5-MTHF 1 mg, 60 capsules by Thorne is a high-strength dietary supplement containing the biologically active folate form L-5-methyltetrahydrofolate. Each capsule provides 1 mg of active folate as Extrafolate-S®.
Folate is vitamin B9 and contributes to normal blood formation, normal amino-acid synthesis and normal homocysteine metabolism. It also has a role in cell division and contributes to normal psychological and immune-system function and the reduction of tiredness and fatigue.
During pregnancy, folate contributes to maternal tissue growth. Because this product provides 1 mg per capsule, it is primarily intended for adults whose doctor, gynaecologist or pharmacist has recommended targeted active-folate supplementation.

At one capsule daily, a pack of 60 capsules lasts approximately 60 days. Do not increase the dose based on directions printed on packs intended for other markets unless professionally advised.

- when increased folate needs have been medically assessed,

- when a single-ingredient product without added iron is preferred.
Having an MTHFR gene variant does not automatically mean that high-dose methylfolate is required. The form and amount should be selected according to diet, laboratory results, pregnancy needs and other supplements.
5-MTHF is a dietary supplement and not a medicine for anaemia, depression, infertility, neuropathy, elevated homocysteine or another medical condition.
One capsule provides 1,000 µg of active folate. This is higher than the general recommendation of 400 µg of folic acid daily for most women who may become pregnant. Use during preconception and pregnancy should be agreed with a gynaecologist.
Women at increased risk of a neural-tube defect may be prescribed a substantially higher folic-acid dose. This is a medical recommendation and should not be independently replaced with methylfolate.
Vitamin B12 status should be considered before prolonged high-dose folate use. Folate can improve blood-count abnormalities while neurological consequences of vitamin B12 deficiency remain unrecognised.
Consult a doctor before use with methotrexate, antiepileptic medicines, trimethoprim, sulfasalazine, chemotherapy or other medicines affecting folate metabolism.
Do not automatically combine it with prenatal vitamins, B-complex products, multivitamins or other folic-acid or methylfolate supplements without checking the total intake.
The product is not a complete prenatal supplement because it does not provide vitamin B12, iron, iodine, vitamin D, choline, DHA or other nutrients that may be required during pregnancy.
Mild gastrointestinal discomfort, headache or an individual hypersensitivity reaction may occur. Stop use in cases of a rash, swelling or another pronounced reaction.
The product is not intended for children without paediatric advice.
Check the labels of all prenatal, multivitamin and B-complex products before starting. Folate is commonly present in several supplements, so the total daily intake can easily be duplicated.
Fatigue, pallor, palpitations, tingling, balance problems or an abnormal blood count should not automatically be attributed to low folate. Folate and vitamin B12 deficiency can cause overlapping symptoms but require different management.
Women planning pregnancy should discuss folate before a positive pregnancy test. However, a 1 mg methylfolate capsule is not automatically the appropriate dose for every woman.
