  • severe liver or kidney impairment
  • diabetes mellitus
  • phaeochromocytoma, hyperthyroidism, angle-closure glaucoma, enlarged prostate

  • chronic or persistent cough (e.g. in asthma) or cough with excessive phlegm
  • use of MAO inhibitors or within two weeks after stopping them
  • concomitant use of SSRI antidepressants, beta-blockers or other cold medicines and decongestants

Precautions and warnings

  • Talk to a doctor or pharmacist if you have mildly to moderately raised blood pressure, heart disease, liver or kidney disease, epilepsy, if you are over 65 or if you know you are a slow metaboliser via the CYP2D6 enzyme.
  • Dextromethorphan combined with certain antidepressants and antipsychotics may cause serotonin syndrome: agitation, hallucinations, fever above 38 °C, rapid heartbeat, unstable blood pressure, muscle stiffness, poor coordination and digestive symptoms.
  • Abuse of dextromethorphan has been reported; special caution is needed in adolescents, young adults and people with a history of substance abuse. Treatment should be short.
  • Pseudoephedrine may cause ischaemic colitis and ischaemic optic neuropathy – stop treatment immediately in case of sudden abdominal pain, rectal bleeding or sudden loss of vision.
  • If widespread redness with fever and small pus-filled spots (AGEP) appears, usually in the first two days, stop treatment.
  • Do not use other paracetamol-containing medicines or other cough medicines; avoid alcohol and coffee.

Interactions with other medicines, alcohol and food

  • Contraindicated with MAO inhibitors (and for two weeks after them), SSRI antidepressants, beta-blockers and other sympathomimetic decongestants.
  • Strong CYP2D6 inhibitors (e.g. fluoxetine, paroxetine, quinidine, terbinafine) may markedly increase dextromethorphan levels.
  • Warfarin and other coumarin anticoagulants: prolonged use of paracetamol may increase the risk of bleeding.
  • Metoclopramide and domperidone speed up, and cholestyramine slows down, the absorption of paracetamol; enzyme inducers (antiepileptics, oral contraceptives, rifampicin) may increase paracetamol toxicity.
  • Pseudoephedrine with tricyclic antidepressants, other sympathomimetics, cardiac glycosides, ergot alkaloids, oxytocin and blood pressure medicines may increase the risk of blood pressure rises and rhythm disorders.
  • Alcohol, antihistamines, psychoactive medicines and other CNS depressants increase drowsiness; alcohol with the medicine can damage the liver – do not drink alcohol during treatment.
  • Avoid coffee, tea and caffeine products (nervousness, rapid heartbeat). The medicine can be taken with or without food.

Pregnancy and breastfeeding

The medicine must not be used during pregnancy and breastfeeding.

Driving and using machines

The medicine may cause drowsiness and dizziness, especially at the start of treatment, and these effects are increased by alcohol and sedatives. Do not drive or use machines until you know how the medicine affects you.

Possible side effects

The leaflet lists side effects by body system, mostly without frequencies:

  • heart and blood vessels: palpitations, rapid heartbeat, rhythm disorders, raised blood pressure
  • nervous system and mental health: headache, tremor, restlessness, agitation, irritability, dizziness, insomnia, drowsiness, confusion, hallucinations (especially in children), rarely convulsions
  • digestive system: nausea, vomiting, diarrhoea; ischaemic colitis (frequency not known)
  • urinary system: difficulty passing urine, urinary retention
  • blood: low platelet count
  • breathing: bronchospasm in people sensitive to acetylsalicylic acid or NSAIDs, respiratory depression
  • skin: allergic reactions, very rarely severe skin reactions (Stevens-Johnson syndrome), AGEP
  • eyes: ischaemic optic neuropathy (frequency not known)
  • liver: liver damage, especially after overdose

Suspected side effects can be reported to a doctor, pharmacist or the Medicines and Medical Devices Agency of Serbia (ALIMS).

Overdose, missed dose and stopping treatment

  • Overdose: contact a doctor or hospital immediately, even if you feel well – liver damage from paracetamol can appear later. Possible symptoms include nausea, vomiting, dizziness, slurred speech, involuntary eye movements, drowsiness, confusion, agitation, hallucinations, palpitations, high blood pressure, pallor and abdominal pain.
  • Missed dose: the medicine is taken as needed; never take a double dose to make up for a missed one.
  • Stopping treatment: the medicine is used while symptoms last, for no more than 5 days without medical advice.

When to seek medical help immediately?

  • difficulty breathing, swelling of the lips, tongue, throat or face, marked rash
  • sudden abdominal pain or rectal bleeding
  • sudden loss of vision or gaps in the visual field
  • widespread skin redness with fever and small pus-filled spots, blisters or peeling skin
  • agitation, hallucinations, high fever, rapid heartbeat and muscle stiffness (possible serotonin syndrome)
  • taking more of the medicine than recommended, even without symptoms

Pharmacist’s advice

Before taking it, read the ingredients of all medicines you are already using: paracetamol is found in many pain, fever and cold medicines, and taking it twice can seriously damage the liver.

During treatment avoid coffee, tea and energy drinks – together with pseudoephedrine they can cause nervousness and palpitations. If you notice insomnia, do not take the last dose just before bedtime.

If you have high blood pressure, diabetes, glaucoma or prostate problems, or take antidepressants, be sure to consult a pharmacist before use.

Frequently asked questions

Can I take paracetamol or another fever medicine together with Caffetin Cold?

Not without the advice of a doctor or pharmacist. Caffetin Cold already contains 500 mg paracetamol per tablet, so additional paracetamol-containing medicines increase the risk of overdose and liver damage.

May I drink coffee while using Caffetin Cold?

The leaflet advises avoiding coffee, tea and caffeine products during treatment, because together with pseudoephedrine they can cause nervousness and rapid heartbeat.

Can people with high blood pressure use Caffetin Cold?

In severe hypertension and coronary artery disease the medicine must not be used. People with mildly to moderately raised blood pressure should consult a doctor or pharmacist before use.

Is Caffetin Cold suitable for a productive (chesty) cough?

No. Dextromethorphan is intended for a dry, irritating cough; the medicine must not be used for chronic or persistent cough or for cough with excessive phlegm.
