Zodol 10 mg is a prescription medicine containing ketorolac trometamol. Ketorolac belongs to the group of non-steroidal anti-inflammatory drugs, or NSAIDs, and reduces pain, swelling and inflammation.

Zodol film-coated tablets are intended for short-term relief of moderate pain following surgery. Treatment is generally started with parenteral ketorolac in a healthcare institution, followed by a switch to oral tablets as soon as clinically appropriate.

Important warning
Zodol is not intended for an ordinary headache, toothache, menstrual pain, chronic back pain or long-term self-treatment. The total duration of ketorolac therapy, including injections and tablets, must not exceed five days. The medicine may cause serious gastrointestinal bleeding, kidney injury and an increased risk of postoperative bleeding.

What is Zodol used for?

Zodol is used for short-term relief of moderate pain following a surgical procedure. It may reduce pain, swelling, redness and other signs of inflammation.

Ketorolac is a potent NSAID analgesic, but it is not intended for chronic pain, long-term rheumatic conditions or repeated unsupervised treatment of pain.

How is Zodol taken?

Take Zodol exactly as prescribed. It is recommended during or immediately after a meal with sufficient water.

The usual adult dose is:

  • one 10 mg tablet every four to six hours as required;
  • a maximum oral dose of 40 mg daily;
  • the lowest effective dose for the shortest possible period.

Do not increase the dose because pain has not disappeared immediately. A higher dose may not provide better pain relief but may significantly increase the risk of bleeding, kidney injury and other adverse effects.

How long may Zodol be used?

The total duration of ketorolac treatment must not exceed five days. This includes both:

  • the period during which ketorolac injections were administered;
  • the subsequent period of oral tablet treatment.

Persistent pain after five days requires renewed medical assessment and selection of another treatment.

Do not extend treatment independently
The risks of ulceration, gastrointestinal bleeding, perforation, kidney injury and cardiovascular complications increase with the dose and duration of treatment.

Who must not take Zodol?

  • people allergic to ketorolac or another ingredient;
  • people who developed asthma, bronchospasm, hives or swelling after aspirin or another NSAID;
  • patients with a current or previous stomach or duodenal ulcer;
  • patients with current or previous gastrointestinal bleeding or perforation;
  • patients with moderate or severe kidney impairment;
  • patients with severe liver impairment or severe heart failure;
  • patients with a bleeding or clotting disorder;
  • patients taking anticoagulant or high-risk antiplatelet treatment;
  • patients with severe dehydration or reduced circulating blood volume;
  • patients taking another NSAID;
  • people younger than 16 years;
  • women during the final three months of pregnancy, labour or breastfeeding.

Zodol is not used before surgery as preventive pain relief or where there is a high risk of surgical bleeding.

Gastrointestinal bleeding

Ketorolac may cause ulcers, bleeding or perforation of the stomach and intestines. These reactions may develop without warning and may be life-threatening.

The risk is higher in older patients, people with a history of ulcer or bleeding, and patients taking corticosteroids, anticoagulants, antiplatelet medicines or certain antidepressants.

Stop taking Zodol and seek urgent medical help if you develop:
  • vomiting blood or material resembling coffee grounds;
  • black, tarry or bloody stools;
  • severe or sudden abdominal pain;
  • marked weakness, pale skin, cold sweating or fainting;
  • unexplained bruising, prolonged bleeding or blood in the urine.

Taking the medicine after food may reduce stomach discomfort but does not prevent an ulcer or gastrointestinal bleeding.

Kidney function and fluid retention

Ketorolac may reduce blood flow through the kidneys and worsen kidney function. The risk is greater in dehydrated patients, older people, heart failure, pre-existing kidney disease and people taking diuretics, ACE inhibitors or angiotensin-receptor blockers.

Contact the doctor immediately in the event of reduced urination, swollen legs, rapid weight gain, breathlessness, blood in the urine or severe pain in the kidney area.

Heart, blood pressure and stroke risk

NSAIDs may be associated with a small increase in the risk of heart attack or stroke, particularly at higher doses and with longer treatment.

Seek emergency help for:
  • chest pain, pressure or tightness;
  • sudden weakness on one side of the body;
  • difficulty speaking or sudden confusion;
  • sudden severe breathlessness;
  • loss of consciousness or a marked heart-rhythm disturbance.

Allergic and severe skin reactions

Ketorolac may cause bronchospasm, anaphylaxis, angioedema and severe skin reactions. The risk is higher in people with asthma, hay fever, chronic hives or nasal polyps.

Stop treatment immediately if swelling of the face, lips, tongue or throat, breathing difficulty, widespread hives, blisters, skin peeling or mucosal sores occur.

Interactions with other medicines

Zodol must not be combined with another NSAID, including ibuprofen, diclofenac, naproxen, aspirin used for pain relief or another ketorolac product.

Important interactions include:

  • warfarin, heparin, clopidogrel and other medicines affecting blood clotting;
  • corticosteroids;
  • selective serotonin reuptake inhibitors;
  • diuretics and blood-pressure medicines;
  • lithium;
  • methotrexate;
  • ciclosporin and tacrolimus;
  • pentoxifylline;
  • probenecid;
  • zidovudine;
  • mifepristone.

Do not take ibuprofen or diclofenac between Zodol doses. This combination may considerably increase the risk of gastrointestinal bleeding and kidney injury.

Pregnancy, breastfeeding and fertility

During the first six months of pregnancy, Zodol should be avoided unless it is absolutely necessary and specifically recommended by a doctor. The lowest dose should then be used for the shortest possible period.

Zodol must not be used during the final three months of pregnancy, labour or breastfeeding.

Use for more than several days after week 20 of pregnancy may affect fetal kidney function, reduce amniotic fluid and affect the fetal ductus arteriosus.

Ketorolac may temporarily impair female fertility and is not recommended for women attempting to conceive.

Driving and using machines

Zodol may cause dizziness, drowsiness, fainting, headache, tiredness, insomnia, visual disturbance or depression. Do not drive or operate machinery if affected.

Possible side effects

Possible adverse effects include:

  • heartburn, indigestion and abdominal pain;
  • nausea, vomiting, diarrhoea or constipation;
  • bloating and flatulence;
  • headache, dizziness or drowsiness;
  • fluid retention and swelling;
  • increased blood pressure;
  • rash, itching or increased sweating;
  • abnormal kidney or liver function;
  • bleeding or changes in blood-cell counts.

Serious effects include gastrointestinal bleeding or perforation, acute kidney injury, severe allergy, serious skin reactions, heart failure, heart attack and stroke.

Overdose and missed dose

An overdose may cause severe abdominal pain, nausea, vomiting, ulceration or bleeding, drowsiness, dizziness, kidney dysfunction, breathing difficulty or loss of consciousness.

Contact a doctor or healthcare institution immediately if too much medicine has been taken.

After a missed dose, skip the forgotten tablet and take the next dose at the scheduled time. Do not take a double dose.

Zodol is not simply a stronger form of ibuprofen for routine home use. It is ketorolac intended for a very short period of postoperative treatment. Never combine it with ibuprofen, diclofenac, naproxen or another NSAID. Taking it with food may reduce stomach discomfort, but it does not remove the risks of bleeding and kidney injury. Black stools, vomiting blood, reduced urination, swelling or sudden severe weakness require immediate medical assessment.

Is injectable Zodol also available?

Yes. Zodol is also registered as a 30 mg/mL solution for injection in a pack of five 1 mL ampoules. The injectable formulation is administered intramuscularly or intravenously only in an inpatient healthcare institution and is a separate product.

Composition, appearance and packaging

Each film-coated tablet contains 10 mg of ketorolac trometamol.

The tablet core contains microcrystalline cellulose, lactose monohydrate and magnesium stearate. The coating contains hypromellose, titanium dioxide and macrogol.

The tablets are round and white to cream white. The pack contains one PVC/aluminium blister with 10 film-coated tablets.
