals, at regular intervals during the day, and swallow the tablet whole – because of its bitter taste and slight numbing of the mouth, it should not be chewed. Avoid grapefruit and grapefruit juice completely, and talk to your pharmacist first about any new medicine, including non-prescription ones.
Regular ECG check-ups are part of the treatment, because your doctor adjusts the dose based on them. If you feel stronger palpitations, faintness or dizziness, do not wait for the next check-up.
