Methylergometrin HF oral drops contain methylergometrine maleate 0.25 mg/ml (1 ml = 20 drops). Methylergometrine is a semi-synthetic derivative of ergometrine, an ergot alkaloid. It increases the tone, rate and strength of rhythmic contractions of the uterine smooth muscle, which shortens the third stage of labour and reduces blood loss.

The medicine is used to prevent and treat bleeding after childbirth and abortion and to speed up shrinking of the uterus after childbirth. This page describes the oral drops (10 ml bottle); the same manufacturer also has a 0.2 mg/ml solution for injection, used in healthcare facilities, with a separate leaflet.

Important warning

  • Supply status in Serbia: the oral drops are prescription-only.
  • Must not be used in pregnancy, nor with high blood pressure, pre-eclampsia or eclampsia.
  • Do not breastfeed during treatment and for at least 12 hours after the last dose; milk expressed during this time should be discarded.
  • The most common side effect is a rise in blood pressure – severe headache, chest pain or convulsions require emergency help.
  • This page is for information only and is not an offer to buy the medicine online.

Interactions with other medicines, alcohol and food

The effect of the medicine may be increased by: macrolide antibiotics (erythromycin, troleandomycin, clarithromycin), quinupristin/dalfopristin, HIV medicines (ritonavir, indinavir, nelfinavir, saquinavir, delavirdine), antifungals (ketoconazole, itraconazole, voriconazole, fluconazole, clotrimazole), the antidepressants nefazodone, fluoxetine and fluvoxamine, cimetidine, zileuton, beta-blockers, other ergot alkaloids, triptans (migraine medicines), sympathomimetics (including in local anaesthetics), prostaglandins and grapefruit juice.

The effect may be reduced by: nevirapine, rifampicin, the anaesthetics halothane and methoxyflurane. Methylergometrine may weaken the effect of nitroglycerin and other angina medicines.

Grapefruit juice may cause a potentially dangerous interaction – talk to your doctor before taking it.

Pregnancy and breastfeeding

Count the drops precisely – 10 to 15 drops three times daily, for no longer than 7 days, as prescribed by the gynaecologist. Check your blood pressure from time to time during treatment and report severe headache, chest pain or cold, white fingers to the doctor immediately.

If you are breastfeeding, plan ahead: during treatment and for 12 hours after the last dose, express and discard the milk to maintain lactation. Do not drink grapefruit juice and tell the doctor about any antibiotics, antifungal or migraine medicines you take.
