What is Progestogel used for?
According to the official leaflet, Progestogel is used for premenstrual breast pain (essential premenstrual mastodynia), provided there are no pathological changes in the breast tissue.
Premenstrual mastodynia is pain, tension and tenderness of the breasts occurring in the second half of the cycle, before menstruation. New lumps, skin changes on the breast, nipple discharge or pain unrelated to the cycle are not an indication for using the gel on your own – they must be examined by a doctor.
How is it used and dosed?
| Parameter | According to the leaflet |
|---|---|
| Period of use | from day 10 to day 25 of the menstrual cycle |
| Daily dose | 5 g gel per day – 2.5 g on each breast |
| Maximum daily dose | 5 g |
| Method of use | cutaneous, after showering, using the dosing applicator, gently rubbing in until absorbed |
| Duration | may be used for a longer period, with medical check-ups every three months |
- Squeeze the gel evenly from the tube into the groove of the dosing applicator up to the 2.5 g mark.
- Breasts and hands must be clean during application.
- Do not apply the gel to injured skin – absorption of progesterone is then increased.
- If the cycle is irregular, the doctor may recommend a combination with oral progesterone or a progestin.
Special patient groups
- Men: the medicine is not intended for use in men.
- Pregnant and breastfeeding women: use is not recommended.
Contraindications – when the medicine must not be used
Do not use Progestogel:
- if you are allergic to progesterone or any of the excipients;
- if you have breast cancer.
Precautions and warnings
- A medical examination is needed before treatment to exclude a hormone-dependent tumour (e.g. breast cancer).
- For external use only – do not swallow.
- This treatment has no contraceptive effect.
- A risk of side effects in another person after direct contact with skin where the gel was applied cannot be excluded.
- On injured skin there is a risk of increased progesterone absorption and systemic side effects – use is not recommended.
- The gel contains macrogolglycerol hydroxystearate, which may cause skin reactions; it also contains ethanol.
Interactions with other medicines, alcohol and food
The leaflet does not list specific interactions, but tell your doctor or pharmacist about all medicines you use, including hormonal products and medicines applied to the skin of the breasts.
Pregnancy and breastfeeding
Use of Progestogel during pregnancy and breastfeeding is not recommended. If you are pregnant, breastfeeding or planning a pregnancy, ask your doctor. The gel does not protect against pregnancy.
Driving and using machines
Not applicable – the medicine does not affect the ability to drive and use machines.
Possible side effects
- Most common side effect: earlier onset of menstruation (1–2 days earlier), without change in the amount of bleeding.
- Isolated cases of hypersensitivity or non-specific skin irritation.
- When applied to injured skin, the risk of systemic effects increases: menstrual changes, absence of periods (amenorrhoea), breakthrough bleeding and headache.
Because of the way it is applied, systemic exposure to progesterone is unlikely under the recommended conditions of use. Report suspected side effects to your doctor, pharmacist or ALIMS.
Overdose, missed dose and stopping treatment
- Too much gel: if you have used more than you should, tell your doctor or pharmacist.
- Missed dose: do not use a double dose; continue as prescribed by your doctor.
- Stopping: agree with your doctor on the duration of treatment and the three-monthly check-ups.
When to seek medical help immediately?
- a new lump or thickening in the breast, retraction of the nipple or skin, nipple discharge
- marked redness, swelling or blisters on the skin where the gel was applied
- swelling of the face, lips or throat, difficulty breathing (allergic reaction)
- unusual bleeding or absence of periods with suspected pregnancy
Pharmacist's advice
Apply the gel after showering to clean, dry breast skin, only from day 10 to day 25 of the cycle – mark the dates in a calendar. Rub it in gently until fully absorbed and wash your hands afterwards; avoid direct skin contact of your partner or children with the treated area until the gel has been absorbed.
Do not apply it to scratches or irritated or injured skin. Use an opened tube within 30 days. Regular breast self-examination and doctor check-ups every three months are part of the treatment – pain unrelated to the cycle or a new lump requires examination.
