Cardiopirin 100 mg gastro-resistant tablets (acetylsalicylic acid) – prevention of blood clots after heart attack or stroke

Cardiopirin 100 mg is a medicine in the form of gastro-resistant tablets containing acetylsalicylic acid. At a low dose it prevents blood platelets (thrombocytes) from sticking together and thereby reduces the likelihood of a blood clot forming.

The medicine is used for the secondary prevention of cardiovascular and cerebrovascular thrombotic events, i.e. in people who have already had a heart attack or a stroke, and after coronary artery bypass surgery.

Cardiopirin 100 mg – use, dosage and precautions

What is Cardiopirin used for?

According to the approved package leaflet, Cardiopirin is used:

• to prevent another heart attack in patients who have already had one;
• to prevent another ischaemic stroke in patients who have already had one;
• to prevent blood clots from forming after a heart attack or stroke;
• after bypass surgery – surgical bridging of the blood vessels of the heart.

The medicine is not intended for self-directed primary prevention in a healthy person who has not had a cardiovascular event. The balance of benefits and risks must be assessed by a doctor, particularly because of the possibility of serious bleeding.

How is Cardiopirin taken?

For regular long-term treatment, the gastro-resistant tablet should be swallowed whole with plenty of liquid, preferably with a meal. It should not be broken, crushed or chewed.

The official leaflet mentions chewing the tablet only in a special situation at the time of diagnosis, for faster absorption. Patients should not do this on their own as a first-aid measure.

If a dose is forgotten, do not take a double dose. Do not stop treatment without first consulting your doctor.

When must Cardiopirin not be used?

• allergy to acetylsalicylic acid, salicylates or any of the excipients;
• if aspirin or other non-steroidal anti-inflammatory drugs have previously caused an asthma attack or an allergic reaction;
• active stomach or duodenal ulcer;
• increased tendency to bleed;
• severe kidney or liver impairment;
• severe heart failure;
• together with methotrexate at a dose of 15 mg per week or more;
• in the last trimester of pregnancy;
• in children under 16 years, unless specifically decided by a doctor.

When is special caution needed?

Before use, tell your doctor or pharmacist if you have:

• a previous ulcer or bleeding from the stomach or intestine;
• bronchial asthma, hay fever or nasal polyps;
• impaired kidney, liver or heart function;
• G6PD enzyme deficiency;
• gout or raised uric acid;
• uncontrolled high blood pressure;
• a planned operation, tooth extraction or other medical procedure;
• treatment with other medicines that affect blood clotting.

Even minor procedures may be accompanied by prolonged bleeding. Always tell your doctor, dentist and surgeon that you are taking acetylsalicylic acid. Do not stop the medicine before a procedure without their instruction.

Interactions with other medicines

Special caution is needed with:

• anticoagulants and other antiplatelet medicines, such as warfarin, apixaban, rivaroxaban or clopidogrel;
• ibuprofen, naproxen, diclofenac and other NSAIDs;
• metamizole, which can weaken the antiplatelet effect of low-dose acetylsalicylic acid;
• methotrexate;
• corticosteroids;
• valproic acid;
• digoxin;
• diuretics and ACE inhibitors, especially in impaired kidney function;
• medicines for diabetes;
• medicines for gout, such as probenecid and benzbromarone.

Ibuprofen and naproxen can reduce the antiplatelet effect of Cardiopirin when taken at an inappropriate time interval. Choose a painkiller with the advice of a doctor or pharmacist.

Alcohol should be avoided, as it increases damage to the stomach lining and the risk of gastrointestinal bleeding.

Pregnancy and breastfeeding

During the first and second trimesters of pregnancy, Cardiopirin is used only when prescribed by a doctor, at the lowest appropriate dose and for the shortest necessary period.

The medicine must not be used in the last trimester of pregnancy. If you find out during treatment that you are pregnant, contact your doctor.

Acetylsalicylic acid passes into breast milk in small amounts. With regular, prolonged use or higher doses, the doctor may recommend stopping breastfeeding.

Possible side effects

The following may occur:

• heartburn, discomfort or pain in the stomach;
• nausea and inflammation of the lining of the digestive tract;
• stomach or duodenal ulcer;
• hidden or visible gastrointestinal bleeding;
• bruising, nosebleeds or bleeding gums and prolonged bleeding from wounds;
• anaemia due to long-term blood loss;
• rash, itching, hives or an asthmatic reaction;
• impaired kidney or liver function.

Dizziness, hearing impairment and ringing in the ears may be signs of too high a dose of acetylsalicylic acid.

How should it be stored?

Keep out of the sight and reach of children, at a temperature up to 30°C. Do not use after the expiry date stated on the pack.

Return unused or expired tablets to a pharmacy that collects pharmaceutical waste.
