Trimetacor MR 35 mg is a prescription medicine containing trimetazidine dihydrochloride. It is used in adults as add-on symptomatic treatment for stable angina when the condition is not adequately controlled by first-line antianginal treatment or when that treatment is not tolerated.
Trimetazidine supports energy metabolism in heart-muscle cells exposed to reduced oxygen supply. It is not an immediate vasodilator and does not provide emergency relief from acute chest pain.
Trimetacor MR must not be used to treat an acute angina attack, as initial treatment for unstable angina or for an acute myocardial infarction. New, stronger, longer-lasting or more frequent chest pain requires urgent medical assessment and use of the emergency treatment prescribed by the cardiologist.

What is Trimetacor MR used for?
Trimetacor MR is used in adults with stable angina pectoris in combination with other medicines prescribed by a cardiologist.
Stable angina commonly causes pain, pressure or tightness behind the breastbone during physical exertion or emotional stress, when the heart muscle temporarily receives insufficient oxygen.
Trimetacor MR is not first-line treatment and must not be started as a replacement for beta blockers, calcium-channel blockers, nitrates or other prescribed antianginal medicines.
How is Trimetacor MR taken?
The recommended adult dose is:
- one 35 mg tablet in the morning;
- one 35 mg tablet in the evening;
- take both doses during a meal with a glass of water.
Swallow the tablet whole. As it has a prolonged-release formulation, do not break, crush or chew it.
The doctor should reassess the benefit of treatment after approximately three months. Treatment should be reviewed or discontinued if there has been no satisfactory response.
Kidney impairment and older patients
For patients with moderate kidney impairment and creatinine clearance between 30 and 60 mL/min, the recommended dose is generally one 35 mg tablet in the morning with breakfast.
Patients older than 75 years may require careful dose adjustment according to kidney function.
Trimetacor MR must not be used in severe kidney impairment when creatinine clearance is below 30 mL/min.
Who must not take Trimetacor MR?
- people allergic to trimetazidine or another ingredient;
- patients with Parkinson’s disease;
- patients with symptoms of parkinsonism, including tremor, stiffness or slow movement;
- patients with restless legs syndrome;
- patients with another movement disorder;
- patients with severe kidney impairment.
The medicine is not recommended for children and adolescents younger than 18 years because its safety and efficacy have not been established.
Movement disorders and risk of falls
Trimetazidine may cause or worsen tremor, stiffness, slow movement, restless legs syndrome and an unsteady gait. This risk is especially important in older patients.
- new trembling of the hands or fingers;
- stiffness of the arms or legs;
- slow or involuntary movements;
- an unsteady gait or repeated falls;
- marked restlessness in the legs, especially while resting.
If a movement disorder develops, trimetazidine treatment should be stopped according to medical instructions. Symptoms are usually reversible after treatment is discontinued.
Low blood pressure, dizziness and impaired balance may increase the risk of falling, particularly in people taking antihypertensive medicines.
What should be done during chest pain?
Trimetacor MR does not stop an acute angina attack. Use the emergency medicine prescribed by the cardiologist and follow the individual angina action plan.
- chest pain occurs at rest;
- the pain is stronger, lasts longer or occurs more often than before;
- the prescribed emergency medicine does not provide the expected relief;
- the pain is accompanied by breathlessness, cold sweating, nausea or severe weakness;
- the pain spreads to an arm, the back, neck or jaw;
- there is loss of consciousness or a severe heart-rhythm disturbance.
Other medicines and interactions
No specific interactions with other medicines are reported in the official product information. Nevertheless, tell the doctor and pharmacist about all medicines being used, including treatment for blood pressure, angina, heart rhythm and blood clot prevention.
Trimetacor MR does not replace other medicines for stable angina. Do not independently stop nitrates, beta blockers, antiplatelet treatment, statins or another prescribed medicine.
Pregnancy and breastfeeding
As a precaution, trimetazidine should be avoided during pregnancy. Contact the doctor if pregnancy occurs during treatment.
Use during breastfeeding is not recommended because it is not known whether trimetazidine or its metabolites pass into human milk.
Driving and using machines
Trimetacor MR may cause dizziness or drowsiness. Do not drive or operate machinery if these symptoms, an unsteady gait or impaired balance occur.
Possible side effects
Common side effects may include:
- dizziness and headache;
- abdominal pain;
- diarrhoea and indigestion;
- nausea and vomiting;
- rash, itching or hives;
- weakness and tiredness.
Rare effects include palpitations, extra heartbeats, a rapid heartbeat, low blood pressure, a fall in blood pressure when standing, dizziness, falls and facial flushing.
Effects of unknown frequency include sleep disorders, drowsiness, vertigo, constipation, movement disorders, severe skin reactions, angioedema, blood-cell disorders and hepatitis.
- swelling of the face, lips, tongue or throat;
- difficulty swallowing or breathing;
- a widespread red rash with blisters or pustules;
- high temperature, recurrent infections or a severe sore throat;
- unusual bruising or bleeding;
- yellowing of the skin or eyes;
- dark urine, pale stools, loss of appetite and marked weakness;
- new tremor, stiffness or an unsteady gait.
Overdose, missed dose and stopping treatment
Information about trimetazidine overdose is limited. Contact a doctor or emergency medical service immediately if more medicine than prescribed has been taken.
Do not take a double dose to make up for a forgotten tablet. Take the next dose according to the normal schedule.
Do not stop treatment independently. Discuss lack of effect, adverse reactions or planned discontinuation with the cardiologist.
Trimetacor MR is not a tablet for every episode of chest pain. It is an add-on treatment for stable angina and has no role in the emergency relief of an acute attack. Always carry the emergency medicine prescribed by the cardiologist and do not ignore a change in the pattern of chest pain. New tremor, stiffness or an unsteady gait must not be dismissed as normal ageing while trimetazidine is being used.
