Mendilex 2 mg is a prescription medicine containing biperiden hydrochloride. It is an anticholinergic antiparkinsonian medicine used to treat parkinsonism and certain movement disorders caused by other medicines.

Biperiden modifies cholinergic activity in the central nervous system and may reduce rigidity, tremor and certain extrapyramidal symptoms. The dose must be individually prescribed and adjusted by a doctor.

Mendilex – what is it used for and how should it be taken?

What is Mendilex used for?

  • different forms of parkinsonism;
  • rigidity and tremor associated with parkinsonian syndromes;
  • medicine-induced extrapyramidal symptoms;
  • pseudoparkinsonism, akinesia, rigidity and akathisia caused by neuroleptic medicines;
  • acute medicine-induced dystonia.

Mendilex is not suitable for every type of tremor or involuntary movement. Biperiden may worsen certain disorders, including tardive dyskinesia caused by neuroleptic treatment.

How should Mendilex be taken?

Dosing is individual. Treatment is generally started with a low dose and increased gradually according to the therapeutic response and adverse effects.

  • For parkinsonism in adults, the official leaflet states an initial dose of 1 mg twice daily, with gradual individual adjustment. The maximum daily dose is 16 mg.
  • For medicine-induced extrapyramidal symptoms, the doctor may prescribe 1–2 mg two or three times daily.
  • Use in children aged 3–15 years is generally limited to short-term treatment of medicine-induced extrapyramidal symptoms under strict medical supervision.

This is not a self-dosing guide. The exact dose depends on the diagnosis, age, kidney and liver function, other medicines and sensitivity to anticholinergic effects.

  • Take each dose with sufficient water.
  • Take the medicine during or immediately after food when possible.
  • The tablet can be divided into two equal doses.
  • Do not increase the dose independently.
  • Do not stop treatment suddenly.

If a dose is missed: take the next dose at the scheduled time. Do not take a double dose.

Important warning

Mendilex should not be stopped abruptly. Sudden discontinuation may worsen the underlying condition or produce withdrawal symptoms. The dose must be reduced gradually according to the doctor’s plan.

Older patients are particularly susceptible to confusion, memory impairment, hallucinations, constipation, urinary retention and falls.

When should Mendilex not be taken?

  • allergy to biperiden or another ingredient;
  • untreated angle-closure glaucoma;
  • mechanical narrowing of the gastrointestinal tract;
  • ileus;
  • megacolon.

Special warnings and precautions

Tell the doctor if you have an enlarged prostate, difficulty urinating, glaucoma, an abnormal heart rhythm, myasthenia gravis, seizures, kidney or liver disease, dementia, memory problems or a history of confusion or hallucinations.

Regular eye-pressure monitoring may be required. Biperiden can reduce sweating and increase the risk of overheating during hot weather, fever or intensive physical activity.

Interactions

Important interactions may occur with other anticholinergic medicines, antipsychotics, sedating antihistamines, other antiparkinsonian medicines, antispasmodics, levodopa, levodopa/carbidopa, quinidine, metoclopramide and pethidine.

Combining several anticholinergic medicines may increase dry mouth, constipation, urinary retention, blurred vision, drowsiness, confusion and hallucinations.

Alcohol and driving

Avoid alcohol because its effects may be increased. Mendilex may cause tiredness, dizziness, drowsiness, visual disturbance and reduced concentration. Do not drive or operate machinery until you know how it affects you.

Pregnancy and breastfeeding

During pregnancy, biperiden should be used only after an individual medical assessment. Biperiden passes into breast milk and breastfeeding is not recommended during treatment.

Possible side effects

  • dry mouth, nausea and stomach discomfort;
  • constipation;
  • tiredness, dizziness or drowsiness;
  • rapid or slow heartbeat;
  • blurred vision, dilated pupils and sensitivity to light;
  • reduced sweating;
  • difficulty urinating or urinary retention;
  • memory impairment;
  • agitation, anxiety, insomnia or confusion;
  • hallucinations, delirium or euphoria, particularly at higher doses;
  • involuntary movements, impaired coordination or speech;
  • increased susceptibility to seizures;
  • allergic skin reactions.
When should immediate medical help be sought?
  • inability to urinate with lower-abdominal pain or swelling;
  • severe eye pain, a red eye, blurred vision, coloured halos, nausea or vomiting;
  • high temperature, hot and dry skin and inability to sweat;
  • severe confusion, delirium, hallucinations or sudden behavioural changes;
  • seizures or loss of consciousness;
  • difficulty breathing or swelling of the face, lips, tongue or throat;
  • a pronounced rapid or irregular heartbeat;
  • sudden deterioration after stopping treatment.

Overdose

An overdose may cause dilated pupils, extreme dry mouth, facial flushing, rapid heartbeat, bowel and bladder paralysis, high temperature, agitation, confusion, hallucinations, impaired consciousness and serious breathing or circulatory problems.

Contact emergency medical services immediately and take the medicine pack with you.

Storage

  • Keep out of the sight and reach of children.
  • No special storage conditions are required.
  • Do not use after the expiry date.
  • Return unused medicine to a pharmacy that collects pharmaceutical waste.

Composition

Each tablet contains 2 mg of biperiden hydrochloride. The other ingredients are lactose monohydrate, microcrystalline cellulose and magnesium stearate.

Pharmacist’s advice

Take Mendilex according to the precise schedule prescribed by the neurologist or psychiatrist. Do not increase the dose because tremor or rigidity becomes worse and do not stop it suddenly.

Monitor constipation, urinary difficulty, pronounced dry mouth, visual disturbance, confusion and hallucinations. These effects may be particularly serious in older people.

Check every memory, nervous-system or circulation supplement against the complete treatment plan before use. A non-prescription product is not automatically safe with complex Parkinson’s treatment.
