Available Options
Hepalpan 300 IU/g is a topical medicine containing heparin sodium. It is available as a cream and as a gel, each supplied in a 40 g tube.
Both formulations are used to relieve symptoms associated with superficial bruising and haematomas and as adjunctive treatment for superficial thrombophlebitis.
The only active ingredient is heparin sodium. Dexpanthenol and allantoin are excipients within the formulations.
Apply only to intact skin. Do not apply to open wounds, infected skin, mucous membranes or weeping eczema.
For thrombosis or superficial thrombophlebitis, apply a thin layer without massage. Sudden one-sided leg swelling, marked pain, redness or breathing difficulty requires urgent medical assessment.

Approved uses
- adjunctive treatment of superficial thrombophlebitis;
- relief of symptoms associated with superficial bruises;
- relief of symptoms associated with haematomas beneath intact skin.

How to apply Hepalpan
- Apply a thin layer to the affected area two to four times daily.
- For bruises, it may be rubbed in very gently.
- For thrombosis or thrombophlebitis, apply without massage.
- Use only on intact, uninfected skin.
- Wash the hands after application unless the hands are being treated.
It is intended for adults, older people and children over five years.
The recommended treatment period is one to two weeks. Consult a doctor if there is no improvement after ten days, symptoms become worse or longer treatment appears necessary.
Do not use Hepalpan
- if allergic to heparin sodium or an excipient;
- with current or previous immune-mediated heparin-induced thrombocytopenia type II;
- on open wounds, infected skin or weeping eczema;
- on mucous membranes or close to the eyes;
- in children under five years.
Other medicines
Clinically significant interactions are not generally expected during ordinary topical use. However, prolonged use or application over larger skin areas requires caution.
Tell a doctor or pharmacist about anticoagulants, antiplatelet medicines, acetylsalicylic acid, clopidogrel, warfarin or medicines that may increase bleeding.
Pregnancy and breastfeeding
The current leaflets do not identify evidence that Hepalpan cream or gel cannot be used during pregnancy or breastfeeding. Medical or pharmaceutical advice is nevertheless recommended before use.
Possible side effects
Very rare skin reactions include redness, itching, irritation and contact dermatitis. Stop application if a skin reaction develops.
The cream contains lanolin and methyl parahydroxybenzoate E218. The gel contains propylene glycol, methyl parahydroxybenzoate E218 and propyl parahydroxybenzoate E216.
- sudden swelling of one leg;
- severe or increasing leg pain;
- redness, heat or a painful hard area along a vein;
- shortness of breath or chest pain;
- swelling of the face, lips, tongue or throat;
- unusual bleeding or rapidly enlarging bruises.

Storage
Store below 25°C in the original packaging and keep out of the sight and reach of children.
After first opening, use within 28 days.
Hepalpan cream and gel are supplied without a prescription in Serbia, but medicines cannot be ordered through the online shop. Check current availability and price directly with a pharmacy.
Do not massage an area with a painful, red or swollen vein. Sudden one-sided leg swelling, heat, increasing pain or breathing difficulty requires medical assessment rather than continued self-treatment.
Ask a pharmacist before prolonged use if you take an anticoagulant or antiplatelet medicine.
