Efferalgan with vitamin C is an effervescent tablet containing 330 mg paracetamol and 200 mg ascorbic acid (vitamin C) per tablet. Paracetamol is an analgesic and antipyretic – it relieves pain and reduces fever.
According to the leaflet approved by ALIMS, the medicine is used for mild to moderate pain and/or fever, e.g. headache, flu-like states and muscle pain, in adults and children weighing 27 kg or more (approximately from 8 years).
Important warning
- Supply status in Serbia: medicine available without a prescription (according to the leaflet approved by ALIMS).
- Never take more than 4 g of paracetamol a day from all sources; doses above 3 g a day only on a doctor's recommendation.
- Do not use if you have previously had kidney stones caused by high doses of vitamin C (more than 1 g).
- One tablet contains 330 mg sodium and 300 mg sorbitol. Do not use for more than 3 days without medical advice.

What is Efferalgan with vitamin C used for?
According to the patient leaflet, Efferalgan with vitamin C is used to relieve mild to moderate pain and/or fever, for example in:
- headache
- flu-like states
- muscle pain
It is intended for adults and children weighing 27 kg or more. For children under 27 kg, other forms of paracetamol exist, which should be chosen with the advice of a doctor or pharmacist.
How is it used and dosed?
The dose is based on body weight. Usual dosage according to the leaflet:

- For severe pain adults may take up to 12 tablets (4 g paracetamol) a day, but doses above 3 g a day only on a doctor's recommendation.
- Method of administration: dissolve the tablet completely in a full glass of water and drink immediately; do not swallow or chew the whole tablet.
- Because of the vitamin C content, avoid taking it at bedtime.
- Duration: do not use for more than 3 days in a row without medical advice.
Special patient groups
- Children under 27 kg: this form is not intended for them; use other forms of paracetamol.
- Adults under 50 kg, severe liver impairment, Gilbert's syndrome, chronic alcoholism, chronic malnutrition or dehydration: the lowest possible dose, no more than 60 mg/kg a day (maximum 3 g a day).
- Kidney impairment: 6 hours between doses (creatinine clearance above 10 ml/min) or 8 hours (below 10 ml/min); a total of no more than 2 g paracetamol a day.
Contraindications – when the medicine must not be used
- hypersensitivity to paracetamol, vitamin C or any of the excipients
- severe liver disease
- history of kidney stones caused by high doses of vitamin C (more than 1 g)
Precautions and warnings
- Ask your doctor before use if you are an adult weighing under 50 kg, have severe kidney or liver disease, Gilbert's syndrome, G6PD deficiency, drink 3 or more alcoholic drinks a day, are malnourished or dehydrated.
- Because of vitamin C, caution is needed in metabolic disorders and a tendency to form kidney or urinary stones.
- Check that other products you use, including non-prescription ones, do not contain paracetamol or vitamin C, so that the daily intake is not exceeded.
- In acute viral hepatitis tell your doctor – they may stop the treatment.
- In children, combining with another fever-reducing medicine is justified only if paracetamol is ineffective, and only as prescribed and supervised by a doctor.
- Sodium: 330 mg per tablet (16.5% of the maximum recommended daily intake for adults); sorbitol (E420) 300 mg and sodium benzoate (E211) 50 mg per tablet – people with hereditary fructose intolerance should consult a doctor first.
Interactions with other medicines, alcohol and food
- Oral anticoagulants: high doses of paracetamol may enhance their effect; your doctor may adjust the dose.
- Flucloxacillin: increased risk of metabolic acidosis, especially with severe kidney impairment, sepsis, malnutrition or chronic alcoholism.
- Deferoxamine and deferiprone: may increase blood iron and aluminium levels – tell your doctor.
- Ciclosporin: tell your doctor.
- Laboratory tests: paracetamol affects uric acid and blood sugar measurements; vitamin C above 2 g a day may affect creatinine and glucose results in blood and urine.
Avoid alcohol during treatment because of the increased risk of liver damage.
Pregnancy and breastfeeding
If you are pregnant, think you may be pregnant, are planning a pregnancy or are breastfeeding, ask your doctor or pharmacist before use. If necessary, the medicine is used in pregnancy only under medical supervision, at the lowest effective dose for the shortest possible time. The leaflet advises avoiding use during breastfeeding. Paracetamol may affect female fertility; the effect is reversible after stopping.
Driving and using machines
According to the leaflet, the medicine has no or negligible influence on the ability to drive and use machines.
Possible side effects
- Paracetamol – rare (up to 1 in 1,000 patients): rash, redness, hives and purpura (small red spots on the skin); stop treatment and never take paracetamol-containing medicines again.
- Paracetamol – very rare (up to 1 in 10,000 patients): severe skin reactions; low white blood cell or platelet count, which may show as nosebleeds or bleeding gums.
- Paracetamol – frequency not known: angioedema, anaphylactic shock, diarrhoea, abdominal pain, liver function disorder.
- Vitamin C – frequency not known: with doses above 1 g a day digestive complaints (heartburn, diarrhoea, abdominal pain) and urinary changes; above 3 g a day risk of haemolysis in G6PD deficiency; dizziness, hives and rash have also been reported.
Suspected side effects can be reported to your doctor, pharmacist or the Medicines and Medical Devices Agency of Serbia (ALIMS).
Overdose, missed dose and stopping treatment
- Overdose: seek help from a doctor or the nearest emergency department immediately, even if you feel well – overdose can be fatal due to severe liver damage. The main symptoms (nausea, vomiting, pallor, malaise, sweating, loss of appetite, abdominal pain) usually appear within the first 24 hours.
- Missed dose: do not take a double dose to make up for a missed one.
- Stopping treatment: the medicine is taken only while pain or fever persist.
When to seek medical help immediately?
- swelling of the face, lips or throat, difficulty breathing (signs of an anaphylactic reaction)
- rash with blisters, peeling skin or small red spots on the skin
- nosebleeds or bleeding gums without an obvious cause
- taking more than the recommended dose – even without symptoms
Pharmacist's advice
Always calculate a child's dose by body weight, not age – the leaflet table starts at 27 kg. If the child is also taking a "cold" syrup or drops, check whether they contain paracetamol.
If you already take vitamin C as a food supplement, count the 200 mg in each tablet as well. Do not take the last dose right before bedtime, as the leaflet advises.
