Amoksicilin HF is a broad-spectrum penicillin antibiotic containing the active substance amoxicillin (as amoxicillin trihydrate). The medicine was previously authorised under the name “Amoksicilin” (Hemofarm) and is now supplied as Amoksicilin HF in the form of 250 mg and 500 mg hard capsules and 250 mg/5 mL granules for oral suspension, intended mainly for young children. The medicine is available on prescription.

Amoxicillin acts against bacteria sensitive to this antibiotic and is used for infections of the airways, ear, throat, urinary tract, teeth and other bacterial infections, as well as in combination therapy to eradicate Helicobacter pylori. Antibiotics do not work against viral infections such as colds and flu.

What is Amoksicilin HF used for?

According to the approved summary of product characteristics, amoxicillin is indicated in adults and children for the treatment of the following infections:

  • acute bacterial sinusitis and acute middle ear infection (otitis media);
  • acute streptococcal tonsillitis and pharyngitis;
  • acute exacerbations of chronic bronchitis and community-acquired pneumonia;
  • acute cystitis, asymptomatic bacteriuria in pregnancy and acute pyelonephritis;
  • typhoid and paratyphoid fever;
  • dental abscess with spreading cellulitis;
  • prosthetic joint infections;
  • eradication of Helicobacter pylori (in combination with other medicines);
  • Lyme disease.

Amoxicillin is also used to prevent endocarditis (infection of the heart valves) before certain procedures. The doctor follows official guidance on the appropriate use of antibiotics.

How is it used and dosed?

The dose and duration of treatment are decided by the doctor according to the type and severity of infection, age, body weight and kidney function. The doses below are from the official leaflet and are for information only.

Patient groupUsual dose according to the PIL
Adults, elderly and children ≥ 40 kg (capsules)250–500 mg three times daily or 750 mg–1 g every 12 hours; severe infections 750 mg–1 g three times daily; maximum 6 g daily
Acute cystitis (adults)3 g twice daily for one day
Community-acquired pneumonia; prosthetic joint infections500 mg–1 g every 8 hours
Typhoid and paratyphoid fever500 mg–2 g every 8 hours
Lyme disease (adults)early stage: 500 mg–1 g every 8 hours (up to 4 g daily) for 14 days (10–21); late stage: 500 mg–2 g every 8 hours (up to 6 g daily) for 10–30 days
Helicobacter pylori (adults)750 mg–1 g twice daily for 7 days, with another antibiotic and a proton pump inhibitor
Prevention of endocarditis (adults)usually 2 g as a single dose 30–60 minutes before the procedure
Children < 40 kg (capsules or suspension)usually 40–90 mg/kg daily in 2–3 doses (suspension: 20–90 mg/kg daily), maximum 100 mg/kg daily
Children – Lyme disease (suspension)early stage 25–50 mg/kg daily in 3 doses for 10–21 days; late stage 100 mg/kg daily in 3 doses for 10–30 days
Children – prevention of endocarditis (suspension)50 mg/kg as a single dose 30–60 minutes before the procedure
  • Capsules are swallowed whole (do not open) with enough fluid; they are not suitable for people who cannot swallow them (risk of choking). For children under 6 years the suspension is recommended.
  • Preparing the suspension: fill the bottle with purified water up to the mark and shake well; shake the bottle vigorously before each use and measure with the enclosed spoon.
  • At least 4 hours should pass between two doses.

Special patient groups

  • Children: the dose is calculated by body weight (usually 40–90 mg/kg daily in two or three doses, maximum 100 mg/kg daily); from 40 kg adult doses apply. For children under 6 years the suspension form is recommended.
  • Elderly patients: dosed as adults; with reduced kidney function the doctor adjusts the dose.
  • Kidney impairment: the dose is reduced depending on the degree of impairment.
  • Liver impairment: caution and regular monitoring of liver function are needed.

Contraindications – when the medicine must not be used

Do not take Amoksicilin HF:

  • if you are allergic to amoxicillin, other penicillins or any of the excipients;
  • if you have ever had an allergic reaction (rash, swelling of the face or throat) to another beta-lactam antibiotic, e.g. a cephalosporin, carbapenem or monobactam.

Precautions and warnings

  • Tell your doctor if you have infectious mononucleosis (fever, sore throat, swollen glands, extreme tiredness) – amoxicillin may then cause a rash; if you have seizures, a tendency to allergies, impaired kidney function or do not pass urine regularly.
  • If at the start of treatment you develop redness over the whole skin with small pus-filled spots and fever (AGEP), the medicine is stopped immediately and must never be used again.
  • When treating Lyme disease a Jarisch-Herxheimer reaction may occur (fever, chills, headache, muscle pain, rash) – it is usually transient.
  • With longer treatment a fungal infection (candidiasis) may develop, and the doctor may check blood count, liver and kidney function. With high doses drink enough fluids (risk of crystals in the urine).
  • Amoxicillin may affect urine glucose tests, liver function tests and oestriol tests in pregnancy – tell the laboratory that you are taking an antibiotic.
  • The 500 mg capsule contains azo dyes (E110, E122), which may cause allergic reactions. The suspension contains sucrose (caution in sugar intolerance), about 10.7 mg sodium per 5 mL and 25 mg sodium benzoate per 5 mL (may increase jaundice in newborns up to 4 weeks old).

Interactions with other medicines, food and drink

Tell your doctor about all medicines you take. According to the leaflet, the following are important:

  • allopurinol (for gout) – greater likelihood of an allergic skin reaction;
  • probenecid (for gout) – slows the excretion of amoxicillin, the doctor may adjust the dose;
  • anticoagulants (warfarin, acenocoumarol) – additional blood tests may be needed;
  • other antibiotics such as tetracyclines – amoxicillin may be less effective;
  • methotrexate (for cancer and severe psoriasis) – side effects may increase.

Pregnancy and breastfeeding

Amoxicillin can be used in pregnancy when the potential benefit outweighs the risk; limited data do not indicate an increased risk of birth defects.

Amoxicillin passes into breast milk in small amounts, so the infant may develop diarrhoea, fungal infection or sensitisation; during breastfeeding the medicine is used only after the doctor’s assessment.

Driving and using machines

Possible side effects such as allergic reactions, dizziness and convulsions may affect your ability to drive and use machines. Do not drive if you do not feel well.

Possible side effects

Stop taking the medicine and contact your doctor immediately (very rare): severe allergic reaction (swelling of the face, lips, tongue, difficulty breathing); vasculitis with rash and joint pain; delayed allergic reaction 7–12 days after starting (rash, fever, swollen lymph glands); erythema multiforme and other severe skin reactions; DRESS; SDRIFE (red rash on buttocks, groin, armpits); signs of blood disorders (fever, sore throat, tendency to bleed); Jarisch-Herxheimer reaction; colitis (diarrhoea, sometimes with blood); severe liver damage (jaundice, dark urine); chest pain as a sign of an allergic reaction (Kounis syndrome); drug-induced enterocolitis syndrome (repeated vomiting 1–4 hours after a dose).

FrequencyOther side effects (from the PIL)
Common (up to 1 in 10)skin rash, nausea, diarrhoea
Uncommon (up to 1 in 100)vomiting; mild itchy rash or hive-like swellings
Very rare (up to 1 in 10,000)candidiasis (vagina, mouth, skin folds), kidney problems, convulsions (with high doses or reduced kidney function), dizziness, aseptic meningitis, linear IgA disease, hyperactivity, crystals in the urine, tongue discolouration, anaemia, low white blood cell count, prolonged blood clotting

These reactions may occur during treatment or up to several weeks after it has ended.

Overdose, missed dose and stopping treatment

  • Overdose: nausea, vomiting, diarrhoea or crystals in the urine (cloudy urine, problems passing urine) may occur. Contact your doctor as soon as possible and take the pack with you.
  • Missed dose: take the dose as soon as you remember and then continue as scheduled, making sure at least 4 hours pass between two doses (for the suspension: if the next dose is almost due, skip the missed one). Do not take a double dose.
  • Stopping treatment: keep taking the medicine for as long as prescribed – if some bacteria survive, the infection may return. If you still feel unwell after finishing the course, see your doctor again.

When to seek medical help immediately?

  • difficulty breathing, wheezing, swelling of the face, lips, tongue or throat;
  • widespread rash with blistering, peeling skin or pus-filled spots with fever;
  • severe or persistent diarrhoea, especially with blood or mucus, with abdominal pain and fever;
  • yellowing of the skin or eyes, dark urine and pale stools;
  • chest pain during an allergic reaction;
  • repeated vomiting 1–4 hours after a dose (especially in children).

Pharmacist's advice

Take amoxicillin at evenly spaced intervals during the day (at least 4 hours between two doses) and finish the whole prescribed course – stopping too early is a common reason for the infection returning and for bacterial resistance.

Mild diarrhoea is common with antibiotics; many patients use probiotics with Saccharomyces boulardii or lactic acid bacteria, taken at a different time from the antibiotic. If diarrhoea is severe, persistent or bloody, contact your doctor.

Keep the prepared suspension in the refrigerator and use it within 14 days; measure doses only with the enclosed spoon, and return any leftover to the pharmacy after the course.

Frequently asked questions

Are “Amoksicilin” and “Amoksicilin HF” the same medicine?

Yes – Hemofarm’s amoxicillin was previously authorised under the name “Amoksicilin” and is now called Amoksicilin HF, with the same capsule strengths (250 mg and 500 mg) and a 250 mg/5 mL suspension.

How is the suspension dose calculated for a child?

The doctor sets the dose according to the child’s body weight in kilograms and the type of infection – usually 20–90 mg/kg daily divided into two or three doses, maximum 100 mg/kg daily. Use only the enclosed 2.5 mL / 5 mL spoon for measuring.

How long does the prepared suspension last?

The prepared suspension is stored in the refrigerator at 2–8 °C and can be used for 14 days. Shake the bottle vigorously before each use.

Does amoxicillin help with a cold or flu?

No. Antibiotics only work on bacterial infections, while colds and flu are usually viral. Take amoxicillin only when prescribed by a doctor.
