No-Spa 40 mg is a medicine containing the active substance drotaverine hydrochloride. It belongs to the antispasmodics – medicines that relax smooth muscle and can therefore relieve pain caused by its cramping contractions. Drotaverine is an isoquinoline derivative that acts directly on smooth muscle (ATC A03AD02).
The medicine is used for short-term symptomatic treatment of spastic pain originating from the biliary or urinary tract – in biliary colic and stones in the gallbladder or bile ducts, and in kidney and ureteral stones. No-Spa relieves the spasm but does not remove the stone, infection, duct blockage or any other cause of the pain, so it is used only on a doctor's advice and for as short a time as possible.

What is No-Spa used for?
According to the official package leaflet, No-Spa is used for:
- short-term symptomatic treatment of smooth muscle spasm in biliary tract disorders – uncomplicated spasms (colic) without infection, stones in the gallbladder or bile ducts (cholecystolithiasis, choledocholithiasis, cholangiolithiasis);
- short-term symptomatic treatment of smooth muscle spasm originating from urinary tract disorders – kidney stones and ureteral stones.
Severe abdominal or flank pain can have many causes. No-Spa is not intended to mask unexplained, persistent or sudden pain without a diagnosis.
How is it used and dosed?

- If the pain has not clearly decreased 30 minutes after the first 40 mg dose, the dose may be repeated.
- If a second dose has no effect either, stop using the medicine and consult a doctor immediately.
- Swallow the tablet with enough water; there are no special instructions regarding food and drink.
- Do not increase the number of tablets yourself and do not keep taking the medicine for days just because the pain temporarily eases.
Special patient groups
- Children under 12 years: must not be given the medicine.
- Severe liver or kidney impairment: the medicine must not be used.
- Severe heart failure (low cardiac output syndrome): the medicine must not be used.
- People with low blood pressure: special caution is required.
Contraindications – when the medicine must not be used
Do not take No-Spa:
- if you are allergic to drotaverine or any ingredient of the medicine;
- in severe liver, kidney or heart impairment (severe renal and hepatic insufficiency, heart failure with low ejection fraction);
- in children under 12 years.
Precautions and warnings
- Hypotension: drotaverine can lower blood pressure further – take care if you have low blood pressure, dizziness on standing up or use several blood-pressure-lowering medicines.
- Each tablet contains 52 mg lactose – talk to your doctor if you have a rare hereditary sugar intolerance.
- Pain that does not settle, keeps returning or comes with fever, jaundice, vomiting or blood in the urine must be examined – infection or obstruction are possible and No-Spa does not treat them.
Interactions with other medicines, alcohol and food
Tell your doctor or pharmacist about all the medicines you take. Given together with levodopa, drotaverine reduces its effect on the symptoms of Parkinson's disease – muscle stiffness and tremor may increase. The leaflet has no special notes on taking the medicine with food, drink or alcohol.
Pregnancy and breastfeeding
Studies have not shown harmful effects on the mother or the foetus, but use in pregnancy still requires special caution and a doctor's assessment. No data are available on use during breastfeeding, so it is not recommended while breastfeeding. No data are available on effects on fertility.
Driving and using machines
At usual oral doses drotaverine does not affect the ability to drive or use machines. If you feel dizzy after taking the medicine, avoid driving and operating machinery.
Possible side effects
Side effects are rare (in 1–10 in 10,000 patients):
- headache, dizziness, insomnia;
- nausea, constipation;
- palpitations, a fall in blood pressure;
- allergic reactions – angioedema, hives, rash, itching.
If any side effect becomes serious, tell your doctor or pharmacist. You can also report suspected side effects to ALIMS (the Serbian medicines agency).
Overdose, missed dose and stopping treatment
- An overdose of drotaverine can cause heart rhythm and conduction disorders, including cardiac arrest, which may be fatal – contact a doctor or emergency service immediately and take the pack with you.
- Missed dose: do not take a double dose to make up for a forgotten one.
- Stopping treatment: the medicine is meant for short-term use; ask your doctor or pharmacist about the duration of use.
When to seek medical help immediately?
- swelling of the face, lips, tongue or throat, difficulty breathing or swallowing;
- loss of consciousness or a marked fall in blood pressure;
- very fast, slow or irregular heartbeat;
- severe pain in the upper right abdomen with fever or jaundice;
- severe flank pain with fever, vomiting or blood in the urine;
- inability to pass urine or markedly reduced urine output;
- pain that does not ease even after a second 40 mg dose.
Pharmacist's advice
No-Spa should be used briefly and only when the cause of the spasm has been assessed. Do not keep repeating doses for days just because the pain temporarily decreases.
In renal or biliary colic, watch your temperature, urine colour, urination, vomiting and any yellowing of the skin – these may point to infection or obstruction that needs urgent examination. If you take levodopa or have low blood pressure, tell your doctor before taking the medicine.
Herbal teas and food supplements for the bile or urinary system are not a substitute for examination, ultrasound, urine tests or treatment prescribed by a doctor.
